Beyond the Classic Near Triad: Diagnostic Insights from a Case of Accommodative Spasm.

BACKGROUND: Accommodative spasm (AS) is an involuntary condition characterized by excessive contraction of the ciliary muscle, leading to exaggerated accommodative response and fluctuating refractive error. The reported prevalence of accommodative spasm ranges from 2.6% in an Indian clinic cohort to 37.19% in Chinese school children. Since the COVID-19 pandemic, accommodative spasm has been reported in 20% of symptomatic patients associated with increased near work and digital screen exposure. It often presents with headache, photophobia, blurred distance vision, and eye strain. This case repo

Efficacy comparison of atropine, orthokeratology and repeated low-level red-light therapy for myopia control in children: a systematic review and network meta-analysis.

OBJECTIVES: This study aimed to compare the efficacy of different interventions for myopia control in children, including 0.01% atropine (AP), orthokeratology (Ortho-k), and repeated low-level red-light therapy (RLRL), and their combinations by conducting a network meta-analysis. METHODS: We searched for randomised controlled trials (RCTs) in PubMed, Web of Science and Embase. The primary outcomes were the mean changes in the cycloplegic spherical equivalent (SE) and axial length (AL) at the 12-month follow-up. A Bayesian random-effects network meta-analysis was performed to estimate pooled w

A case of unilateral progression of myopia after strabismus surgery.

We report the case of a 6-year-old boy with stable bilateral simple myopia who underwent strabismus surgery of the left eye for intermittent exotropia. One month following surgery, myopia had progressed to 2.5 D in the operated eye, increasing to 4.5 D by 1 year. Myopic progression was stabilized following treatment with low-dose atropine. The refractive error in the unoperated right eye showed mild increase. The mechanisms that may have precipitated postoperative progressive myopia in this case are discussed.

Comparative analysis of cycloplegic and non-cycloplegic refraction techniques for assessing refractive error in adults: a population-based study.

CLINICAL RELEVANCE: Accurate determination of refractive error is essential for optimal vision correction. However, the necessity of performing cyclo-refraction in adults remains a subject of ongoing ‎debate.‎. BACKGROUND: This study aimed to assess the agreement between cycloplegic retinoscopy, non-cycloplegic objective refraction methods (autorefraction and retinoscopy) and subjective refraction, in adults aged 40 to 64 years. METHODS: In this population-based cross-sectional study, ‎a multi-stage sampling technique was employed to select the sample from this population. Measurements of un

Ocular surface temperature measurement in diabetic retinopathy.

Infrared thermography provides functional imaging by picturing the temperature pattern of the region imaged. The temperature correlates to the blood flow pattern and is used in the diagnosis of diseases like breast cancer, peripheral vascular disorders, diabetic neuropathy and fever screening. In the present study, the usage of ocular thermography for diagnosis of diabetic retinopathy is explored. Ocular thermograms using infrared imaging camera were obtained for normal subjects (80 volunteers - 40 males and 40 females) age groups 21-30, 31-40, 41-50 and 51-60 years, non-proliferative diabetic

Ultrasound biomicroscopy in acetazolamide-induced myopic shift with appositional angle closure.

We report a case of a 49-year-old female who presented to the emergency department with blurred vision and vomiting, hours after taking two tablets of 250 mg of acetazolamide. The anterior chamber was bilaterally flat, with normal intraocular pressure in both eyes. An ultrasound biomicroscopic (UBM) examination showed bilateral ciliary effusion and complete appositional angle closure in all quadrants. Acetazolamide-induced bilateral angle closure was diagnosed. Steroid and cycloplegic therapy were initiated, and acetazolamide was discontinued. The following day, the anterior chamber had regain

Cardiopulmonary arrest following administration of Cyclomydril eyedrops for outpatient retinopathy of prematurity screening.

