Acute hyperglycemia during hospitalization, nutritional support, and longer-term cardiovascular outcomes: a nationwide real-world data cohort study.

BACKGROUND: Acute hyperglycemia is not only a marker of acute illness severity but may also contribute to worse patient outcomes. However, the longer-term association between acute hyperglycemia and cardiovascular (CV) outcomes, particularly in the context of nutritional support influencing the relation, remains unexplained. OBJECTIVES: To evaluate the long-term association between acute hyperglycemia during hospitalization and incident CV events after discharge, and to examine whether inpatient nutritional support modifies this risk. METHODS: This retrospective cohort study used United Stat

Bile Acid Profiling Reveals Distinct Signatures in Undernourished Children with Environmental Enteric Dysfunction.

BACKGROUND: Intestinal inflammation and malabsorption in environmental enteric dysfunction (EED) are associated with early childhood growth faltering in impoverished settings worldwide. OBJECTIVES: The goal of this study was to identify candidate biomarkers associated with inflammation, EED histology, and as predictors of later growth outcomes by focusing on the liver-gut axis by investigating the bile acid metabolome. METHODS: Undernourished rural Pakistani infants (n = 365) with weight-for-height Z score (WHZ) < -2 were followed up to the age of 24 mo and monitored for growth, infections,

Situational analysis of infant and young child nutrition policies and programmatic activities in the Islamic Republic of Mauritania.

Progress towards reducing mortality and malnutrition among children <5 years of age has been less than needed to achieve related Millennium Development Goals (MDGs), so several international agencies joined to 'reposition children's right to adequate nutrition in the Sahel', starting with an analysis of current activities related to infant and young child nutrition (IYCN). The main objectives of the situational analysis are to compile, analyse, and interpret available information on infant and child feeding, and the nutrition and health situation of children <2 years of age in Mauritania as on

Comprehensive Assessment of Nutrition and Dietary Influences in Hypermobile Ehlers-Danlos Syndrome-A Cross-Sectional Study.

INTRODUCTION: Disorders of gut-brain interaction (DGBI) are common in patients with hypermobile Ehlers-Danlos syndrome/hypermobility spectrum disorder (hEDS/HSD). Food is a known trigger for DGBI symptoms, which often leads to dietary alterations and, increasingly, nutrition support. We aimed to explore dietary behaviors and influencing factors in patients with hEDS/HSD. METHODS: In a cross-sectional study, patients with hEDS/HSD were recruited from Ehlers-Danlos Support UK (nontertiary) and tertiary neurogastroenterology clinics to complete questionnaires characterizing the following: dietar

The consequences of ultra-processed foods on brain development during prenatal, adolescent and adult stages.

Ultra-processed foods (UPFs) are highly refined formulations of non-nutritive compounds containing elevated amounts of sugar, fat, sodium, food additives, and dietary emulsifiers. Consumption of UPF is robustly linked to a range of non-communicable diseases, including obesity, type 2 diabetes, cardiovascular disease, and mental disorders in adults. While substantial research highlights the negative health impacts of UPFs in adulthood, their effects on brain development during critical periods of biological vulnerability, pregnancy, childhood, and adolescence, remain underexplored. During pregn

Conference on "Multidisciplinary approaches to nutritional problems". Rank Prize Lecture. Global nutrition challenges for optimal health and well-being.

Optimal health and well-being are now considered the true measures of human development. Integrated strategies for infant, child and adult nutrition are required that take a life-course perspective to achieve life-long health. The major nutrition challenges faced today include: (a) addressing the pending burden of undernutrition (low birth weight, severe wasting, stunting and Zn, retinol, Fe, iodine and folic acid deficits) affecting those individuals living in conditions of poverty and deprivation; (b) preventing nutrition-related chronic diseases (obesity, diabetes, CVD, some forms of cancer

[Artificial nutrition in the home. Annual information 1996.Group NADYA-SENPE].

Using a simplified questionnaire from the NADYA group, data referring to age, sex, diagnosis, access route, duration, form of administration, complications, and quality of life have been gathered from 1,400 patients (57% male, 43% female) who receive home enteral nutrition, and from 38 patients (20% male and 18% female) who receive home parenteral nutrition. All of these patients come from the 1996 national registry. The most common indication for home enteral nutrition are neoplasias (39%) followed by neurological alterations (33%). The most common access route is oral (48%), followed by a na

Clinical nutrition and the role of hospital pharmacist in the management of covid patient.

