A 57-year-old man was admitted to our hospital due to repeated chest pain. Coronary spastic angina was diagnosed by emergent coronary angiography. His chest attack was not suppressed with vasodilator therapy; however, it finally improved after administration of 20 mg prednisolone. His symptoms were controlled and elevation of the eosinophil count was normalized, even after tapering the dosage. His episodes of asthma, hypereosinophilia, mononeuropathy, and pulmonary infiltrate led to a diagnosis of eosinophilic granulomatosis with polyangiitis. <Learning objective: Coronary spastic angina (CSA)

Neurosarcoidosis when encountered by neurologists most commonly presents as cranial neuropathy, peripheral mononeuropathy, polyneuropathy, myopathy, meningitis or myelopathy. There are limited reports in the current literature on the cases of neurosarcoidosis patients presenting with ischaemic stroke. We discuss a 52-year-old patient with a known previous history of cutaneous sarcoidosis presenting with an acute third nerve palsy, facial weakness and ataxia. His magnetic resonance imaging (MRI) brain demonstrated focal signal changes in the midbrain consistent with an acute ischaemic event in t

UNLABELLED: Radial mononeuropathy most commonly manifesting as wrist drop is generally secondary to penetrating trauma to the radial nerve or compression injuries. It may also involve sensory changes depending on the location of the lesion. However, it has never been described as a sign of an inflammatory process, in particular an autoimmune disease. We describe the case of a 55-year-old man who was admitted for wrist drop with bilateral paraesthesia of the upper extremities. Based on his medical history, we diagnosed Behçet's disease and subsequently neuro-Behçet's disease. He is receiving tr

INTRODUCTION: Anomalous anastomoses between the nerves of the hand are not as rare as thought to be. Amongst these, the Riche-Cannieu anastomosis (RCA) is a connection between the ulnar nerve and median motor nerve in the palm. Presence of this can sometimes be misdiagnosed as a severe median mononeuropathy at the wrist/carpal tunnel syndrome (CTS).We describe two cases that were referred for evaluation of CTS and were found to have incidental RCA.
RESULTS: Electrophysiological studies showed typical findings consistent with RCA and coexistent mild CTS. Patients in whom median sensory and mot

Bilateral facial palsy, which is usually combined with other diseases, occurs infrequently. It may imply a life-threatening condition. Therefore, the differential diagnosis of bilateral facial palsy is important. However, the etiology is variable, which makes diagnosis challenging. We report a rare case of progressive bilateral facial palsy as a manifestation of granulomatosis with polyangiitis (GPA). A 40-year-old male with otitis media and right facial palsy was referred for electroneurography (ENoG), which showed a 7.7% ENoG. Left facial palsy occurred after 2 weeks, and multiple cavitary o

We report a 53-year-old male with Cogan's syndrome. He was admitted to our hospital because of a fever of 2-weeks duration, blurred vision for 10 days, hypoacusis, and numbness of the left hand for 3 days. In addition to uveitis, hypoacusis, and aseptic meningitis, multiple mononeuropathy was diagnosed based on a nerve conduction study. Furthermore, positron emission tomography/computed tomography (PET/CT) revealed diffuse aortitis. Accordingly, the patient was diagnosed with Cogan's syndrome. After starting steroid-pulse therapy followed by 1 mg oral prednisolone/kg/day, the uveitis and hypoa

Carpal tunnel syndrome, resulting from median nerve compression at the wrist, is a common and often disabling mononeuropathy. Risk factors include female sex, family history, repetitive hand use, obesity, pregnancy and a variety of medical comorbidities including diabetes mellitus, rheumatoid arthritis, and other connective tissue diseases. In many cases, an accurate diagnosis can be reached on the basis of clinical history and supportive examination findings alone. Neurophysiological investigations are essential for confirming the diagnosis, assessing severity and excluding more generalised n

Periarteritis nodosa is a systemic vasculitis involving arteries with a diameter greater than 70-150 microns. Large vaccination against hepatitis B virus explains the decreased frequency of its association with hepatitis B virus observed in less than 10% of cases. Weight loss, fever, livedo, myalgias or arthralgias, mononeuropathy, gastrointestinal tract involvement, renal insufficiency, and cardiac failure are the most frequent clinical manifestations. Inflammatory syndrome and increased white blood cell count are common. Renal or digestive microaneurysms are present in more than 60% of cases

Although neurologic pathology is a well-documented sequela of major electric burns, there are few reports of complications arising from less extensive electrically induced trauma. Two cases of patients with significant disease subsequent to minor electric burns are presented. In one patient with a 2% superficial partial-thickness burn of the right forearm, nerve conduction studies, electromyography, and somatosensory evoked potential studies were consistent with right median mononeuropathy at the wrist and a distal slowing of the right ulnar nerve. The second patient presented with persistent

Motor and sensory nerve conduction velocities (NCV) were recorded in several nerves of a total of 99 patients with juvenile rheumatoid arthritis (JRA), and the results were compared to those obtained for two reference groups, one consisting of healthy children and the other of 30 children with different articular manifestations. The JRA and the affected reference group were not found to differ significantly from each other. NCV values were systematically slightly higher and the distal median nerve latencies slightly shorter in the patients with JRA than in the healthy reference group. Several

Type 1 Gaucher disease is currently categorized as non-neuronopathic, although recent studies suggest peripheral neurological manifestations. We report prevalence and incidence data for peripheral neuropathy and associated conditions from a multinational, prospective, longitudinal, observational cohort study in patients with type 1 Gaucher disease, either untreated or receiving enzyme replacement therapy. The primary outcome parameters were the prevalence and incidence of polyneuropathy, evaluated by standardized assessments of neurological symptoms and signs, and electrophysiological studies.

BACKGROUND: Dengue fever usually presents as a self-limiting acute febrile illness with worsening thrombocytopenia, with a small minority of patients developing hemorrhagic or life-threatening complications. Organ specific manifestations like myocarditis, acalculous cholecystitis, encephalitis has been described but are uncommon presentations. Even more rarely, such manifestations are the presenting complaint of Dengue fever. In this case report, we highlight a case of Dengue fever where unrelated neuropathies were the presenting complaint.

Mononeuritis multiplex is characterized by an asymmetric pattern with affection of the peripheral nervous system; this form of polyneuropathy is often seen in non-systemic vasculitis. We present a case of multiplex neuropathy in a patient with histologicaly verified Hailey-Hailey disease. With the exception of this comorbidity--in its characteristic form presenting additionally with a superinfected subdermal node--we did not find any other possible etiologic factor possibly causative of multiplex neuritis. The diagnosis was confirmed by electrophysiological testing. To our knowledge, this is t

Safety in the operating room environment should be a priority for every surgeon. Two areas where safety can often be overlooked and taken for granted are patient positioning and electrosurgical use. Understanding neuroanatomical relationships can help the surgeon optimally position patients and minimize the incidence of postoperative neuropathy while maintaining functional and unobstructed access to the patient and surgical field. Given the widespread use of radiofrequency devices in daily surgical practice, a working knowledge of the principles of electrosurgery is vital to the gynecologic su

It has been previously shown that unmyelinated afferent fibres in human skin are differentiated not only by their receptor characteristics, but also by their profiles of activity-dependent slowing. One type of profile, described originally as 'type 3', is different from that of nociceptors (type 1), cold afferents (type 2) and sympathetic efferents (type 4), in that these fibres display a minimal activity-dependent slowing (∼1% at 2 Hz). However, their function remains to be determined. Here we describe one unit with a typical 'type 3' activity-dependent slowing profile recorded from an undama
