The value of neurophysiologic approaches in the anticipation and evaluation of neonatal hypoglycemia.

The association of low blood glucose with central nervous system (CNS) injury was first described in 1937 by Hartmann and Jaudon. In the early 60 years since publication of these observations the effects of hypoglycemia upon the brain remain poorly understood. Technology capable of accurately determining plasma glucose concentrations has been developed. Investigators have sought to establish critical values below which glucose levels should not be allowed to fall. Despite these efforts the definitive level of glucose capable of producing brain injury in any particular patient remains unknown.

Mechanisms of low back pain: a neurophysiologic and neuroanatomic study.

Idiopathic low back pain has confounded health care practitioners for decades. The cellular and neural mechanisms that lead to facet pain, discogenic pain, and sciatica are not well understood. To help elucidate these mechanisms, anesthetized New Zealand white rabbits were used in a series of neurophysiologic and neuroanatomic studies. These studies showed the following evidence in support of facet pain: an extensive distribution of small nerve fibers and endings in the lumbar facet joint, nerves containing substance P, high threshold mechanoreceptors in the facet joint capsule, and sensitizat

The neurophysiological basis and the role of inflammatory reactions in dentine hypersensitivity.

Recent studies indicate that intradental A-type nerve fibres are responsible for the sensitivity of dentine and are activated by fluid movements in dentinal tubules (hydrodynamic mechanism). The patency of the tubules affects dentine sensitivity to a great extent. Both A delta- and A beta-type nerve fibres respond to dentinal (hydrodynamic) stimulation in a similar way. Only a few studies have been made on the regional sensitivity of dentine or the receptive areas of intradental nerve fibres. The results indicate that the fibres innervating different parts of coronal dentine are equally sensit

Neurophysiological assessment of peripheral neuropathy in primary Sjögren's syndrome.

Peripheral nervous system complications are rare in patients with primary Sjögren's syndrome. We investigated a group of six women aged 43-64 years who complained of pain and sensory symptoms. Conventional neurophysiological tests reflecting large nerve fiber function revealed normal motor conduction in all patients, whereas sensory nerve action potentials were absent in two. On the other hand, quantitative thermometry and autonomic nerve function tests indicating small nerve fiber function were more sensitive in the assessment of nerve dysfunction; these showed abnormalities in all cases. [Clinical, neurophysiologic and biopsy findings in neurotoxic amiodarone syndrome].

PATIENTS: Three patients complained of proximal weakness and paraesthesia of the legs and difficulties in walking during amiodarone treatment. Examination showed signs of a predominantly distal sensory neuropathy, a proximal myopathy, and a cerebellar gait disorder. All had amiodarone dosages of 600 mg per day, amiodarone serum levels above 2.7 mg/l and a total amount of amiodarone ingestion of 300 to 500 g. The clinical symptoms subsided within six to eleven months after treatment was stopped. RESULTS: The neurophysiologic investigations showed slowing of sensory and motor nerve conduction v

Neurophysiological basis of functional recovery in the neonatal spinalized rat.

It had been shown previously that, following spinalization of the neonatal rat on postnatal day 7 (PN7), at the middle thoracic level, there was spontaneous recovery of coordinated stepping in the hindlimbs, enabling the animal to execute quadrupedal locomotion, with an ataxic gait. No significant recovery occurred in rats that were similarly spinalized on PN14. Despite the functional recovery in the PN7 group, their hindlimbs were paralyzed when not in contact with a surface. In the present experiments, at 16-18 weeks after spinalization, muscle spindle GpIa and cutaneous afferents were teste

[The central neurophysiological and neurochemical mechanisms of vomiting (a review of the literature)].

The review contains the present-day evidence on neurophysiological and neurochemical mechanisms of functioning the central components of vomiting reflex. The main attention is concentrated on the role of some brain structures in motion sickness-induced vomiting. At the same time, the literature data casting doubt on traditional view of a necessary involvement of some brain structures in the genesis of vomiting induced by motion sickness are discussed. Also there are findings of studying an effect of physiologically active substances (classical neuromediators and regulatory peptides) at a neuro

Myoclonic epilepsy in an HIV positive patient: neurophysiological findings.

An HIV-positive patient is described who presented with action myoclonus. Unusually, focal sinusoidal EEG burst complexes with a frequency of 40 and 55 Hz could be recorded over the sensorimotor cortex. Activity having a similar frequency was evoked in this area by posterior tibial nerve stimulation in the affected leg, and jerk-locked averaging showed that myoclonic jerks were preceded by similar cortical sinusoidal waves.

