Breastfeeding and delivery room neonatal collapse.

Sudden unexpected neonatal collapse in the delivery room is a rare occurrence in healthy term infants. Upper airway obstruction may occur from improper positioning of the newborn even while breastfeeding. Such occlusion may have dire consequences if not recognized immediately. We report 2 healthy term neonates who suffered respiratory arrest while in the mother's arms and attempting breastfeeding. In each case, rapid response by the delivery room nurse averted tragedy. Metabolic and infectious evaluations were unremarkable. Both babies have been well on subsequent examinations. We conclude tha

Human neonatal infections with hookworms in an endemic area of Southern Nigeria. A possible transmammary route.

Examination of faecal smears of 316 neonates (4-5 weeks old) in a human hookworm endemic area of southern Nigeria showed that 33 (10%) of them were hookworm infected. The majority of the neonatal infections (88%) were due to Ancylostoma duodenale although Necator americanus was the more prevalent hookworm species in the area. The incidence of neonatal infections was pronounced (64%) for mothers who were A. duodenale-positive 5-6 weeks postpartum. A thorough screening of 12.4 litres of colostral milk from all the 316 nursing mothers failed to show the presence of infective hookworm larvae. Thes

Transcutaneous bilirubin levels in newborns <35 weeks' gestation.

OBJECTIVE: In infants <35 weeks' gestation, we sought to define the transcutaneous bilirubin (TcB) levels at which a total serum bilirubin (TSB) level suggesting the need for phototherapy is unlikely to occur and a TSB measurement can, therefore, be avoided. STUDY DESIGN: Nursing staff performed 896 TcB measurements within 1 h of a TSB on 225 neonates 26 0/7-34 6/7 weeks' postmenstrual age (PMA). Generalized linear models were fit with generalized estimating equations (GEEs) to model the probability of having a TSB level at or above the phototherapy initiation cutpoint as a function of the Tc

The Validity and Clinical Utility of the COVERS Scale and Pain Assessment Tool for Assessing Pain in Neonates Admitted to an Intensive Care Unit.

OBJECTIVES: Infants admitted to a neonatal intensive care unit (NICU) are routinely subject to a range of painful procedures. However, pain assessments in NICUs are under-utilized due to a lack of a gold standard pain measure. In this study we assessed the psychometric properties and clinical utility of the COVERS and Pain Assessment Tool (PAT), in a neonatal unit. METHODS: We had 72 nurses use the scales to assess pain at baseline and during a heel-lance procedure in 80 NICU infants. An independent research observer and the infant's mother also completed pain ratings. After the study, we ass

A pilot study to assess the safety, efficacy and ease of use of a novel hearing protection device for hospitalized neonates.

BACKGROUND: Loud noises in the neonatal intensive care unit (NICU) exacerbate patient cardiac and respiratory activity, disrupt sleep, and may contribute to hearing deficits, speech and language disorders, and neurodevelopmental delays among NICU graduates. AIMS: This study evaluated infant-patient tolerance and nurse ease of use of a novel frequency-selective hearing protection device, DREAMIES (NEATCap Medical, LLC). STUDY DESIGN AND SUBJECTS: Fifty neonates receiving care in a Level III NICU participated in a 2-phase prospective study. In Phase 1, 25 infants (mean 36.6 wks GA) wore DREAMI

Effects of three different auditory interventions on physiological and behavioral responses to neonatal heel lance.

BACKGROUND: Neonatal heel lance procedures cause significant distress, requiring effective, non-pharmacological pain management strategies. This study aimed to determine the effects of white noise, classical music and mother-sung lullabies on the heel lance process, by comparing them with the control group (gentle voice). METHODS: A single-blind, four-arm, parallel randomized controlled trial (NCT06167551) was conducted in a hospital NICU between November 2023 and June 2024. A total of 100 newborns were randomly assigned to one of four groups: white noise, classical music, mother-sung lullabi

Trust in medical professionals and its influence on the stress experience of parents of premature infants.

AIM: To examine parents' perceptions of stress and their trust in physicians and nursing staff and to investigate whether trust influences the parental perceptions of potential stressors resulting from their infant's hospitalisation in a neonatal intensive care unit. METHODS: Parents of very and extremely low birth weight infants were surveyed in a nationwide retrospective cross-sectional study 6-18 months after their child's birth. Parental stress was measured utilising the PSS:NICU_German/2-scales, and trust was measured by the scales Trust in Physicians and Trust in Nursing Staff. In addit

[The mother and her sick newborn (author's transl)].

