[Treatment of nail mycoses].

Onychomycosis is defined as an infection of the nail unit by fungi. Four clinical types of onychomycoses are recognized by the manner in which the nail plate is invaded. A combined therapy with elimination of the nail by surgical extraction or a chemical onycholysis, a short systemic antifungal therapy and a topical treatment until the healthy nail has grown is recommended.

Nail abnormalities and autoimmunity.

Eight patients are described with both vitiligo and nail abnormalities. Two had additional autoimmune disorders but these did not develop at the same time as the nail abnormality. Both had thyrotoxicosis. The proposed association with certain autoimmune disorders is discussed.

Transfixion intramedullary nail.

The Modny transfixion intramedullary nail has been used in 261 cases of complex femoral fractures with excellent results. Proximal, medial, and distal comminutions of all sizes were accurately transfixed by this open method. Transfixion screws and biologic ingrowth fixed the fragments rapidly, resulting in no measurable shortening or rotational deformity, and rapidity of healing was enhanced. The nail was also used in five knee fusions.

Yellow nail syndrome.

The case of a patient with yellow nail syndrome (YNS), an infiltrating duct carcinoma of the breast, and giant cell interstitial pneumonitis (GIP) is presented. YNS has not been previously described in association with GIP. There was improvement of the yellow fingernails following surgery and chemotherapy for the breast cancer. The possible pathogenesis of the yellow nails in this case and its management are presented. A wide variety of conditions are associated with YNS, which has been reported to respond to various treatment modalities. The most likely cause in our case is impaired lymphatic…

[The nail in vascular pathology].

The nail is part of the ungual unit; it is affected indirectly and late in vascular disease. The authors recall the anatomy and the blood supply of the ungual unit and describe the alterations in terms of the level of the lesion. They describe the changes seen in systemic diseases such as the collagen disease, in vascular disease, venous, arterial and lymphatic disease and in vascular acrosyndromes. These alterations are often a result of several diseases, but may, nevertheless, help orient the diagnosis.

[Surgery of the nail region].

Operations of the nail apparatus are done under sterile conditions and regional anaesthesia and in local anaemia. The knowledge of the complicated anatomy and physiology is the prerequisite for good surgical results. Nail extractions are commonly done too frequently. Subungual haematomas require a different treatment depending on the severity. Removal of the proximal third of the nail plate quickly clears acute paronychia resistent to antibiotics. The treatment of choice for the ingrown toenail is the surgical narrowing of the matrix. Kind and size of subungual tumours determine their surgical

Nail-patella syndrome.

Three cases with collagenation of glomerular basement membrane are presented. The ages of the patients are 8, 13, and 27 years. An 8-year-old boy presented with nephrotic syndrome; a 13-year-old girl presented with recurrent urinary tract infections, proteinuria, and edema; and a 27-year-old woman was noted during the evaluation of a cardiac murmur to have proteinuria and renal insufficiency. The changes on electron microscopy were identical to those observed in nail-patella syndrome, a rare hereditary disease with ectodermal and mesodermal involvement, manifested as bony and nail abnormalitie

Biomechanical analysis of anatomical intramedullary nails and proximal femoral anti-rotation-II intramedullary nails for the treatment of unstable intertrochanteric fractures (AO type 31-A2.2) in older patients.

PURPOSE: To explore the biomechanical properties of Zimmer natural nail (ZNN) and proximal femoral nail antirotation (PFNA)-II nails in treating older patients with unstable intertrochanteric fractures, thereby guiding the selection of internal fixation devices in clinical practice. METHOD: ZNN and PFNA fixation models for unstable intertrochanteric fractures (AO type 31-A2.2) were established using the three-dimensional finite element method and in vitro mechanical experiments, respectively. We compared the Von Mises stress distribution, peak stress, implants and femur locations, displacemen…

Periprosthetic distal femur fractures treated by retrograde intramedullary nails with a 10-degree distal bend achieve significantly better post-operative radiographic alignment when compared to conventional retrograde nails.

INTRODUCTION: Periprosthetic distal femur fractures (PPDFFs) are a common complication after total knee arthroplasty (TKA). In the setting of well-fixed TKA components, treatment options include retrograde intramedullary nailing (rIMN) or lateral locked plating. Treatment with rIMN has historically been associated with potential for extension deformity when using conventional nails. We hypothesized that the PPDFFs treated with an intramedullary nail with a specifically designed 10-degree distal bend for periprosthetic fractures would result in improved post-operative sagittal alignment compare…

Leprosy with dystrophy in twenty nails: Detection of acid-fast bacilli in nail biopsy.

Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae (M. leprae) and/or M. lepromatosis that has a wide spectrum of clinical presentations and multi-organ involvement, primarily on the skin and peripheral nerves. Recent studies have addressed the possibility of nail involvement in leprosy, but to this day limited studies have provided histological evidence confirming the presence of acid-fast bacilli (AFB) in the nail apparatus. A case of lepromatous leprosy-related dystrophy in 20 nails in a 37-year-old male was reported. Physical examination revealed, on all 2…
