Reduction of Nonoperative Time Using the Induction Room, Parallel Processing, and Sugammadex: A Randomized Clinical Trial.

BACKGROUND: An important variable in the operating room is the nonoperative time (NOT), the time between skin closure on a previous case and … ear processing for induction of anesthesia and OR cleaning and neuromuscular blockade reversal with neostigmine/glycopyrrolate) and the active group (parallel processing for induction of anesthesia and OR cleaning and neuromuscular blockade reversal with sugammadex). The primary outcome measured is the difference in the NOT. The secondary outcomes are surgeon …

Acute colonic pseudo-obstruction.

Acute colonic pseudo-obstruction (ACPO) is a syndrome of massive dilation of the colon without mechanical obstruction that develops in hospi … ent for greater than 6days. Appropriate management includes supportive therapy and selective use of neostigmine and colonoscopy for decompression. Early recognition and management are critical in minimising complications.

[Studies on predilection area of gastric ulcer viewed from gastric motility (author's transl)].

Clinically, most gastric ulcers are located in the region of the incisura angularis, but they are least found along the greater curvature. To elucidate this difference, following experiments were carried out. A total of 22 mongrel dogs were used. A set of bipolar electrodes and/or a water-proof strain gage were chronically implanted at both the incisura and the corresponding site of the greater curvature. Electrical and contractile activities of these two sites were compared at the fasted state and under the stimulations by food, neostigmine and tetragastrin. Results are summarized as follows.…

Overlap of limited cutaneous systemic sclerosis with ocular myasthenia gravis- report of a case and a brief review of literature.

A 39-year-old woman with a 4-year history of limited cutaneous systemic sclerosis (lcSSc) and no prior exposure to D-penicillamine presented with bilateral fluctuating ptosis of 1 week duration. On evaluation, she was found to have elevated acetylcholine receptor antibody levels and a positive neostigmine test confirming the diagnosis of ocular myasthenia gravis (MG). The patient was treated with oral prednisolone and pyridostigmine resulting in significant improvement within 2 weeks. The co-occurrence of systemic sclerosis (SSc) and MG is rare and when such an association is seen, it is usual…

Pharmacological inhibition of acetylcholinesterase improves the locomotion defective phenotype of a SCA3 C. elegans model.

Inhibition of acetylcholinesterase (AChE) is a common used treatment option for Alzheimer's disease. However, there has been limited researc … nd is characterized by motor impairments. Our study shows that administration of the AChE inhibitor neostigmine is beneficial in treating the locomotion defective phenotype of a SCA3/MJD model of C. elegans and highlights the potential contribution of AChE enzymes to mutant ataxin-3-mediated toxicity.

Inhibitors of acetylcholinesterase and butyrylcholinesterase meet immunity.

Acetylcholinesterase (AChE) inhibitors are widely used for the symptomatic treatment of Alzheimer's disease and other dementias. More recent … immune response using as examples the commonly available drugs, donepezil, galantamine, huperzine, neostigmine and pyridostigmine. Major attention is given to the cholinergic anti-inflammatory pathway, a well-described link between the central nervous system and terminal effector cells in the immune system.

M(2)/M(4)-muscarinic receptors mediate automodulation of acetylcholine outflow from mouse cortex.

Acetylcholine outflow can be modulated through inhibitory presynaptic muscarinic autoreceptors. This study was to identify which subtype is involved in mouse cortex. Five muscarinic antagonists and their ability to elevate stimulation-induced (S-I) acetylcholine outflow were tested in the presence of neostigmine, which decreased S-I outflow. The potency of each antagonist was determined, expressed as a ratio of the potency of each other antagonist and compared with the potency ratios of the antagonists for each of the defined muscarinic receptors (M(1)-M(4)), as recorded in the literature. Lin…

Synaptosomal phospholipase D potential role in providing choline for acetylcholine synthesis.

