Cold weather operations: Preventive strategies in a military context.

Military cold weather operations (CWOs) introduce a range of challenges, including extreme temperatures, strong winds, difficult terrain, and exposure to snow, ice, and water. Personnel undertaking these missions face a heightened risk of cold weather injury (CWI), such as hypothermia, freezing cold injuries, and non-freezing cold injuries. The risk of these injuries is influenced by various factors, including age, sex, and body composition. To ensure optimal and safe performance in CWOs, it is crucial to implement effective preventive measures against CWI. This article emphasizes the most per

Translation, Cross-Cultural Adaptation and Preliminary Validation of a French Version of the Trauma-Related Nightmare Survey (TRNS-FR) in a PTSD Veteran Population.

INTRODUCTION: In the military population, trauma-related nightmares (TRNs) are highly associated with deployments and combat-related events. Trauma-related nightmares are also correlated with severity, treatment resistance, and chronicity of Post-Traumatic Stress Disorder (PTSD). However, to date, no specific measure of TRNs has been validated for use in the French language. This study aimed to translate and culturally adapt the English version of the Trauma-Related Nightmare Survey into French and to evaluate the psychometric properties of the translation on veterans. Correct Diabetes Diagnosis and Treatment Allows Sailor to Remain on Active Duty.

Diabetes is a growing epidemic worldwide and among our active duty population, posing a significant threat to maintaining military health and operational readiness. Latent autoimmune diabetes in adults (LADA) is a growing clinical phenotype of diabetes, with overlap between traditional type 1 diabetes mellitus and type 2 diabetes mellitus. In this case, a 27-year-old active duty male presented with polydipsia, polyuria, polyphagia, and recent weight loss. He was diagnosed with LADA, placed on a period of limited duty status and started on insulin. Eight months after diagnosis, he was transitio

[Impact of the obstructive sleep hypopnea-apnea syndrom treated on employment for military employment].

INTRODUCTION: The diurnal hypersomnia associated with obstructive sleep apnea (OSA) affects soldier employment and deployability. In this study, we examined the impact of OSA on the employment of soldiers on continuous positive airways pressure (CPAP) therapy by measuring several items: work absenteeism using the Work Productivity and Activity Impairment (WPAI) questionnaire, diurnal hypersomnia, and career advancement. METHODS: A questionnaire was sent by regular mail to 940 soldiers with OSA on CPAP therapy who are insured by the French Military Healthcare Fund (Caisse Nationale Militaire d

The effect of sensorineural hearing loss and tinnitus on speech recognition over air and bone conduction military communications headsets.

Military personnel are at risk for hearing loss due to noise exposure during deployment (USACHPPM, 2008). Despite mandated use of hearing protection, hearing loss and tinnitus are prevalent due to reluctance to use hearing protection. Bone conduction headsets can offer good speech intelligibility for normal hearing (NH) listeners while allowing the ears to remain open in quiet environments and the use of hearing protection when needed. Those who suffer from tinnitus, the experience of perceiving a sound not produced by an external source, often show degraded speech recognition; however, it is

Gender differences in posttraumatic stress symptoms among OEF/OIF veterans: an item response theory analysis.

Establishing whether men and women tend to express different symptoms of posttraumatic stress in reaction to trauma is important for both etiological research and the design of assessment instruments. Use of item response theory (IRT) can reveal how symptom reporting varies by gender and help determine if estimates of symptom severity for men and women are equally reliable. We analyzed responses to the PTSD Checklist (PCL) from 2,341 U.S. military veterans (51% female) who completed deployments in support of operations in Afghanistan and Iraq (Operation Enduring Freedom/Operation Iraqi Freedom

Symptoms of post-concussional syndrome are non-specifically related to mild traumatic brain injury in UK Armed Forces personnel on return from deployment in Iraq: an analysis of self-reported data.

