BACKGROUND: Brain tumor surgery near or within eloquent regions is increasingly common and is associated with a high risk of neurologic injury. Awake craniotomy with mapping has been shown to be a valid method to preserve neurologic function and increase the extent of resection. However, the technique used varies greatly among centers. Endoscopic skull-base surgery (ESBS) is a rapidly growing surgical area that involves collaboration of otolaryngology-head and neck surgeons and neurosurgeons. Various tumor pathologies and extents have been successfully treated with ESBS, and diverse reconstruction methods have been adopted since its introduction. The optimal reconstructive strategy should be based on heterogeneous surgical situations and tumor extent. Nevertheless, there are few current guidelines for selecting reconstructive methods. Therefore, we review diverse options for endoscopic skull-base reconstruction.

Patients with neurological illnesses often require prolonged enteral or parenteral nutrition. The complications of this type of feeding may be unfamiliar to neurosurgeons and neurologists, however, these complications may be an important source of ongoing morbidity. We present a case of severe anemia due to copper deficiency in a patient requiring enteral nutrition after SAH. The anemia resolved with copper supplements. A high index of suspicion is needed to recognise and treat complications of enteral nutrition.

In the treatment of Menière's disease, surgical procedures are available when conservative measures have failed. The most distressing symptom of vertigo can frequently be arrested, tinnitus reduced and hearing improved. Sudden deafness due to round window rupture is treatable. The most important disorder with labyrinthe symptoms is the acoustic neuroma, the smaller ones of which can be operated on by otologists in collaboration with neurosurgeons using the transtemporal or translabyrinthine approach with preservation of the facial nerve.

Craniopagus is a rare and intriguing condition with an incidence of one in 2.5 million births. The chance of a neurosurgeon seeing a case in a working lifetime is unlikely. The chances of two cases from the same community within 12 months are remote in the extreme. The authors present a second case of craniopagus born and separated in Brisbane, Australia, in 2001 and discuss the intricacies of surgical separation and the lessons learned.

Knowledge of each differential diagnosis of prosopalgia is important to any neurosurgeon who treats facial pain. Pain control is possible with treatment specific to the diagnosis, including those forms of facial pain known to be the most difficult to treat. An outline for the management of atypical facial pain, anesthesia dolorosa, and postherpetic neuralgia is presented with a review of the correlative anatomy for each surgical procedure.

Intraoperative sonography is a useful tool for the neurosurgeon to achieve rapid orientation. This is of particular importance, since planning of an operation is usually based on standard cuts obtained via CT or MR, whereas surgical approach is governed by the prevailing anatomico-topographical conditions. Stereotactic radiotherapy and radiosurgery allow delivery of high irradiation doses in a limited volume. These techniques are specially adapted to brain and nervous pathologies. Indication are not only cancers and tumors but also non tumor tissues such as arteriovenous malformations. In some case purpose of stereotactic radiotherapy is solely functional, for example for trigeminal neuralgia. We detail the questions that raise treatment of these non-tumor pathologies. These pathologies imply a multidisciplinary approach that associate radiation oncologists, neuro-radiologist and neurosurgeons.
