Retrospective three-dimensional analysis of bone resorption volumes of free microvascular scapular and fibular grafts.

Defects in head and neck regions are standardly treated with microvascular grafts, such as free scapular (SFF) and fibular flaps (FFF), which are subject to a certain amount of bone resorption over time. The aim of this study was the 3-dimensional evaluation of bone resorption volume in both grafts. Over a period of 10 years, computed tomograms (CT) of patients with mandibular reconstructions with SFFs and FFFs were examined. The respective grafts were segmented as well as 3-dimensionally measured. Furthermore, factors such as gender, age, nicotine abuse, previous disease with type 2 diabetes,

Management of an Imminent Pathological Fracture of a Fibular Neomandible Via a Minimally Invasive Approach-A Case Report.

Fibular free flap reconstruction remains the workhorse of postmandibulectomy reconstruction. Dental implantation to support a dental prosthesis is a sought-after outcome when the area of resection involves tooth-bearing zones. Chronic perisoft tissue pedicle hyperplasia with secondary infection leading to gradual bone loss is a simple complication to manage in the general population, but it becomes a serious issue in the fibula mandibular reconstruction patient in that it can lead to pathological fracture of the fibula. A case of a patient with a near fracture of his fibula mandibular reconstr

Primary Leiomyosarcoma of the Mandible: Posttreatment Malocclusion Treated with an Overpartial Mandibular Resection Prosthesis.

Leiomyosarcoma is a rare malignant condition occurring in the maxillofacial region in which the mandible may be affected. Management of this tumor in the mandible may include segmental mandibulectomy, osteocutaneous fibula free-flap reconstruction, endosseous implants, and postoperative radiotherapy. Posttreatment malocclusion can be managed with an overpartial mandibular resection prosthesis. The purpose of this report is to describe the expeditious oral rehabilitation of an oncologic patient with posttreatment malocclusion, demonstrating the utility of an overpartial mandibular resection pro

Kissing dentigerous cysts involving mandibular canines: report of unusual case with review of literature.

Dentigerous cysts are odontogenic developmental cysts, which mostly surround the crown of unerupted teeth, odontomas or supernumerary teeth. These cysts are mostly solitary in occurrence and their bilateral presentation is rare, especially in the absence of syndromes such as Maroteaux-Lamy, basal cell nevus or cleidocranial dysplasia. We present an unusual case of a 24-year-old female with nonsyndromic bilateral dentigerous cysts associated with unerupted mandibular canines that were diagnosed histopathologically as dentigerous cysts.

Osteosarcoma of the mandible mimicking an odontogenic abscess: a case report and review of the literature.

UNLABELLED: Inflammatory lesions, like periapical/odontogenic abscesses, are by far the most common pathologic condition of the jaws. Radiographically, these lesions commonly manifest as widening of periodontal ligament space, discontinuity of lamina dura and ill-defined periapical radiolucency. There are some rare disorders which could cause similar radiographic changes in the jaw bone. With careful scrutiny of periapical radiolucency, regular periodic follow-up radiographs and histo-pathologic examination, the periapical abscess or infection can be differentiated from rare fatal disorders.

The clinical and radiographic characteristics of condylar osteochondroma.

Osteochondroma is one of the most common benign bone tumors, but it is rare in the mandibular condyle. The purpose of this study was to increase the clinical and radiographic cognition of osteochondroma in the mandibular condyle. Thirty-four patients with radiographic and pathologic features of unilateral condylar osteochondroma were included in this retrospective study. All cases received clinical and radiographic examinations before tumor resection. Common manifestations included facial asymmetry, hypomobility, malocclusion, joint dysfunctions, and even external auditory canal stenosis. Oste

Electrophysiologic evidence for extremely late sensory collateral reinnervation in humans.

We present a patient in whom unexpectedly late sensory recovery occurred over 5 years after removal of a 3-cm piece of the right inferior alveolar nerve (IAN) in tumor surgery of the mandible. For a year after surgery, the distribution of the mental nerve, the terminal branch of the IAN, was totally anesthetic. Thereafter, a gradual subjective sensory recovery occurred centripetally from the surrounding skin distributions. Five years after surgery, findings in electrophysiologic tests were consistent with a total lesion of the right IAN. Two years later, electrophysiologic tests gave, for the

Masticatory Function and Associated Factors in Patients With Prosthetic Rehabilitation After Mandibulectomy for Oral Cancer: A Retrospective Cross-Sectional Study.