BACKGROUND: Mild traumatic brain injury (mTBI) is being claimed as the 'signature' injury of the Iraq war, and is believed to be the cause of long-term symptomatic ill health (post-concussional syndrome; PCS) in an unknown proportion of military personnel. METHOD: We analysed cross-sectional data from a large, randomly selected cohort of UK military personnel deployed to Iraq (n=5869). Two markers of PCS were generated: 'PCS symptoms' (indicating the presence of mTBI-related symptoms: none, 1-2, 3+) and 'PCS symptom severity' (indicating the presence of mTBI-related symptoms at either a moder

Combat and warfare in the early paleolithic and medically unexplained musculo-facial pain in 21st century war veterans and active-duty military personnel.

In a series of recent articles, we suggest that family dentists, military dentists and psychiatrists with expertise in posttraumatic stress disorder (especially in the Veterans Health Administration) are likely to see an increased number of patients with symptomatic jaw-clenching and early stages of tooth-grinding (Bracha et al., 2005). Returning warfighters and other returnees from military deployment may be especially at risk for high rates of clenching-induced masticatory muscle disorders at early stages of incisor grinding. The literature we have recently reviewed strongly supports the con…

Operation Joint Endeavor in Bosnia: telemedicine systems and case reports.

OBJECTIVE: For the last several years the U.S. Department of Defense (DoD) has operated a telemedicine test bed at the U.S. Army Medical Research and Material Command's Medical Advanced Technology Management Office. The goal of this test bed is to reengineer the military health service system from the most forward deployed forces to tertiary care teaching medical centers within the United States by exploiting emerging telemedicine technologies. METHODS: The test bed has conducted numerous proof-of-concept telemedicine demonstrations as part of military exercises and in support of real-world t

The Political Determination of Gaza's Health System Destruction and Reconstruction and the Limitations of International Medical Deployments.

Gaza's health system has been devastated by a confluence of political determinants that long predated the 2023-25 Israeli military assault and were dramatically intensified during it. Using historical, political economy, ethical, and health systems lenses, this article argues that settler colonialism, military occupation, and a protracted blockade created chronic shortages, workforce erosion, and institutional fragility, leaving services acutely vulnerable to targeted destruction of facilities and personnel. We examine the role and limitations of international medical deployments and field hos

Trauma-related nightmares among U.S. veterans: Findings from a nationally representative study.

STUDY OBJECTIVES: Trauma-exposed veterans may be at an increased risk for nightmares. To date, however, no known study has examined the prevalence, risk factors, and comorbidities of nightmares in a nationally representative sample of veterans. METHODS: A nationally representative sample of 4069 US military veterans completed a survey that assessed trauma-related nightmares, health histories, and functioning. Multivariable analyses examined the association between trauma-related nightmares, and sociodemographic, military, and trauma characteristics, and psychiatric and functioning measures.

Traumatic Brain Injuries: Unreported and Untreated in an Army Population.

INTRODUCTION: In 2008, it was reported that 19.5% of service members previously deployed experienced a mild traumatic brain injury (mTBI). Fifty-seven percent of those did not seek medical care. It was suggested that concerns with seeking care involved confidentiality and career issues. Objective: This study addressed mTBI history, medical treatment history, and stigmas associated with mTBI/concussion. MATERIALS AND METHODS: An anonymous questionnaire was developed. Data collection occurred throughout March 2018 in conjunction with Brain Injury Awareness Month activities. RESULTS: All 5,174

Prevalence of, risk factors for, and consequences of posttraumatic stress disorder and other mental health problems in military populations deployed to Iraq and Afghanistan.

This review summarizes the epidemiology of posttraumatic stress disorder (PTSD) and related mental health problems among persons who served in the armed forces during the Iraq and Afghanistan conflicts, as reflected in the literature published between 2009 and 2014. One-hundred and sixteen research studies are reviewed, most of which are among non-treatment-seeking US service members or treatment-seeking US veterans. Evidence is provided for demographic, military, and deployment-related risk factors for PTSD, though most derive from cross-sectional studies and few control for combat exposure,

Has Current French Training for Military Orthopedic Surgeons Deployed in External Operations Been Appropriately Adapted?