Neuroradiological, neurophysiological and anatomopathological study of a case of locked-in syndrome.

We report an anatomoclinical case of locked-in syndrome together with the neuroradiological and neurophysiological findings. MRI confirmed its value in the diagnosis of posterior cranial fossa pathology while the neurophysiological work-up (BAEPs, SEPs, MAPs and TEPs) revealed that the lesion may have broader functional implications than may be gauged from the neuroradiological and pathological evidence. Evoked potentials may therefore be of considerable importance in pinpointing brainstem lesions.

Neurophysiological effects of recombinant interferon-gamma and -alpha in man.

Treatment of malignant tumors with interferon (IFN) is in some patients accompanied by serious neurological side effects. The present study assessed neurophysiological changes in spontaneous EEG activity, visual-evoked cortical potentials (VEPs), and brainstem auditory-evoked potentials (BAEPs) during IFN-gamma or IFN-alpha therapy in 9 patients. In addition, blood pressure, heart rate and body temperature were monitored. In all sessions under IFN, the latency of the P100 component of the VEP was shortened as compared to baseline conditions. IFN also reduced latencies of BAEP components, and d

A neurophysiological study of children with reading, writing and spelling difficulties.

Recordings of early and late auditory evoked potentials and spectral analysis of scalp EEGs of 14 eight- and nine-year-old boys with difficulties in reading, writing and spelling were compared with those of 18 matched controls. The recordings of the poor readers lacked the normal asymmetry of central conduction time and wave amplitude in the early (brainstem) auditory evoked potentials, and late (cortical) evoked potential components were reduced in amplitude. Spontaneous EEG activity was significantly increased in power in all spectral bands, and this activity was less responsive to changes i

Neurophysiological abnormalities in genuine female stress urinary incontinence.

Perineal sensory and motor function was investigated in 28 women with genuine stress incontinence of urine and compared with a matched control group. Electrosensitivity of the dorsal nerve of the clitoris and of the urethral mucosa was significantly diminished in these patients (eight measurements 'insensitive'). Three different reflex latency measurements (dorsal nerve to external anal sphincter, dorsal nerve to urethral sphincter, urethral mucosa to external anal sphincter) were prolonged in incontinence (14 absent reflexes). Mean motor unit potential duration of the external anal sphincter

[Neurophysiologic aspects of the etiology and pathogenesis of congenital hip dislocation].

Till today the meaning of neuromuscular factors in etiology and treatment of congenital dislocation of the hip is not quite clear. Our observation in 62 children with 78 dislocated hips show that neurophysiological mechanisms play of very important role in the development and upkeep of CDH. The neurological examination in 17 children pointed to the typical asymmetries of the children which correlated with the side of the dislocated hip. The knowledge of neurophysiological disturbances on the dislocated hip is of great value for the diagnosis of CDH and even more for the exercise treatment in h

Gabaergic mechanisms and anxiety: an overview and a new neurophysiological model.

GABA is one of the principal inhibitory neurotransmitters in the mammalian brain and an ever increasing wealth of information suggests that GABAergic mechanisms have a special role in the neurophysiology of anxiety. All of the most commonly used antianxiety drugs (the benzodiazepines, the barbiturates, ethanol) selectively enhance only GABA-mediated synaptic transmission. Furthermore, the relative affinities of pharmacologically active benzodiazepines for the benzodiazepine receptor correlate well with their ability to antagonize GABA-modulin (the endogenous inhibitor of GABA receptors) in vit…

A neurophysiological study among Chinese CS2-exposed workers.

A neurophysiological study was carried out to examine peripheral neurotoxicity of extremely low levels of CS2, that is, less than 2 ppm (TWA-8hr) among Chinese viscose rayon workers. From the subjects who participated in a 1981 cross-sectional medical survey, 70 male workers exposed to CS2 and age-matched unexposed workers were randomly selected for the present neurophysiological examination. The conduction velocities of motor, sensory and slower motor fibres of the right ulnar nerve were measured using the same methods as those in the study by Seppäläinen (1974). Skin temperature was measured

Neurophysiological changes following traumatic spinal lesions in man.

Neurophysiological observations were made on normal subjects and on 57 patients who had had injuries to the spinal cord. The amplitude of the muscle compound action potential (M response) recorded from triceps surae in response to supramaximal stimulation of the tibial nerve was reduced in the patients indicating that there are changes in motor units below the level of a spinal lesion in man. In the patients who were clinically spastic it was found that: (1) The proportion of the triceps surae motoneuron pool reflexly activated either by tapping the Achilles tendon or by stimulating the tibial
