Dynamics of loss and recovery of the nerve monitoring signal during thyroidectomy predict early postoperative vocal fold function.

BACKGROUND: The characteristics of segmental type 1 and global type 2 injuries to the recurrent laryngeal nerve (RLN) and the extent and dynamics of nerve recovery are poorly understood. METHODS: This investigation of 785 patients who underwent thyroidectomy under continuous intraoperative nerve monitoring aimed at exploring the dynamics of loss and recovery of the nerve monitoring signal and its relationship to early postoperative vocal fold palsy. RESULTS: Persistent complete loss of signal and signal recovery <50% identified all (based on 12 and 4 patients with type 1 injuries) or most (b

Neuromuscular Monitoring and Wake-Up Test During Scoliosis Surgery.

A typical spine is straight and symmetrical, with all of the vertebrae facing forward when viewed from the posterior. Scoliosis is a term used to describe a lateral deviation of the spine's normal vertical line that is greater than 10° on an X-ray. More severe curves are sometimes accompanied by medical issues such as increased back discomfort and cardiorespiratory difficulties along with the cosmetic components of the deformity. The test for spinal cord integrity prior to the widespread adoption of intraoperative neurophysiologic monitoring (IOM) was the wake-up test. In this article, we revi

Performance monitoring and post-error adjustments in adults with attention-deficit/hyperactivity disorder: an EEG analysis.

BACKGROUND: Recently, research into attention-deficit/hyperactivity disorder (ADHD) has focused increasingly on its neurobiological underpinnings, revealing (among other things) frontal lobe alterations. Specifically, action-monitoring deficits have been described, including impaired behavioural adjustments following errors. Our aim was to examine the neurophysiological background of post-error behavioural alterations in an adult ADHD sample for the first time, hypothesizing that people with ADHD would differ from controls in neurophysiological markers of cognitive preparation and stimulus pro

Muscle quantitative MRI as a novel biomarker in hereditary transthyretin amyloidosis with polyneuropathy: a cross-sectional study.

BACKGROUND: The development of reproducible and sensitive outcome measures has been challenging in hereditary transthyretin (ATTRv) amyloidosis. Recently, quantification of intramuscular fat by magnetic resonance imaging (MRI) has proven as a sensitive marker in patients with other genetic neuropathies. The aim of this study was to investigate the role of muscle quantitative MRI (qMRI) as an outcome measure in ATTRv. METHODS: Calf- and thigh-centered multi-echo T2-weighted spin-echo and gradient-echo sequences were obtained in patients with ATTRv amyloidosis with polyneuropathy (n = 24) and h

Monitoring of motor pathways during brain stem surgery: what we have achieved and what we still miss?

Intraoperative neurophysiological monitoring (IOM) has established itself as one of the paths by which modern neurosurgery can improve surgical results while minimizing morbidity. IOM consists of both monitoring (continuous "on-line" assessment of the functional integrity of neural pathways) and mapping (functional identification and preservation of anatomically ambiguous nervous tissue) techniques. In posterior-fossa and brainstem surgery, mapping techniques can be used to identify - and therefore preserve - cranial nerves, their motor nuclei and corticospinal or corticobulbar pathways. Simil…

Inside a Programmer's Brain: Dissociating Code and Language Comprehension Networks Using a Novel fMRI-ESM Protocol.

Code comprehension and natural language share structural similarities but engage partially distinct neural systems. While language predominantly recruits the classical perisylvian language network, code comprehension has been linked to the multiple-demand system, associated with mathematical thinking and problem solving. We report the case of a professional software engineer with an IDH-mutant astrocytoma in the left middle frontal gyrus, a region where these networks converge. To address the potential risk of impairing computer programming abilities during surgery, we implemented a personaliz

Duration of EEG monitoring needed to ensure a low risk of seizure recurrence in hospitalized patients.

