Best practice guidelines and recommendations on the molecular diagnosis of myotonic dystrophy types 1 and 2.

Myotonic dystrophy is an autosomal dominant, multisystem disorder that is characterized by myotonic myopathy. The symptoms and severity of myotonic dystrophy type l (DM1) ranges from severe and congenital forms, which frequently result in death because of respiratory deficiency, through to late-onset baldness and cataract. In adult patients, cardiac conduction abnormalities may occur and cause a shorter life span. In subsequent generations, the symptoms in DM1 may present at an earlier age and have a more severe course (anticipation). In myotonic dystrophy type 2 (DM2), no anticipation is desc…

Poor sleep quality and fatigue but no excessive daytime sleepiness in myotonic dystrophy type 2.

BACKGROUND: In myotonic dystrophy type 1 (DM1), sleep disorders are common, with excessive daytime sleepiness (EDS) as a predominant feature. In myotonic dystrophy type 2 (DM2), the presence of sleep disturbances is unknown. OBJECTIVE: To investigate the frequency of EDS, poor sleep quality and fatigue in DM2. METHODS: 29 genetically proven DM2 patients were surveyed using the Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index (PSQI) and Checklist Individual Strength. The results were compared with 29 adult onset DM1 patients and 65 population controls, both matched for age and sex. R…

Role of anterior and occipital white matter lesions for smooth eye tracking in myotonic dystrophy.

BACKGROUND: The degradation of smooth pursuit eye movements (SP) in myotonic dystrophy (MD) has been shown to result from lesions in the central nervous system. Imaging and histopathological studies have shown considerable areas of white matter lesions (WMLs) with periventricular emphasis. OBJECTIVE: To discover which of these WMLs are relevant in SP impairment? METHOD: Horizontal sinusoidal SP, the vestibulo-ocular reflex (VOR), and its suppression by fixation (VOR-S) were investigated in 12 patients with MD, and the data obtained were compared with those from a control group. RESULTS: Par…

The expression of ion channel mRNAs in skeletal muscles from patients with myotonic muscular dystrophy.

We investigated gene expression patterns of ion channels including the apamin-sensitive small-conductance Ca(2+)-activated K(+) (SK3) channel, the adult isoform of the skeletal muscle Na(+) channel (SkM1), the fetal isoform of skeletal muscle Na(+) channel (H1), and the Cl(-) channel (ClC-1) by using the semiquantitative reverse transcriptase-polymerase chain reaction (RT-PCR) for muscle samples from patients with adult onset myotonic dystrophy (DM), amyotrophic lateral sclerosis, and polymyositis. Patients with DM showed a significant increase in SK3 mRNA but not in mRNAs for other ion channe…

Leptin after IGF-I generation test in a patient with hypopituitarism and myotonic dystrophy disease.

A 54-years-old woman diagnosed of myotonic dystrophy (MyD) with past medical history of massive postpartum haemorrhage at age 28 and panhypopituitarism was studied. BMI and body composition were determined and we determined baseline serum IGF-I, IGFBP3, insulin and leptin levels and after the IGF-I generation test performed after the GH administration of 0.1 U/kg/day s.c each evening for 4 days. As expected the patient had lower baseline IGF-I and IGFBP3 with high insulin and leptin levels. After IGF-I generation test, IGF-I, IGFBP3 and insulin levels increases without changes in body composit…

Direct molecular analysis of myotonic dystrophy in the German population: important considerations in genetic counselling.

Myotonic dystrophy (DM) is associated with the expansion and instability of a trinucleotide (CTG) repeat at the DM locus on chromosome 19. Direct genomic analysis in the German population was carried out on 18 DM families, six families with equivocal diagnosis, 69 subjects with equivocal clinical diagnosis, and 100 controls using the polymerase chain reaction (PCR) and a refined Southern protocol. In the majority of the cases molecular analysis confirmed the clinical diagnosis. These included seven cases of congenital DM (CDM) with widely differing gene expansions and instabilities. In most DM…

Adrenocorticotropin hyperresponse to the corticotropin-releasing hormone-mediated stimulus of naloxone in patients with myotonic dystrophy.

We previously showed that CRH-mediated stimuli, including exogenous CRH, cause ACTH hypersecretion in many myotonic dystrophy (DM) patients. We confirmed this by giving naloxone, a stimulator of endogenous CRH release, to a large number of DM patients and controls. DM patients, first degree relatives, and normal controls received i.v. naloxone at 1400 h, and blood was taken for ACTH (RIA) and cortisol (high pressure liquid chromatography) measurements from 15 min before to 120 min after naloxone treatment. DM patients had basal ACTH levels approximately twice those of controls, and their ACTH …

Larger expansions of the CTG repeat in muscle compared to lymphocytes from patients with myotonic dystrophy.

