Proper Performance and Interpretation of Electrodiagnostic Studies. [Corrected].

The AANEM strongly recommends that EDX procedures be performed by physicians with comprehensive knowledge of neurological and musculoskeletal disorders to assure accurate interpretation and diagnosis. Individuals without a medical education in neuromuscular disorders and without special training in EDX procedures typically are not qualified to interpret the waveforms generated by NCSs and needle EMGs or to correlate the findings with other clinical information to reach a diagnosis. It is also the AANEM's position that the same physician should directly supervise and interpret the NCSs includin

A conduction block in sciatic nerves can be detected by magnetic motor root stimulation.

Useful diagnostic techniques for the acute phase of sciatic nerve palsy, an entrapment neuropathy, are not well established. The aim of this paper is to demonstrate the diagnostic utility of magnetic sacral motor root stimulation for sciatic nerve palsy. We analyzed the peripheral nerves innervating the abductor hallucis muscle using both electrical stimulations at the ankle and knee and magnetic stimulations at the neuro-foramina and conus medullaris levels in a patient with sciatic nerve palsy at the level of the piriformis muscle due to gluteal compression related to alcohol consumption. On

[Old or new medicine? Vitamin B12 and peripheral nerve neuropathy].

Methylcobalamin is a vitamin B12 analog that is necessary for nervous system maintenance. Although methylcobalamin has some positive effects on peripheral nervous system disorders, the mechanism through which it affects neurons are not entirely known. Recent studies have revealed its intracellular signaling pathway and some of its molecular actions on neurons. In this article, I review interactions between methylcobalamin and neurons that have been revealed through in vitro studies, in vivo studies, and clinical use. Methylcobalamin participates in nervous system maintenance through several me

Conduction block and axonal degeneration co-occurring in a patient with axonal Guillain-Barré syndrome.

Guillain-Barré syndrome can present as demyelinating or axonal subtypes. Recent studies suggest that the neurophysiology of the axonal subtype not only exhibits axonal degeneration but can also show reversible conduction failure at the early stages. It is less certain if reversible conduction failure is evident in only a minority of patients with a specific subtype associated with better prognosis. We describe a 73-year-old man with Guillain-Barré syndrome and electrodiagnostic features of both reversible conduction block and possible axonal degeneration. Initial features of conduction failure

Electrically silent muscle visible by ultrasound.

A 65-year-old woman with multiple chronic cranial neuropathies had spinal accessory innervated muscles that were virtually invisible to electromyography. Ultrasound imaging revealed the extensive atrophy and increased echogenicity that corresponded to the thinness of the muscles and their loss of insertional activity. In patients with severe atrophy of trapezius or sternocleidomastoid muscles, ultrasound may help in identify chronically denervated muscle.

Correlation of sonographic and electrophysiological findings in a patient with multifocal motor neuropathy.

BACKGROUND AND PURPOSE: Multifocal motor neuropathy (MMN) is an acquired, immune mediated, and commonly associated with antiganglioside antibodies against GM1 lower motor neuropathy, with an incidence of 1 per 100,000. The usual age of onset is between 20 and 50 years and men appear to be more often affected than women. Patients usually present with multifocal weakness that can be localized to named nerve distributions. METHODS: The gold standard for the diagnosis is the electrophysiological detection of focal, partial conduction block of motor fibers without involvement of sensory fibers, at

Ulnar neuropathy in hansen disease: clinical, high-resolution ultrasound and electrophysiologic correlations.

OBJECTIVE: To assess the relationship between the cross-sectional area (CSA) of the ulnar nerve by ultrasound (US) with clinical and electrophysiologic findings in Hansen ulnar neuropathy. METHODS: Twenty-one patients (42 arms) with Hansen disease (mean age 30.0 ± 12.97, range 13-61 years, borderline tuberculoid 29%, borderline lepromatous, 19% lepromatous leprosy 42%, and pure neuritic type 10%) were examined clinically for ulnar sensory and motor weakness. The ulnar nerve was ultrasonographically examined from the wrist to the axilla, and CSA was measured at the level of maximum enlargement

Carpal tunnel syndrome diagnosed by general practitioners: an observational study.

The aim of this study was to investigate the results of both clinical testing and standardised nerve conduction studies performed on patients with Carpal tunnel syndrome (CTS) complaints, who had been referred to the neurologist by their general practitioners. Analysis of the data of neurological examination and electrodiagnostic tests (EDX) were performed on patients that had been referred by general practitioners. A total of 232 patients with clinically defined CTS, who had been referred by general practitioners, were seen by a neurologist and subsequently underwent electrodiagnostic testing

Motor root conduction block in the Lewis-Sumner syndrome.

BACKGROUND: The Lewis-Sumner syndrome (LSS) is a rare immune-mediated peripheral nerve disorder presenting with asymmetric upper limb sensory complaints and motor weakness. Asian patients with LSS have not been reported in the English literature. METHODS: Three Asian patients with features of LSS were prospectively studied. RESULTS: Our patients tended to older, female, and have involvement of the upper limbs exclusively than those in the West. They have a markedly longer disease duration before a diagnosis was made, which could also be the result of difficulty in eliciting motor root conduc

Acute shoulder-girdle neuralgic amyotrophy (Parsonage-Turner syndrome with saphenous nerve involvement.

Parsonage-Turner syndrome (PTS) is a relatively rare cause of upper extremity weakness and pain. There is currently no effective treatment for PTS although corticosteroids are recommended. Here we report the case of a man with acute PTS and exceptional involvement of both saphenous nerves, advantageously treated with intravenous immunoglobulin.

Ulnar neuropathy with normal electrodiagnosis and abnormal nerve ultrasound.

Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy. It is diagnosed with electrodiagnostic studies, but they can yield false-negative results. Ultrasound was used to examine 4 patients with UNE and negative electrodiagnostic findings, and it showed ulnar nerve enlargement near the elbow in all cases, with a mean cross-sectional area of 20.1 mm. This indicates that ultrasound may be a useful tool for assessing those with UNE symptoms and normal electrodiagnostic findings.

Clinical and electrophysiological characterization of myokymia and neuromyotonia in Jack Russell Terriers.

BACKGROUND: Generalized myokymia and neuromyotonia (M/NM) in Jack Russell Terriers (JRTs) is related to peripheral nerve hyperexcitability syndrome in humans, a symptom complex resulting from diverse etiologies. OBJECTIVE: Clinical and electrodiagnostic evaluation is used to narrow the list of possible etiological diagnoses in JRTs with M/NM. ANIMALS: Nine healthy JRTs and 8 affected JRTs. METHODS: A prospective study was conducted comparing clinical and electrophysiological characteristics in 8 JRTs affected by M/NM with 9 healthy JRT controls. RESULTS: All affected dogs except 1 had clin

Botulism in 2 urban dogs.

Two dogs from the same owner were referred for ascending weakness and paresis of 2 to 3 days duration. Electromyography and electroneurography determined that there were normal F-waves, decreased compound action potential, and decreased activity on repetitive nerve stimulation. These findings were valuable in diagnosing botulism in the dogs.
