We report the case of a 39-year old male patient who presented with anaphylactoid shock and diffuse bleeding with prolonged activated partial thromboplastin time at the emergency room. The diagnosis of aggressive mastocytosis was suspected and then confirmed by raised tryptase level and mastocytic infiltration of the bone marrow. The outcome was favorable with supportive measures, antihistamine drugs, and imatinib mesylate.

An 18-year-old patient with catatonic schizophrenia developed hypertension when administration of antipsychotic medication was discontinued. Elevated blood pressure was sustained for three days until it was discovered that intramuscular administration of benztropine mesylate immediately reduced it to normal. The fact that the hypertension was not noted earlier while the patient was receiving the antipsychotic drugs suggests that it was a withdrawal phenomenon.

CONTEXT: Duodenal gastrointestinal stromal tumors (GISTs) are uncommon and constitute a relatively small subset of GISTs which presents a unique dilemma having various surgical options. Radiological imaging may show enlargement of the pituitary gland. In these cases a primary diagnosis of hypothyroidism may not be considered. Tardive dyskinesia appeared in a 17-year-old boy following withdrawal of pimozide (Orap) and thioridazine (Melleril). The choreodyskinetic movements, which were limited to the limbs and the trunk, cleared with anticholinergic drugs but were dramatically worsened by dopaminergic receptor blockers. Therefore, therapy with an initial high dose of benztropine mesylate (Cogentin) associated with bromazepam (Lexotan) was instituted and was well tolerated.

BACKGROUND: Gastrointestinal stromal tumors (GISTs) harbor gain-of-function mutations of the c-kit tyrosine kinase receptor. Imatinib mesylate is an inhibitor of c-kit and is indicated in the treatment of chronic myeloid leukemia and GISTs. Reported adverse effects of imatinib include hypopigmentation, depigmentation, and hyperpigmentation. Although the exact mechanism by which these occur is unclear, it is likely that inhibition of c-kit leads to downstream inhibition of the tyrosinase gene promoter and thus to inhibition of pigment production.

Imatinib mesylate, a BCR/ABL fusion protein inhibitor, is the first-line treatment against chronic myelogenous leukemia. In spite of its advantageous viewpoints, imatinib still has genuine impediments like undesirable side effects and tumor resistance during chemotherapy.