The discovery that there is an expansion of a CTG repeat underlying myotonic dystrophy has led to new approaches in diagnosis and genetic counselling for this disorder. The size of the expansion correlates to a reasonable degree with the clinical symptoms within a given family. We report comparisons of the length of the expansion seen in lymphocytes and muscle samples from eight patients. In all cases the length of the expansion seen in DNA isolated from muscle was larger than that seen in lymphocytes from the same patient. There was no progression of the expansion over a period of 10-15 years…

Incidence, severity, and time-course of motoneurone dysfunction in myotonic dystrophy: their significance for an understanding of anticipation.

The numbers of functioning motor units and the amplitudes of the maximum evoked muscle responses have been measured in 198 muscles of 102 patients with myotonic dystrophy. Losses of units could be demonstrated in most of the extensor digitorum brevis and thenar muscles but less commonly in the hypothenar groups. A more proximal limb muscle, the soleus, was also shown to be involved frequently. Investigation of two premature infants withmyotonic dystrophy also revealed reductions of functiong units; in one infant clinical improvement was associated with increased muscle innervation. Repeated ex…

Maturational arrest of fetal muscle in neonatal myotonic dystrophy. A pathologic study of four cases.

Skeletal muscles from four infants with a severe neonatal form of myotonic muscular dystrophy showed histopathologic features of immaturity. … e and fiber types often could not be distinguished. Muscle fiber degeneration and other features of myotonic dystrophy in adult muscle were lacking. Electron microscopy showed fine granular chromatin and convoluted nuclear membranes of centronuclear fibers, dialated transvers tubules that were aligned longitudinally as in fetal myotubes, poorly formed Z-bands, simpl …

Generation of iPSC lines from myotonic dystrophy type 1 patients with varying CTG repeat lengths.

An expanded CTG trinucleotide repeat in the Dystrophia Myotonica Protein Kinase (DMPK) gene underpins myotonic dystrophy type 1 (DM1), an autosomal dominant neuromuscular disorder that affects almost every organ system, especially the skeletal muscle, central nervous system and the heart. In this study, we describe the generation of induced pluripotent stem cell (iPSC) lines from patient-derived fibroblasts carrying varying expanded (CTG)n repeats in DMPK. These lines provide a valuable resource of investigating how CTG repeat length correlates with DM1-related cellular phenotypes in different…

Lower urinary tract dysfunction in adults with myotonic dystrophy type 1: a multimodal clinical assessment.

Lower urinary tract dysfunction is an underrecognized manifestation of myotonic dystrophy type 1 (DM1), despite the multisystem nature of the disease. This study aimed to characterize the prevalence and clinical profile of lower urinary tract symptoms (LUTS), urinary incontinence, and pelvic floor dysfunction in adults with DM1 and to explore their association with functional status. In this cross-sectional study, 36 adults with genetically confirmed DM1 (50% men; median age 43 years, IQR 33-52.8) completed validated questionnaires assessing LUTS, urinary incontinence, and overactive bladder (…

MBNL depletion drives stem cell fusion and immature myonuclear states in myotonic dystrophy type 1.

Myotonic dystrophy type 1 is caused by the expression of expanded CTG repeats in the DMPK gene and the resulting loss of function of MBNL protein. Affected skeletal muscle displays abundant centrally located nuclei despite limited immune-cell-associated fibre necrosis, complicating interpretation of muscle damage and remodelling mechanisms. Here we show that muscle stem cells are activated and fuse with existing muscle fibres in myotonic dystrophy type 1. Single-nucleus transcriptomics in patient's muscle identifies increased activated muscle stem cells and distinct myonuclear populations exhi…

Response to noninvasive home mechanical vEntilation in Myotonic Dystrophy type 1: The multicenter REMeDY study.

Myotonic dystrophy type 1 (DM1) frequently leads to chronic respiratory failure, yet the effectiveness of noninvasive home mechanical ventilation (HMV) remains understudied. Our objective was to assess the effects of HMV on gas exchange, health-related quality of life (HRQL), and daily functioning, and to explore baseline predictors of treatment response. A prospective multicenter study was conducted in which clinical data, pulmonary function tests, blood gas analysis, polysomnography and overnight pulse oximetry with transcutaneous CO2 monitoring, and validated questionnaires were collected a…

Cognitive decline over time in myotonic dystrophy type 1: a systematic review of longitudinal studies.

BACKGROUND: Myotonic Dystrophy Type 1 (DM1) is a multisystem genetic disorder and the most common muscular dystrophy in adults. Central nervous system involvement is well-documented in DM1, including cognitive impairment. Cross-sectional studies have described a heterogeneous neuropsychological profile; however, the trajectory of cognitive functioning over time remains unclear, highlighting the need to focus on longitudinal studies. This systematic review aimed to examine longitudinal evidence on cognitive progression in DM1 and to analyze patterns of decline across clinical phenotypes. METHO…
