Systemic Inflammation and Molar Incisor Hypomineralisation: A Cohort Study.

Molar incisor hypomineralisation (MIH) is a common developmental dental defect of unknown etiology and considerable morbidity. Observational and experimental studies suggest systemic inflammation as a key mechanism, but there are no prospective human data. This study aimed to describe how the prevalence and severity of MIH vary with early childhood inflammation. It also aimed to investigate the extent to which early childhood inflammation affects the risk of MIH. Data were from the Barwon Infant Study, a population-based prebirth cohort. Glycoprotein acetyls (GlycA), measured by nuclear magnet…

Expert consensus on the management of third molar health.

The third molar is the most developmentally delayed of the permanent teeth and has the highest incidence of pericoronitis and odontogenic space infections. Impacted third molars significantly increase the risk of periodontitis, dental caries, and external root resorption of adjacent second molars. Third molars are associated with complex surgical procedures, and treatment decisions and clinical management of third molars in this context remain controversial. This expert consensus was generated by oral surgeons and related specialists, who synthesized the current evidence-based literature and c…

Key concepts for clinical management of Molar Incisor Hypomineralization.

Clinical management of patients with molar incisor hypomineralization (MIH) requires a comprehensive and multidimensional assessment that integrates clinical, behavioral, radiographic, functional, and psychosocial parameters. This article discusses key aspects of MIH management, including diagnosis and severity grading; non-invasive strategies aimed at preventing enamel breakdown; restorative management of severely affected molars, including the role of timed extractions; esthetic treatment to mask opacities in anterior teeth; and desensitizing approaches for the management of hypersensitivity…

Bayesian network for predicting mandibular third molar extraction difficulty.

BACKGROUND: This study aimed to establish a model for predicting the difficulty of mandibular third molar extraction based on a Bayesian network to meet following requirements: (1) analyse the interaction of the primary risk factors; (2) output quantitative difficulty-evaluation results based on the patient's personal situation; and (3) identify key surgical points and propose surgical protocols to decrease complications. METHODS: Relevant articles were searched to identify risk factors. Clinical knowledge and experience were used to analyse the risk factors to establish the Bayesian network.…

Plasmonic Single-Molecule Affinity Detection at 10-20 Molar.

DNA can be readily amplified through replication, enabling the detection of a single-target copy. A comparable performance for proteins in immunoassays has yet to be fully assessed. Surface-plasmon-resonance (SPR) serves as a probe capable of performing assays at concentrations typically around 10⁻⁹ molar. In this study, plasmonic single-molecule assays for both proteins and DNA are demonstrated, achieving limits-of-detections (LODs) as low as 10⁻2⁰ molar (1 ± 1 molecule in 0.1 mL), even in human serum, in 1 h. This represents an improvement in typical SPR LODs by eleven orders-of-magnitude. T…

Retro molar papilla pedunculated flap: A retrospective case series.

BACKGROUND: The primary purpose of this paper is to introduce a new flap design to improve keratinized tissue (KT) around implants in the posterior mandible, avoiding the need for harvesting tissue from the palate, which is associated with significant discomfort for patients. METHODS: Eleven patients requiring implants in the molar regions with a KT width (KTW) < 2 mm at the buccal aspect were treated using this surgical technique. Outcomes, including the increase in KT width and postoperative discomfort, were measured at intervals of 1, 4, and 10 months. RESULTS: An average increase of 4.2 …

Why orthodontists need to know about molar incisor hypomineralization.

Molar incisor hypomineralization (MIH) is a systemic developmental qualitative defect of the enamel that affects 1-4 first permanent molars with or without incisor involvement. Enamel hypomineralization is identified visually as a demarcated opacity with a clear border with varying extensions and can be white, yellow, or brown. This opacity is characterized by a reduced quality of enamel that is of normal thickness but not fully mineralized. It is very common for the affected teeth to present with posteruptive breakdown, making them susceptible to caries, leading to their subsequent loss. Thus…

Effect of Molar Distalization on Condyle-Glenoid Fossa Relationship.

