The anabolic effects of clenbuterol have been recognized for a long time. Clenbuterol augments the expression of specific muscle proteins with a differential effect on type I and type II fibres. Furthermore, clenbuterol induces the synthesis of endogenous nerve growth factor (NGF) and may itself be a myotrophic factor released by neuron endings. Side effects include tremor and headache and dose dependent abnormalities of laboratory values (hypokalemia, hypoglycemia). After long-term medication increasing fatigue of muscles has been observed. Decreased expression of beta 2-adrenergic receptors

It has known for a long time that Sydenham's chorea and tics, as seen in Gilles de la Tourette's syndrome (GTS), are phenomenologically very similar. Tics may occur as symptoms of acute Sydenham's chorea or persist over years as residual symptoms. Investigating of children suffering from GTS, including obsessive-compulsive symptoms, have provided signs of a poststreptococcal autoimmune process but also shown that treatment based on immunological interventions has been effective. We treated a 14-year-old boy showing all diagnostic criteria of GTS, familial susceptibility, and an increase in the

OBJECTIVE: To review the epidemiology, phenomenology, and treatment of psychotic disorders in late life.
METHOD: The literature relating to psychotic symptoms in the elderly is reviewed, with a focus on the following categories: primary psychotic disorders, mood disorders, delirium, Parkinson's disease (PD), and somatic hallucinoses (including Charles Bonnet syndrome [CBS] and musical hallucinosis). Practical clinical treatment implications are discussed.
RESULTS: The prevalence of psychotic symptoms increases with age, largely because of underlying medical illnesses such as dementia, deliri

Physiological investigations continue to define the processes underlying different types of myoclonus, particularly cortical forms. Several new aetiologies have been described and genetic advances have revealed the basic deficit in familial hyperekplexia.

Recently, a new assessment technique by which to evaluate brain function in the fetus and newborn infant has been developed. The method is based on the assessment of the quality of General Movements (GMs). GMs are complex movements involving all parts of the body. They are present throughout fetal life and postnatally until about 3-4 months post-term. The persistent presence of abnormal GMs during the neonatal period and the absence of age-specific characteristics ("fidgety" movements) around 3 months-post term are both powerful predictors of the development of handicap.

The carotid bodies, located at the bifurcation of the carotid arteries, rarely develop neoplasms or carotid body tumors. The most interesting aspects of this rare tumor are its anatomical location, familial tendency, physiologic alterations related to removal, and surgical management. Tumors of the carotid body can occur alone or bilaterally. Prompt and accurate diagnosis is essential so that resection can be done in the early stage of the disease. Nurses must be aware of the risks and potential complications that may be encountered after surgical excision of this tumor.

Ataxia telangiectasia (AT) is an autosomal recessive hereditary disorder characterized by onset in infancy or childhood of a cerebellar and later extrapyramidal disorder associated with telangiectasias and an immune deficit. Only a handful of cases have been described in which the features were not stereotypic. This report describes a case that is classic except for the absence of telangiectasias through age 17. This and other cases suggest that a new, more inclusive term be used to describe the syndrome of ataxia with immune deficit until the genetic abnormalities in these disorders become id

We report a 67-year-old female with orthostatic and voice tremor. Her orthostatic tremor mainly affected her lower extremities, alternating between antagonist muscle groups at a frequency of 4.4-4.8 Hz. The voice tremor ranged between 4.8 and 8.8 Hz. In this case, the frequency of voice tremor was same as that of orthostatic tremor, suggesting a common origin from a tremor-generating mechanism. These tremors were diagnosed as 'forme fruste' of the essential tremor, not the incipient stage of Parkinson's disease. Medications including clonazepam, perphenazine, Dopa and trihexyphenidyl hydrochlo

The author gives a literature review of the management of spasticity carried out. Physical agents used such as ice, heat and vibration are described. Methods used by physical therapists are discussed. Drugs most commonly used in the treatment of spasticity are Diazepam, Baclofen and Dantrolene. Phenol has been used by injection into motor points, peripheral nerves and intrathecally. Other drugs that have been used are also discussed. Neurosurgical procedures include neurectomies, neurotomies, rhizotomies and more radical procedures such as myelotomies and cordectomies. Orthopaedic procedures i

Every neurosurgeon can appreciate Dandy's recognition that the drainage of brain abscesses causes trauma to the delicate parenchyma. Over the years, brain surgery has evolved toward management of problems by using less and less invasive techniques and thus gaining ever lower morbidity. Clearly, the advent of better imaging techniques has improved the outcome in patients afflicted with intracerebral infections. The combination of stereotaxy with these imaging techniques is contributing a "zero mortality" in the treatment of these infections. In our series of 29 consecutive patients with non-AID

Acoustic neuromas are benign tumors originating from the Schwann cells of the eighth cranial nerve. Symptoms occur related to compression and stretching of cranial nerves which alters their function. Surgical management is by middle fossa, translabyrinthine, suboccipital or combined approach craniotomy. Prevention and early recognition of postoperative complications are important aspects of nursing care for these clients. A holistic approach to client and family nursing needs is presented. Discharge planning should include assessment of client learning and home care needs. The Acoustic Neuroma

We describe four patients with cerebral infarction and active neurosyphilis who had variable angiographic findings. Patients 1 and 2 had evidence of arteritis and an atheromatous ulcerated stenosis of the extracranial carotid artery, respectively. The third and fourth patients had symptoms of lacunar infarction, but cerebral angiography was normal in patient 3, whereas patient 4 had evidence of concomitant atheromatous and arteritic lesions. Neither age nor presence of cerebrovascular risk factors was of aid in predicting angiographic features. The concomitance of cerebral infarction and activ

We describe 2 instances where sciatic nerve block prevented pain in the sciatic nerve distribution caused by the nerve-root tension test in patients with radiculopathy. We hypothesize that antidromic activation of nociceptors from the injured root is a mechanism underlying sciatica.