INTRODUCTION: The current evolution of surgical practices is increasingly trending toward hyper-specialization. For military surgeons, their practice in France does not differ from their civilian counterparts. In contrast, in external operations, they have to deal with specific war injuries in austere conditions. They are also required to take care of local populations. Therefore, specific training is necessary, and the French Military Health Service Academy (Ecole du Val-de-Grâce) Paris has set up a specific training called Advanced Course for Deployment Surgery (ACDS) in 2007. The aim of thi

Stellate ganglion block used to treat symptoms associated with combat-related post-traumatic stress disorder: a case series of 166 patients.

OBJECTIVE: Report the successful use of stellate ganglion blocks (SGBs) in 166 active duty service members with multiple combat deployments experiencing anxiety symptoms associated with post-traumatic stress disorder (PTSD). BACKGROUND: Successful treatment of PTSD symptoms with SGB has been reported previously. This is the largest published case series evaluating SGB with a minimum of 3 months follow-up. METHODS: Following clinical interview including administration of the PTSD Checklist (PCL), 166 service members with symptoms of PTSD elected to receive a SGB. All patients received a SGB o

A Command-Integrated Mass-Casualty Mental Health Intervention.

INTRODUCTION: Mass-casualty incidents occurring in rear-area military settings can disrupt assumptions of safety and predictability, resulting in acute psychological distress and impaired functioning among exposed personnel. Early military mental health interventions aim not only to reduce distress but also to preserve operational continuity and functional capacity. This report describes a command-integrated mental health response implemented following a fatal unmanned aerial vehicle (UAV) strike on a military training base during wartime and outlines the intervention framework, implementation

Confronting the Mental Health Challenges of Large-Scale Combat Operations.

The Department of Defense predicts that future conflicts will involve large-scale combat operations (LSCOs) characterized by high-intensity warfare between technologically advanced nations. These conflicts may result in prolonged exposure to violence, increasing the risk of severe mental health consequences for military personnel, veterans, and civilians. Research on past wars and ongoing conflicts, such as the war in Ukraine, demonstrates the heightened risks of post-traumatic stress disorder, depression, moral injury, and compassion fatigue in LSCOs. Unlike previous conflicts in Iraq and Afg

The association between Gulf War Illness and suicidal thoughts and behaviors among Gulf War Era Veterans.

The rate of death by suicide is elevated among Veterans of all eras. Chronic symptoms of pain, depression, and sleep disturbances are also common among Veterans, and these symptoms are associated with suicidal thoughts and behaviors. About 25% of the 697,000 Gulf War Era Veterans deployed to the Persian Gulf theater in 1990-1991 remain afflicted with chronic, unexplained symptoms known as Gulf War Illness (GWI). This study used data from a national sample of Gulf War Veterans (N = 1142) who completed a survey of demographic, military, and health information. Multivariable logistic regression m

Health assessment of U.S. military personnel deployed to Bosnia-Herzegovina for operation joint endeavor.

In anticipation of U.S. forces deploying to Bosnia-Herzegovina, plans were established to conduct medical surveillance of all military service members. This surveillance would provide the Department of Defense with an overview of the hospitalization and outpatient morbidity experience of U.S. forces. Standardized collection of medical data from all U.S. camps using 14 diagnostic categories based on International Classification of Diseases, 9th Revision, codes began in March 1996. Special assessments for hantavirus and tick-borne encephalitis (TBE) infection risk were also conducted. The averag

Epidemiology of Vascular Injury Within the U.S. Department of Defense Trauma Registry From 2007-2023.

Background: Since WWII, the incidence of vascular trauma in U.S. military casualties has steadily risen, with the highest rates having been reported from the recent conflicts in Iraq and Afghanistan. Within civilian populations, the incidence of vascular trauma is lower, but with disproportionately higher rates of mortality, morbidity, and resource use for these injuries. Analyses of trauma databases have informed technological advances that best help our U.S. Military Health System (MHS) manage vascular injuries. We sought to perform a retrospective cohort analysis in military casualties with
