BACKGROUND: High maternal mortality and morbidity persist, in large part due to inadequate access to timely and quality health care. Attitudes and behaviours of maternal health care providers (MHCPs) influence health care seeking and quality of care.
METHODS: Five electronic databases were searched for studies from January 1990 to December 2014. Included studies report on types or impacts of MHCP attitudes and behaviours towards their clients, or the factors influencing these attitudes and behaviours. Attitudes and behaviours mentioned in relation to HIV infection, and studies of health provi…

Maternal mortality remains exceptionally high in Papua New Guinea (PNG) at 733 per 100,000 live births. There has been little, if any, improvement in maternal mortality or maternity services since the 1980s. In 1992-1993 a survey of 550 women in rural coastal areas of Madang Province was undertaken to investigate the prevalence of maternal risk factors and parous women's utilization of and attitudes towards the existing health services. Women were classified as at risk on the basis of previous obstetric complications, parity, stillbirths and neonatal deaths. On this basis 67% of women were cla

Given that diet, physical activity, and social support are associated with depression, we examined whether a health promotion intervention designed to modify these factors in low-income, postpartum women would reduce depressive symptoms. This study used a randomized, controlled design to examine the effect of the Just for You (JFY) Program, an educational intervention promoting healthy lifestyles through home visits by nutrition paraprofessionals and motivational telephone counseling, on postpartum depressive symptoms. A total of 679 women income-eligible for the Special Supplemental Nutrition

BACKGROUND: Gastroschisis is increasing in incidence worldwide. There is a need for a disease-specific, population-based approach to determining factors linked with gastroschisis and its outcome.
OBJECTIVES: To examine racial, socioeconomic, health and geographic predictors of gastroschisis and its outcome in Canada.
METHODS: 535 cases of gastroschisis from the Canadian Pediatric Surgery Network national database were included from May 2005 to May 2010. Baseline characteristics of mothers were compared with those reported by Statistics Canada. Factors associated with adverse neonatal outcome

Epidemiological studies associated with breastfeeding have provided conflicting results about whether it is preventive or a risk factor for atopic eczema in children. The current prospective study investigated the relationship between breastfeeding and the risk of atopic eczema in Japan. A birth cohort of 763 infants was followed. The first survey during pregnancy and the second survey between 2 and 9 months postpartum collected information on potential confounding factors and atopic eczema status. Data on breastfeeding and symptoms of atopic eczema were obtained from questionnaires in the thi

Health care practitioners and researchers commonly call for greater reliance on evidence as a means to achieve improvement in quality of care. Systematic reviews provide a means to accelerate the use of evidence-based clinical interventions and public health practices. The extent to which these time- and resource-intensive systematic reviews currently address critical maternal health priorities in the intrapartum period is unclear. This analysis summarises key maternal health and research priorities, maps these priorities to existing reviews, identifies gaps in the literature that can be addre…

The continuum of care has become a rallying call to reduce the yearly toll of half a million maternal deaths, 4 million neonatal deaths, and 6 million child deaths. The continuum for maternal, newborn, and child health usually refers to continuity of individual care. Continuity of care is necessary throughout the lifecycle (adolescence, pregnancy, childbirth, the postnatal period, and childhood) and also between places of caregiving (including households and communities, outpatient and outreach services, and clinical-care settings). We define a population-level or public-health framework based

OBJECTIVE: To evaluate the effectiveness of a community-based intervention project aimed at reducing maternal and infant mortality in a poor urban district in the city of Natal, in the Northeast of Brazil.
METHODS: The intervention, called the ProNatal project, introduced a program of integrated community health care to a geographically defined population. The interventions included the establishment of antenatal clinics at the district's health centers, the opening of the maternity facilities at the polyclinic for low-risk deliveries, the introduction of a family planning clinic and a breast

In a health facility-based study to determine the knowledge of mothers regarding recognition of pneumonia in their pre-school children, 400 women were interviewed using a pre-tested structured questionnaire. Sixty-one per cent of them would recognise pneumonia by difficult breathing, 42% by fast breathing and 26.5% by severe cough. Few of the mothers mentioned signs suggestive of 'chest indrawing' (8.5%) and 'central cyanosis' (1%). The maternal knowledge score on pneumonia signs increased significantly with educational status and social class (p < 0.05). While a substantial number of mothers

Women who had experienced an infant or fetal death responded to an open-ended question on the 1988 National Maternal and Infant Health Survey about their perinatal experience. A qualitative analysis of the 413 responses identified six major themes, including need for further information, problems with the mourning process, and unresolved questions about the cause of the death. Implications of the findings for health care practice are discussed.

This study develops an integrated model to identify significant sociodemographic and digital determinants of maternal health service utilization among married women in Nigeria. A cross-sectional analysis of data from the 2018 Nigeria Demographic and Health Survey (NDHS) was conducted on 28,888 married women aged 15-49. All analyses were conducted using the Complex Samples module in IBM SPSS Statistics, applying design-corrected variance estimation appropriate for the stratified cluster design of the NDHS. The design-weighted prevalence of six or more antenatal care visits was 25%, and the prev…

BACKGROUND: There are substantial disparities in American maternal health, with certain groups experiencing disproportionately high rates of pregnancy-related complications and adverse outcomes. Social and Environmental Determinants of Health (SEDH) may play a role in influencing maternal health in pre-pregnancy and gestational scenarios, but remain poorly understood.
OBJECTIVES: We aimed to investigate the association between SEDH with pre-pregnancy and gestational conditions related to maternal cardiometabolic health throughout the US using a machine learning approach.
METHODS: We conducte…

Maternal nutritional status before and during pregnancy is an important modifiable factor that may influence foetal development and long-term offspring health. Both undernutrition and excessive maternal body weight have been associated with adverse metabolic, cardiovascular, neurodevelopmental and immune-related outcomes in offspring. This narrative review summarises selected experimental, clinical and epidemiological evidence on maternal nutritional status, intrauterine programming and offspring health. Relevant literature was identified by searching the PubMed, Scopus and Web of Science data

In the context of the over-representation of women who are Indigenous in prisons and historical and structural barriers to health, we undertook a scoping review to understand research regarding the reproductive and maternal health experiences of Indigenous women in prison in Australia, Canada, New Zealand, and the USA. We searched online databases in May 2022 since inception, as well as Google and governmental websites between December 2022 and March 2023. We identified 8 eligible articles. Most studies (5) were conducted in Australia, while 2 were conducted in Canada and 1 in the USA. There w…

IMPORTANCE: Mothers and children in low-income households are more likely to experience worse mental and physical health than those from higher-income households.
OBJECTIVE: To determine the effect of 4 years of monthly unconditional cash transfers on the mental health of mothers with low-income and the physical health of mothers and children.
DESIGN, SETTING, AND PARTICIPANTS: This was a parallel-group, randomized clinical trial conducted from May 2018 to July 2023. Mother-infant dyads were recruited (May 2018-June 2019) from postpartum wards in 12 hospitals in 4 cities: Omaha, Nebraska; Mi

More than 80% of US maternal deaths are preventable yet maternity care in the United States (US) is becoming increasingly difficult to access. Recent years have seen the rise of maternity care deserts, defined as an area with no hospitals or birth centers offering obstetric care and without any obstetric providers. The number of counties without a birthing facility continues to grow, and 1/3 of US counties lack an obstetric clinician. The US has a maternal mortality rate that is 2-3 times greater than similar high-income countries, a steady rise in severe maternal morbidity, and markedly high