The phospholipase D of the rat brain synaptic membrane possesses the highest activity of this enzyme of any mammalian tissue examined. The synaptic phospholipase D activity is latent and barely detectable in the absence of 4 mM sodium oleate. Several other fatty acids were either less effective or ineffective as stimulators of activity compared to this monounsaturated fatty acid. The activity was decreased by hemicholinium-3, an inhibitor of choline uptake and slightly activated by neostigmine, an acetylcholinesterase inhibitor. Incubation of synaptosomes in the presence of sodium oleate and a…

Allergic Reactions to Sugammadex: A Case Series and Review of the Literature.

Sugammadex is a novel agent for the reversal of neuromuscular blockade; it acts by encapsulating -rocuronium or vecuronium, eliminating the active compound from the circulation, thereby providing rapid and complete recovery even with profound or complete neuromuscular blockade. Clinical advantages, including reduced incidence of residual blockade, decreased nausea and vomiting, decreased dry mouth, less change in heart rate, and reduced pulmonary complications, have been demonstrated when comparing sugammadex to conventional agents, such as neostigmine, that inhibit acetylcholinesterase. Altho…

Sugammadex: a selective relaxant binding agent for reversal of neuromuscular block.

Neostigmine, the currently used agent for reversal of neuromuscular block, has several drawbacks, such as a slow onset of peak effect, inability to reverse deep block and the occurrence of widespread muscarinic effects. Sugammadex is a new class of reversal agent that acts by encapsulating the molecules of the relaxants rocuronium and vecuronium, for which it is a specific antagonist. Clinical trials in over 2000 subjects have shown it to be effective for reversal of block by these two relaxants even from deep levels, in doses of 2.0-4.0 mg/kg. Sugammadex can also reverse an intense (profound)…

Therapy and clinical progress of cattle with dilatation and torsion of the caecum.

One hundred and eleven heifers and cows with caecal dilatation and torsion were examined and their subsequent progress monitored. Using various criteria it was decided whether the animal was to be slaughtered or treated conservatively or surgically. Conservative treatment consisted of a continuous drip infusion containing neostigmine and of purgatives such as liquid paraffin, sometimes in combination with sodium sulphate. Surgical treatment consisted of laparotomy on the right flank with emptying and sometimes partial resection of the caecum. Five animals had to be slaughtered before or during…

Multiple neuromuscular blocking agents and reversal in a patient with absent plasma cholinesterase.

The detailed clinical response of a patient with absent plasma cholinesterase (genotype E1s E1s) who received tubocurarine (3 mg), succinylcholine (120 mg), pancuronium (2 mg), and reversal with neostigmine (3 mg) is reported. The patient's responses were compared to the responses of a group of patients with genotype E1a E1a evaluated prospectively, and with eight other genotype E1s E1s patients reported in the literature. The present patient demonstrated evidence of a phase II block before and after attempted reversal, suggesting that free succinylcholine was present in her plasma and a mixed…

[Treatment of functional ileus: adrenergic blockade and stimulation (author's transl)].

Intestinal stimulation is among the basic therapeutic procedures in functional ileus. But the majority of cases of functional ileus is due not to paralysis but to adrenergic inhibition. Thus the primary treatment should not be to stimulate but to act against the inhibition. In over 100 patients a combined treatment schedule consisted of primary adrenergic blockade (trifluperidol, dihydroergotamine or chlorpromazine) followed by low doses of stimulants (ceruletide, neostigmine). 52 patients who had not responded to high doses of peristalsis-stimulating drugs acted as their own controls: in 48 o…

Contemporary opinions about Mary Walker: a shy pioneer of therapeutic neurology.

OBJECTIVE: To contribute to a better understanding of a poorly appreciated pioneer of therapeutic neurology, Mary Broadfoot Walker, MD. BACKGROUND: At a time when the treatment of myasthenia gravis (MG) was "a source of discouragement to the patient and a cause of nightmare for the physician," Mary Walker demonstrated that temporary relief of myasthenic symptoms could be produced by subcutaneous injection of physostigmine or neostigmine (Prostigmin; Roche, Basel, Switzerland). She also pioneered the concept of a circulating factor as the etiology of myasthenia and was the first to report hypo…