OBJECTIVE: It is essential to be aware of the potential effects of orthodontic treatment on tissues and anatomical structures associated with the masticatory system, especially the temporomandibular joint (TMJ). Little information is available about the consequences of molar distalization on the TMJ. Therefore, this study is aimed at investigating the changes of the condyle-fossa relationship after molar distalization using the distal jet appliance. MATERIALS AND METHODS: The sample consisted of twenty-five patients (mean age 20.4 ± 2.6) who underwent molar distalization by the distal jet app…

Familial recurrent molar pregnancy: positive for KHDC3L gene mutation.

Recurrent hydatidiform moles are defined by the occurrence of two or more molar pregnancies in the same patient. These can be sporadic or familial where familial recurrent hydatidiform mole is rare and inherited as an autosomal recessive condition. Here, we present a case of four consecutive complete molar pregnancies with similar history in the sisters, who was diagnosed with fourth complete molar pregnancy. She underwent suction and evacuation followed by weekly serum β-hCG. On genetic analysis, she was found to be homozygous for KHDC3L gene mutation. She was advised for evaluation of her si…

Therapeutic Strategies of Primary Molar Infraocclusion: A Systematic Review.

BACKGROUND: Infraocclusion of deciduous molars is a clinical disturbance that occurs during primary and mixed dentition and has some orthodontic implications. Infraoccluded teeth are believed to be potential sites of malocclusion, with a risk of tipping neighbouring teeth and losing space. This systematic review aims to analyse the management of primary molars infraocclusion and to provide updated guidelines. METHODS: A literature search was performed using PubMed, Scopus, and Web of Science databases from 1 January 2017 to 28 November 2022. The inclusion criteria were: studies only on human …

[Tips for private practice on molar-incisor hypomineralisation therapy].

The treatment of severe molar-incisor hypomineralisation (MIH) is often a challenge for both the patient and the practitioner. Factors such as hypersensitivity, pulpitis, partially erupted molars, and reduced adhesive bond strength make dental work more difficult and reduce long-term success. It is particularly important for everyday practice that there is a wide range of temporary restoration options even for teeth that are difficult to restore. The present paper deals with the practical recommendations for the therapy of MIH. Therapy recommendations from the European Academy of Pediatric Den…

Endodontic management of maxillary first molar with unusual anatomy.

This article reports nonsurgical endodontic management of bilateral maxillary first molars with an unusual morphology of a single root and a Sert and Bayirli type-IX canal configuration. A 41-year-old female reported with dull continuous pain in the maxillary left and right quadrants. On clinical examination, the maxillary first molars on both sides were carious with pulpal exposure and radiographic evaluation revealed an unusual anatomy of single root and single canal for both of them. Based on clinical and radiographic examination, tooth numbers 16 and 26 were diagnosed with symptomatic irre…

Early root migration after a mandibular third molar coronectomy.

PURPOSE: This prospective cohort study aimed to assess early root migration after a coronectomy of the mandibular third molar at 2 and 6 months after surgery. METHODS: We included all patients treated with a coronectomy of an impacted mandibular third molar. The primary outcome measure was the extent of postoperative root migration after 2 and 6 months. Migration was measured as the distance between the root complex and a fixed point on the inferior alveolar canal. The secondary aim was to identify factors (age, impaction pattern, and patient sex) that affected the extent of root migration. …

Molar occlusion and jaw roll in early crown mammals.

Triconodontidae are considered the first carnivorous crown mammals. A virtual reconstruction of the masticatory cycle in the Late Jurassic Priacodon showed that triconodontid dental function is characterized by precise cutting on elongated crests. The combination of traits linked to both carnivorous diets (e.g. fore-aft cutting edges) and insectivorous diets (transverse crests and lobes) suggests a varied faunivorous diet appropriate to the small body size of most triconodontids. Total length of molar shear decreased with wear, suggesting a dietary shift during ontogeny. Embrasure occlusion is…

Saliva proteomic patterns in patients with molar incisor hypomineralization.

Molar incisor hypomineralization (MIH) is an endemic pediatric disease with an unclear pathogenesis. Considering that saliva controls enamel remineralization and that MIH is associated with higher saliva flow rate, we hypothesized that the protein composition of saliva is linked to disease. To test this, we enrolled 5 children aged 6-14 years with MIH showing at least one hypersensitive molar and 5 caries-free children without hypomineralization. Saliva samples were subjected to proteomic analysis followed by protein classification in to biological pathways. Among 618 salivary proteins identif…
