Functioning transferred free muscle innervated by part of the vascularized ulnar nerve connecting the contralateral cervical seventh root to themedian nerve: case report.

BACKGROUND: The limited nerve sources available for the reconstruction and restoration of upper extremity function is the biggest obstacle in the treatment of brachial plexus injury (BPI). We used part of a transplanted vascularized ulnar nerve as a motor source of a free muscle graft. CASE PRESENTATION: A 21-year-old man with a left total brachial plexus injury had received surgical intercostal nerve transfer to the musculocutaneous nerve and a spinal accessory nerve transfer to the suprascapular nerve in another hospital previously. He received transplantation of a free vascularized gracili…

Anterior vs. posterior approach for spinal accessory nerve transfer to suprascapular nerve in brachial plexus injury: a systematic review and meta-analysis of comparative studies.

Spinal accessory nerve (SAN) to suprascapular nerve (SSN) transfer is an effective surgical option for traumatic brachial plexus injuries (BPIs) when nerve grafting is not applicable. It is performed via two approaches: anterior and posterior. Despite the theoretical advantages of the posterior approach, clinical trials have yielded variable outcomes. This study aimed to compare the outcomes of anterior and posterior approaches for SAN to SSN transfer in restoring the Range of motion (ROM) and strength of shoulder abduction and external rotation in BPIs. We searched PubMed, Embase, Cochrane Li

Functioning Free Muscle Transfer for Brachial Plexus Injury: A Systematic Review and Pooled Analysis Comparing Functional Outcomes of Intercostal Nerve and Spinal Accessory Nerve Grafts.

BACKGROUND: The aim of this study was to compare postoperative elbow flexion outcomes in patients receiving functioning free muscle transplantation (FFMT) innervated by either intercostal nerve (ICN) or spinal accessory nerve (SAN) grafts. METHODS: A comprehensive systematic review on FFMT for brachial plexus reconstruction was conducted utilizing Medline/PubMed database. Analysis was designed to compare functional outcomes between (1) nerve graft type (ICN vs. SAN) and (2) different free muscle graft types to biceps tendon (gracilis vs. rectus femoris vs. latissimus dorsi). RESULTS: A total

Latissimus Dorsi Transfer With Interposed Autograft for Axillary Nerve Palsy After Reverse Shoulder Arthroplasty: A Case Report.

Persistent axillary nerve palsy after reverse shoulder arthroplasty (RSA) results in profound deltoid dysfunction and loss of active shoulder elevation. Traditional reconstructive options such as nerve repair or transfer may not be feasible in chronic cases. Latissimus dorsi tendon transfer augmented with an interposed graft has been described to restore shoulder abduction in deltoid-deficient shoulders, but its application after RSA is not well documented. We present the case of a 73-year-old female patient who developed chronic axillary nerve palsy following RSA performed for complex proxima

Anterior Interosseous Nerve to Pronator Quadratus Transfer to Restore Intrinsic Function: An Adjunct to Cubital Tunnel Decompression.

Background Injury or compression of the ulnar nerve impairs fine motor control, intrinsic hand function, and sensation in the small and ulnar side of the ring finger. Anterior interosseous nerve to pronator quadratus (AINPQ) transfer offers a potential solution, leveraging this expendable nerve to enhance the recovery of the ulnar nerve's motor function. Method This single-unit retrospective case series evaluates seven patients undergoing combined cubital tunnel decompression and AINPQ transfer for severe ulnar neuropathy. Data were collected from December 2020 to January 2023, including age,

Anconeus Muscle Transfer to Correct or Prevent Wrist Radial Deviation in Radial and Posterior Interosseous Nerve Injuries.

PURPOSE: Wrist radial deviation is a possible complication of tendon transfer for restoration of wrist extension in cases of radial nerve paralysis. In posterior interosseous nerve (PIN) injury, this is because of the imbalance caused by the intact extensor carpi radialis longus and paralysis of the extensor carpi ulnaris (ECU). This deformity may also occur following transfer of the pronator teres (PT) to the extensor carpi radialis brevis (ECRB) for radial nerve palsy. To address wrist radial deviation, we propose transferring the anconeus muscle, extended by the intermuscular septum between

Triceps to teres minor motor nerve transfer to restore glenohumeral external rotation after neonatal brachial plexus injury.