The care of the mother during the pre- and postnatal period is of great importance not only for the act of birth but also for the establishment of the mother child relationship. There are indications that the quality of mothering during later childhood is determined by the way the mother child relations are established during the first days of life. It is important to realise the needs of the mother to see and to touch her child and to enable her to do so as often as possible even if this child is under special care. In cases of neonatal illness the mother especially needs the psychological he

Clinical Nurse Specialist Practice Strategies for Children With Medical Complexity.

PURPOSE: Clinical nurse specialists are advanced practice nurses that influence patient care and nursing clinical practice through expert consultation and ongoing support. They play a vital role in caring for children with medical complexity. The purpose of this project is to highlight the practice of the clinical nurse specialist in identifying complex patient care needs and describe the resources needed to successfully transition a patient through the continuum of care by using a case example of a neonate with epidermolysis bullosa. DESCRIPTION OF THE PROJECT: Caring for children with medic

The Iowa hospital visitation program: does outreach education affect management of neonatal resuscitation?

Thirty-nine Iowa hospitals were visited by a perinatal education team to improve management of neonatal resuscitation. Patient care was evaluated by reviewing medical records according to pre-established performance standards. Following patient-care review, educational programs were offered to physicians and nurses. The hospitals were first visited from July 1982 until June 1984. Follow-up visits were made during the next two years. Management of neonates born in meconium was initially inadequate, but improved during the second period. Similarly, the conduct of other neonatal care practices sh

Simulation-Based Summative Assessment of Neonatal Resuscitation Providers Using the RETAIN Serious Board Game-A Pilot Study.

Background: Each year, 13-26 million newborn babies require help to breathe at birth. Healthcare professionals (HCPs) who provide neonatal resuscitative care must be frequently evaluated to maintain and improve the quality of healthcare delivered. While simulation-based competence assessment is preferred, resource constraints hinder uptake. We aimed to examine if the RETAIN simulation-based boardgame can be used to assess HCPs' neonatal resuscitation knowledge. Method: Twenty neonatal HCPs (19 females) from the Royal Alexandra Hospital (Edmonton, Canada) were recruited. First, they completed a

Maternal death from obstetric hemorrhage.

Obstetric hemorrhage remains the leading cause of maternal death in the United States, and 54% to 93% of these deaths may have been preventable. Leaders must honor the lives of women who die from obstetric hemorrhage by reviewing their deaths and sharing lessons learned. Shortening the current 3 to 7 year data gap will allow for timely initiation of quality improvement efforts. Designated leaders and researchers from the Association of Women's Health, Obstetric, and Neonatal Nurses are ideally positioned to lead these quality initiatives.

Brain injury in the infant: the old, the new, and the uncertain.

Although neonatal brain injury occurs most frequently after a perinatal hypoxic-ischemic insult, recently studies have noted that variable causes such as metabolic and reperfusion events can result in, or aggravate, a brain insult. Current data suggest that about 2 to 5 of 1,000 live births in the United States and more so in developing countries experience a brain injury Approximately 20% to 40% of infants who survive the brain injury develop significant neurological and developmental impairments. The resulting impact on the child, family, and society presents a formidable challenge to health

Ophthalmia Neonatorum: To Treat or Not to Treat?

Neonatal conjunctivitis is a common condition that can develop and progress at any stage of infancy, depending on the causative agent, including in otherwise healthy term infants outside of the NICU. Despite its frequency, there is no single, readily accessible reference that concisely integrates causative pathogens, presentation timing, and evidence-based treatment strategies. Given the multiple etiologies, each with distinct pathophysiology and management considerations, accurate identification is essential to prevent complications. This article highlights the most common causes of neonatal

The Ins and Outs of the Complete Blood Count With Differential: Back to Basics.

A complete blood count (CBC) with differential is an essential laboratory test used to evaluate overall physiologic health and detect potential disorders in neonates. This test provides valuable insights into various blood cell types and structures and offers information on infections, immune status, anemia, and other hematologic conditions. Given the unique physiology and susceptibility of neonates to infections and other complications, interpreting the CBC with differential results in this population requires a specialized approach. The purpose of this article is to describe the components o

Variation in Neonatal Inpatient Charges at the State and Local Level.

In an analysis of all Ohio newborn infants discharged home alive between 2007 and 2012, the authors identified that significant variation in hospital charges (among Medicare Severity Diagnostic Related Group categorizations), previously identified nationally, persists at the state and local levels among term and preterm infants (p <.0001). Additionally, the authors identified variation in length of stay among infants with extreme immaturity or respiratory distress syndrome (p <.0001). Charge data remain the best available proxy for closely guarded hospital cost figures; increased pricing trans