BACKGROUND: Continuous EEG (cEEG) monitoring is critical for detecting seizures in hospitalized patients, but optimal durations of EEG monitoring remain unclear, particularly after a seizure has been detected on monitoring. METHODS: We conducted survival analysis on 117 patients with electrographic or electroclinical seizures who underwent cEEG at the University of Wisconsin Hospital (UWH) between 2018 and 2022. Time from the end of seizure-to-seizure recurrence or cEEG termination was analyzed using Cox regression. RESULTS: In univariate analysis, status epilepticus (SE) was the only clinic

Intraoperative electrophysiological balloon test occlusion (BTO): A novel Solution to an inconclusive conventional BTO.

Parent Artery Occlusion (PAO) is a valid treatment choice in giant internal carotid artery (ICA) aneurysms of the cavernous segment when the preoperative balloon test occlusion (BTO) demonstrates an adequate cross circulation from the contralateral side. A high flow arterial bypass is, however, mandatory if the result suggests otherwise or is indeterminate. We present here a case of a 72-year lady where the BTO results were inconclusive. Rather than proceeding for the ICA ligation and a high flow bypass straight away, we replicated the BTO in the operation theatre, using intraoperative electro

Absence of STN Neuronal Activity during MER in DBS Surgery: Diagnostic and Clinical Implications.

UNLABELLED: <p>Introduction: Intraoperative microelectrode recording (MER) is a widely employed technique for the physiological identification of the subthalamic nucleus (STN) during deep brain stimulation (DBS) surgery targeting the STN. However, failure to detect typical STN activity may raise concerns about diagnostic accuracy and treatment efficacy. Objective of this study were to retrospectively evaluate the clinical characteristics and long-term outcomes of patients with advanced Parkinson's disease (PD) in whom STN neuronal activity was not detected during intraoperative MER and to expl

Neuromonitoring of recurrent laryngeal nerve using continuous intraoperative neuromonitoring system during anterior cervical spine surgery: A porcine model study.

PURPOSE: Recurrent laryngeal nerve (RLN) injury is a recognized complication of anterior cervical spine surgery (ACSS), particularly during anterior cervical discectomy and fusion (ACDF). This study explores the use of continuous intraoperative neurophysiological monitoring (CIONM) with an endotracheal electromyography (EMG) tube during ACSS in a porcine model, focusing on adverse EMG signal events during retraction and the differences in RLN injury and recovery between surgical levels and approach side (left or right). MATERIALS AND METHODS: All 12 pigs were intubated with an EMG-equipped en…

Detecting and Addressing Secondary Neural Injuries in Cranial Surgery: Case Report.

Intraoperative neurophysiological monitoring (IONM) is instrumental in mitigating neurological deficits following cranial and spinal procedures. Despite extensive research on IONM's ability to recognize limb-malposition-related issues, less attention has been given to other secondary neural injuries in cranial surgeries. A comprehensive multimodal neuromonitoring approach was employed during a left frontal craniotomy for tumor resection. The electronic medical record was reviewed in detail in order to describe the patient's clinical course. The patient, a 46-year-old female, underwent cranioto…

Facial Nerve EMG: Low-Tech Monitoring with a Stopwatch.

OBJECTIVE: The quantity of A-trains, a high-frequency pattern of free-running facial nerve electromyography, is correlated with the risk for postoperative high-grade facial nerve paresis. This correlation has been confirmed by automated analysis with dedicated algorithms and by visual offline analysis but not by audiovisual real-time analysis. METHODS: An investigator was presented with 29 complete data sets measured during actual surgeries in real time and without breaks in a random order. Data were presented either strictly via loudspeaker (audio) or simultaneously by loudspeaker and comput

Transvenous Onyx Embolization of a Type D Carotid-Cavernous-Fistula: Operative Video.

Carotid-cavernous fistulas (CCFs) are acquired pathologic shunts between the carotid circulation and the cavernous sinus that result in venous congestion.1 They often present with ocular symptoms, such as chemosis, proptosis, and blurry vision. Cranial nerve deficits and increased intraocular pressure are often seen on the neuro-ophthalmologic examination.2 If left untreated, they can lead to cortical venous reflux and intracranial hemorrhage. A cerebral angiogram is the gold standard to diagnose these lesions. The hallmark of dural CCF is opacification of venous structures in the arterial pha