INTRODUCTION: Attaining active glenohumeral external rotation (aGHER), whether via primary reconstruction or spontaneous recovery, is infrequent in patients with neonatal brachial plexus palsy (NBPI). We evaluated the effectiveness of triceps-to-teres minor motor branch transfers to restore this function, both performed primarily (i.e., in conjunction with microsurgical plexus reconstruction) or secondarily (after primary surgery has failed to restore aGHER). PATIENTS AND METHODS: This was a retrospective study of 12 children with NBPI undergoing triceps-to-teres minor motor branch transfer v

Supercharged End-to-Side Anterior Interosseous to Ulnar Motor Nerve Transfer for Hirayama Disease: A Case Report.

Hirayama disease is a rare condition of cervical myelopathy. Its early identification and correction can optimize functional outcomes. However, late presentation and some more severe cases may be associated with loss of hand function. Among the cases described, there are no reports of nerve transfers for this condition. We presented the first case report of a Hirayama disease of isolated ulnar nerve impairment managed with nerve transfer. Electroneuromyography showed isolated preganglionic involvement of C7, C8, and T1, with no sensory changes. The patient underwent nerve transfer with anterio…

Clinical Anatomy of Human Donor C7 Nerve Roots for Surgical Transfer in Patients with Spastic Arm Paralysis.

BACKGROUND: Contralateral C7 (CC7) nerve transfer has successfully restored hand function in patients with spastic hemiplegia from chronic central nervous system injuries. However, little is known about the morphology and anatomy of the donor C7 nerve root in patients undergoing this procedure. This study quantified intraoperative measurements of donor C7 nerve roots during CC7 transfer surgery for spastic hemiplegia in patients treated at a high-volume center to describe observed anatomical variations for successful direct anastomosis. METHODS: A database of images from 21 patients (2 female…

Phrenic to musculocutaneous nerve transfer for traumatic brachial plexus injuries: analyzing respiratory effects on elbow flexion control.

OBJECTIVE: In this study, the authors sought to identify the relationship between breathing and elbow flexion in patients with a traumatic brachial plexus injury (TBPI) who undergo a phrenic nerve (PN) transfer to restore biceps flexion. More specifically, the authors studied whether biceps strength and the maximal range of active elbow flexion differ between full inspiration and expiration, and whether electromyography (EMG) activity in the biceps differs between forced maximum breathing during muscular rest, normal breathing during rest, and at maximal biceps contraction. All these variables

Adenoviral gene transfer of bone morphogenetic protein-7 enhances functional recovery after sciatic nerve injury in rats.

Bone morphogenetic proteins (BMPs), members of the transforming growth factor-β subfamily, function as instructive signals for neuronal lineage commitment and promote neuronal differentiation. However, the mechanism of BMP7 action in vivo after peripheral nerve injury is poorly understood. This study examines the efficacy of gene transfer of adenoviral (Ad) BMP7 on peripheral neuropathy. Transgene expression was found in both Ad-infected sciatic nerves and their respective remote neurons, indicating Ad transduction by a retrograde transport. After AdBMP7 infection to nerves, the sciatic nerves

Passive transfer of Lambert-Eaton myasthenic syndrome induces dihydropyridine sensitivity of ICa in mouse motor nerve terminals.

Mice were injected for 30 days with plasma from three patients with Lambert-Eaton Myasthenic Syndrome (LEMS). Recordings were made from the perineurial sheath of motor axon terminals of triangularis sterni muscle preparations. The objective was to characterize pharmacologically the identity of kinetically distinct, defined potential changes associated with motor nerve terminal Ca2+ currents (ICa) that were affected by LEMS autoantibodies. ICa elicited at 0.01 Hz were significantly reduced in amplitude by approximately 35% of control in LEMS-treated nerve terminals. During 10-Hz stimulation, IC

Supercharged End-to-Side Anterior Interosseous-to-Ulnar Motor Nerve Transfer: A Systematic Review and Meta-analysis.

PURPOSE: Ulnar nerve (UN) injuries frequently result in intrinsic hand muscle weakness and functional impairment, particularly in high-level lesions where prolonged axonal regeneration limits recovery. The supercharged end-to-side (SETS) anterior interosseous nerve (AIN)-to-ulnar motor nerve transfer was developed to enhance distal reinnervation and preserve intrinsic motor endplates. This study aimed to evaluate outcomes following SETS AIN-to-ulnar motor nerve transfer for UN pathology. METHODS: A systematic review and meta-analysis were conducted in accordance with Preferred Reporting Items…