BACKGROUND: Although masticatory function (MF) in patients with prosthetic rehabilitation after mandibulectomy for oral cancer has been reported to be influenced by multiple factors, comprehensive evaluations of MF from clinical perspectives remain insufficient. PURPOSE: To comprehensively evaluate MF and its associated factors in patients with prosthetic rehabilitation after mandibulectomy for oral cancer. METHODS: The potential study participants were patients who had undergone mandibulectomy for oral cancer and received mandibular prosthetic rehabilitation. Age, sex, the number of remaini

Interactive pipeline for mandible reconstruction surgery planning using fibula free flap.

PURPOSE: Mandible reconstruction surgery using fibula free flap is a long and expensive process requiring extensive surgical experience. Indeed, the planning stage, mandible shaping, and therefore osteotomy positioning on the fibula are tedious, often done by hand, and can take months. This delay is unacceptable when mandible deterioration is caused by a time-sensitive disease such as cancer. In this paper, we propose an interactive pipeline for an easy-to-use and time-efficient surgical planning tool tailored to be used directly by the surgeon. METHODS: From CT scans of patient's mandible an

Mandibular Reconstruction Utilizing the Reamer-Irrigator-Aspirator to Obtain Nonvascularized Femur Grafts.

Between July 2021 and February 2024, 6 patients with biopsy-confirmed benign pathologic lesions had mandibular defects reconstructed using a reamer-irrigator-aspirator at the University of Cincinnati Medical Center. Patients' ages ranged from 34 to 73, 5 of which were males and one female. Primary end points were bony continuity of the mandible, recovery time, and ability of the graft to receive implants. The reamer-irrigator-aspirator obtained 40 to 100 cm3 of uncompressed bone from donor sites. All patients ambulated on the day of surgery or postoperative day one. No neurosensory disturbance

Screw-type mouth gag for prevention and treatment of postoperative jaw limitation by fibrous tissue.

Treatment of a severe tethering of the mandible by surgery and subsequent exercise of the jaw with a newly devised mouth gag, consisting of two blocks of acrylic resin with a screw expander between them, is described. The mandible was gradually forced to open and was retained in the opened position by rotating the screw against the resistance of scar contraction without much pain. Possible development of trismus after excision of a tumor in the lower retromolar area because of surgical injury to the medial pterygoid muscle and postoperative scarring was avoided by means of the appliance.

Simple bone cyst. Report of two cases.

Two cases of simple bone cyst are presented. The first is unusual because of its radiologic appearance simulating ameloblastoma and its prolonged healing time, requiring surgical intervention on several occasions. The second is presented because of the occurrence of bilateral cysts, of which there are only eight other reported cases. It is postulated that the first case might represent a single large simple bone cyst that had several chambers with tenuous connections or that it might be an example of multiple simple bone cysts in close proximity.

Mandibular ameloblastoma and maxillary adenoid cystic carcinoma: case report.

We report the case of a 74-year-old woman who developed a follicular ameloblastoma of the right mandible and 22 months later developed a cribriform adenoid cystic carcinoma of the soft palate on the right maxilla. The ameloblastoma was treated by hemimandibulectomy, and the adenoid cystic carcinoma was managed by resection of the soft palate and the surrounding tissue and bone followed by a 6-week course of radiotherapy. Our review of the literature indicates that only one similar case has been previously reported where an odontogenic tumor and a salivary gland tumor involved two different ana

[Chin neuropathy of neoplastic origin].

Three patients with mental neuropathy ("numb chin syndrome") secondary to systemic cancer are described. Two patients had malignant lymphoma and one a probable hypernephroma. In 2 patients, facial numbness was a presenting symptom of the malignancy. Investigations disclosed metastatic lesions of the mandible in one patient and cerebrospinal fluid was normal in 2 patients. Two patients died at 1 and 8 months respectively after onset of the neuropathy. Although benign in appearance, numb chin syndrome is most often, and sometimes initially, associated with visceral neoplasia or malignant hemopat