The nutritional status of everyone represents a fundamental element to maintain a good health and it can be related to infectious agents in some disorders. Prevention, diagnosis and treatment of malnutrition should be included in the management of SARS-CoV-2 patients in order to improve both short- and long-term prognosis. In Covid patients the choice of route of administration for nutrition is closely related to respiratory autonomy. In subjects who are not mechanically ventilated or with non-invasive ventilation (NIV), spontaneous oral feeding is strongly indicated, while considering the pat

Nurse supervised combined refeeding and home parenteral nutrition in traumatic intestinal failure: A case series.

BACKGROUND: Intestinal failure is a decrease in intestinal function under the minimum absorption requirements of macronutrients, water, and electrolytes. Hartman's procedure with jejunostomy is used as a surgical procedure to prevent further damage in cases of abdominal trauma. Providing parenteral nutrition at home is needed to prevent nutritional deficiencies and prolonged length of stay. PRESENTATION OF CASE: We reported two cases, involving two men aged 25 and 14 years old who had peritonitis due to abdominal trauma and received laparotomy. Both patients had Hartman's procedures and jejun

[Treatment of intestinal failure by total parenteral nutrition at home in children and adults].

Intestinal failure is characterised by inability of the intestine to absorb sufficient nutrients to maintain the integrity and function of the body. This can be caused by malabsorption due to too short an intestine or an abnormality of the mucosa, or by a severe motility disorder. In addition to dietary measures, the prescription of total parental nutrition (TPN) at home is sometimes necessary. This treatment is a burden on the patient and the risk of complications must be reduced to a minimum. The risks of long-term parenteral nutrition can be limited and the quality of the provision of servi

Nutritional support of the elderly patient in an intensive care unit.

Critically ill elderly patients are at high risk to develop protein-energy malnutrition as well as micronutrient deficiencies. They have characteristic metabolic alterations which must be understood in order to provide nutritional support. Current nutritional status can be assessed by clinical and laboratory parameters. The enteral and parenteral routes of administering nutrition and their advantages, disadvantages, complications, and monitoring are discussed.

Ornithine alpha-ketoglutarate in nutritional support.

Ornithine alpha-ketoglutarate (OKG) is a salt formed of two molecules of ornithine and one molecule of alpha-ketoglutarate. OKG has been successfully used by the enteral and parenteral route in burn, traumatized, and surgical patients and in chronically malnourished subjects. According to the metabolic situation, OKG treatment decreases muscle protein catabolism and/or increases synthesis. In addition, OKG promotes wound healing. The mechanism of action of OKG is not fully understood, but the secretion of anabolic hormones (insulin, human growth hormone) and the synthesis of metabolites (gluta

Nutrition of the elderly.

The progressively increasing number of elderly people in the Canadian population and the disproportionate expenditure on their health care has stimulated interest in prevention of common illnesses observed in this age group. It is now recognized that nutrition plays an important role in health status, and both undernutrition and overnutrition are associated with greater risk of morbidity and mortality. Nutritional problems in the elderly can be suspected if there are several high-risk factors present--for example, living alone, physical or mental disability, recent loss of spouse or friend, we

Nutritional needs of cancer patients.

Individuals with cancer share many similar nutritional problems, regardless of the primary site of the malignant neoplasm. The presence of protein-calorie malnutrition is the most common secondary diagnosis in cancer and is frequently the cause of death. To date, the inability of health care professionals to halt or adequately control the severity of protein-calorie malnutrition in the individual with cancer indicates that the syndrome is more complex than inadequate ingestion of nutrients. The presence and severity of protein-calorie malnutrition in individuals with cancer cannot be attribute

Biomarkers identified in inborn errors for lysine, arginine, and ornithine.

Inborn errors of lysine, arginine, and ornithine metabolism are very rare: only a few patients affected with these disorders have been carefully investigated, and very few reports on long-term outcome are available. These rare data make it difficult to define safety limits of these amino acids and useful biomarkers from these disorders. Only 4 disorders give rise to an important increase of the plasma amino acid concentration proximal to the metabolic block: lysine in 2-aminoadipic semialdehyde synthase deficiency, arginine in arginase deficiency, ornithine in ornithine amino transferase defic

Complications associated with parenteral nutrition in hospitalized patients.

OBJECTIVE: Malnutrition is common in hospitalized patients. Parenteral nutrition is a therapeutic alternative to meet the nutritional requirements of patients in whom the gastrointestinal tract is unavailable or non-functional. This therapy has been associated with a wide range of infectious, mechanical and metabolic complications. The aim of the study was to assess the incidence of these complications. METHOD: A prospective multicenter observational study in hospitalized adult patients receiving parenteral nutrition ≥72 h was conducted. Follow-up was performed until one week after the discon