Eyedrops used for mydriasis and cycloplegia can be systemically absorbed, causing serious side effects, including oxygen desaturation, apnea, bradycardia, transient hypertension, delayed gastric emptying, and transient paralytic ileus. These effects can be more serious in infants because of their lower body mass and immature cardiovascular and nervous systems. We report a case of a 27-week-old infant who suffered a cardiopulmonary arrest after the administration of only Cyclomydril eyedrops (Alcon Laboratories, Fort Worth, TX) during an outpatient retinopathy of prematurity examination.

The influence of pharmacological mydriasis on biomicroscopic evaluation of the glaucomatous optic nerve head.

AIMS: To investigate whether pharmacological mydriasis influences interobserver agreement or within-observer agreement (comparing estimates made before and after dilation) in the evaluation of the optic nerve head (ONH) of the glaucoma suspect or patient. METHODS: Monoscopic disc photographs of the ONH were assessed by each observer on two separate occasions in order to establish baseline intra- and inter-observer agreement. Then the ONH of 53 eyes of 53 patients was examined by each observer on two separate occasions, the pupil being pharmacologically dilated on only one of these visits. Eac

[Neosynephrine mydriasis combined with general anesthesia with halothane in ocular surgery].

Ophthalmologic surgical interventions are performed in numerous cases under general anaesthesia with halothane, many of which requiring a preoperatory mydriasis. Neosinerfin is alpha-adrenoceptive sympathomimetic. Local use, in a 10% concentration produces a strong mydriasis, resistant to the opening of the aqueous chamber during the intervention. Its systemic effects are remarkable: it induces a peripheral blood vessel constriction which can unleash a reflex bradycardia followed by a shortcoming of the cardiac flow. The halothane has various effects upon the heart: reduces the cardiac frequen

Topographic analysis of Horner's syndrome.

Horner's syndrome or oculosympathetic paralysis is not an uncommon finding in patients with head and neck neoplasms. While in most cases the syndrome is easily established at the bedside, it can be confirmed and topographically defined as a central, preganglionic, or postganglionic lesion through sequential pharmacologic testing. The importance of such localization lies in differentiating neoplasia vs. a benign condition as the cause of the syndrome. Such variants as congenital Horner's, an alternating Horner's, and a pseudo-Horner's syndrome are discussed in regard to their differential featu

Drug reactions reported in a survey of South Carolina.

A survey of the membership of the South Carolina Ophthalmological Society revealed an average of 3 1/2 patient drug reactions per year per physician using topical diagnostic drugs in the office. The reactions to the drugs included topical allergic responses, acute glaucoma, cardiovascular collapse, and one death. All of the reactions had previous been attributed to these drugs in the literature. Suggestions are presented to prevent these reactions from occurring or to better prepare the ophthalmologist and his staff to manage these problems when they do arise.

[Rift valley fever and rickettsial retinitis including fluorescein angiography (author's transl)].

Three cases of Rift Valley fever retinitis and one case of Tick Bite fever retinitis are presented. The clinical features are indistinguishable, manifesting as localised areas of retinal ischaemia and soft exudate formation. Fluorescein angiography shows that there is primary involvement of the retinal circulation without involvement of the choroidal circulation. Vascular occlusion is a feature of the retinal pathology. These patients were treated with steroids and immunosuppressives as their maculae were threatened and they all regained normal to near normal vision except in one case in whom

Vision loss, disability and multimorbidity among older people in Andhra Pradesh and Telangana in South India: the longitudinal eye health, ageing and disability study (LEADS) - study protocol.

INTRODUCTION: Vision impairment (VI), multimorbidity and disabilities are common among older populations. Although the prevalence of VI and its associations are frequently reported, the incidence and risk factors are not well documented in India. The Longitudinal Eye Health, Ageing and Disability Study (LEADS) aims to: (a) determine the prevalence, causes, risk factors and impact of VI; (b) explore the associations between VI and sociodemographic risk factors, systemic health conditions (multimorbidity), physical performance, hearing, depression, social connectedness and cognitive function; an
