Vestibular and Ocular Motor Phenotype in a Case of Paraneoplastic Autoimmune Kelch-Like Protein-11 Encephalitis.

OBJECTIVES: KLHL11 encephalitis is often associated with vestibular symptoms and ataxia. However, the specific disorders of the vestibular and oculomotor systems have only been partially investigated so far. We present a case of paraneoplastic KLHL11 encephalitis in which vestibulo-oculomotor function was quantitatively assessed. METHODS: A 1.5T MRI scan of the brain and cervical spinal cord was performed. KLHL11 IgG in serum and CSF was detected using a fixed cell-based assay. Vestibulo-oculomotor function was assessed by videooculography with rotational chair tests. RESULTS: MRI scans of t

Ocular motor dysfunction in patients with multiple system atrophy and Parkinson's disease.

OBJECTIVE: Differentiating multiple system atrophy (MSA) from Parkinson's disease (PD) remains challenging. This study aimed to investigate the utility of specific ocular motor features in distinguishing MSA from PD, as these dysfunctions may offer valuable diagnostic clues. METHODS: We used video-oculography to assess spontaneous nystagmus, saccadic intrusions or oscillations, gaze, saccades, head shaking, positional nystagmus, and vestibulo-ocular reflex (VOR) cancellation in 23 MSA patients and 49 PD patients. RESULTS: Spontaneous downbeat nystagmus, saccadic intrusions/oscillations, and

Internuclear ophthalmoplegia.

BACKGROUND: Polymorphisms in apolipoprotein E have been associated with worse prognoses in numerous neurodegenerative conditions, including HIV dementia (HIVD). Despite these correlative observations, there has been little evidence suggesting a mechanism whereby the expression of ApoE4 renders neurons susceptible to insult. METHODS: Electrospray ionization tandem mass spectrometry was used to quantify levels of sphingolipids and sterols in brains of HIVD patients. Data were stratified according to APOE genotype. RESULTS: The authors found evidence of dysregulated lipid and sterol metabolism

Cogan's syndrome associated with orbital inflammation.

Cogan's syndrome is a relatively rare inflammatory disorder presenting with a combination of ocular and vestibuloauditory symptoms. The first cases were described by David Cogan in 1945. Typically the ocular signs involve the anterior segment; however there are no descriptions of orbital inflammation associated with Cogan's syndrome. We describe such a case. A 66-year-old immunosuppressed male patient with known Cogan's syndrome presented to the eye department with right-sided proptosis, chemosis and ocular injection. Ocular motility was reduced. Other than Cogan's syndrome there were no other

The pattern of extra-ocular muscle involvement in ocular myasthenia.

BACKGROUND: The ocular motility (OM) deficit in myasthenia has not been studied systematically. Anecdotal reports yield no consensus, with the pattern mimicking infranuclear, internuclear or supranuclear disorders. The current study defines the pattern of extra-ocular muscle (EOM) weakness at presentation for a group of newly diagnosed untreated ocular myasthenics (OMG). METHODS: Cases referred to one consultant neurologist (RAM) between 1991 and 2001 were reviewed. EOM weaknesses were quantified using an established scoring method in 49 patients with OMG and 49 age- and sex-matched controls,

Convergence Insufficiency Symptom Survey scores for required reading versus leisure reading in school-age children.

PURPOSE: To compare Convergence Insufficiency Symptom Survey scores for required reading for school versus scores for leisure reading. METHODS: At a single private practice, we surveyed 100 children with normal binocular vision using modified survey questions specifying either required reading or leisure reading as the near visual activity. Average total symptom scores and subscores for questions about discomfort, fatigue, impaired vision, and cognitive performance were analyzed using t tests; responses to individual questions were analyzed using Mann-Whitney U tests. RESULTS: The average sc

The biomechanical significance of pulley on binocular vision.

BACKGROUND: Pulleys have been reported as the functional origins of the rectus extraocular muscles (EOMs). However, biomechanical significance of pulleys on binocular vision has not been reported. METHODS: Three eye movement models, i.e., non-pulley model, passive-pulley model, and active-pulley model, are used to simulate the horizontal movement of the eyes from the primary position to the left direction in the range of 1°-30°. The resultant forces of six EOMs along both orthogonal directions (i.e., the x-axis and y-axis defined in this paper) in the horizontal plane are calculated using the

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) - a case report and review of literature.

CANVAS (cerebellar ataxia with neuropathy and vestibular areflexia syndrome) is a rare neurological syndrome of unknown etiology. The main clinical features include bilateral vestibulopathy, cerebellar ataxia and sensory neuropathy. An abnormal visually enhanced vestibulo-ocular reflex is the hallmark of the disease. We present a case of 58-year-old male patient who has demonstrated gait disturbance, imbalance and paresthesia of feet for 2 years. On examination ataxia of gait, diminished knee and ankle reflexes, absence of plantar reflexes, fasciculations of thigh muscles, gaze-evoked downbeat

Investigating unstable fixation in patients with macular disease.

PURPOSE: To assess the effect on visual acuity of compensating fixation instability by controlling retinal image motion in people with macular disease. METHODS: Ten patients with macular disease participated in this study. Crowded and noncrowded visual acuity were measured using an eye tracking system to compensate for fixation instability. Four conditions, corresponding to four levels of retinal image motion, were tested: no compensation (normal motion), partial compensation (reduced motion), total compensation (no motion), and overcompensation (increased motion). Fixation stability and the

A new measure of dysconjugacy in INO: the first-pass amplitude.

BACKGROUND: The ratios of abducting to adducting eye movements (versional dysconjugacy index, VDI) for saccadic velocity and acceleration have been useful measures by which to objectively characterize internuclear ophthalmoparesis (INO). Amplitude measures of dysconjugacy have been less useful, given that many patients maintain the ability to ultimately reach a centrifugal fixation target and that traditional amplitude measures of VDI have focused on this 'final amplitude' (FA) position. METHODS: We utilized infrared oculography to define a new amplitude measure of dysconjugacy in 42 multiple

Paraneoplastic Syndromes in Neuro-ophthalmology.

PURPOSE OF REVIEW: This article discusses the varied types of paraneoplastic syndromes that commonly have neuro-ophthalmologic manifestations. Diagnostic considerations and therapeutic options for individual diseases are also discussed. RECENT FINDINGS: Paraneoplastic syndromes can affect the afferent and efferent visual systems. Paraneoplastic syndromes may result in reduced visual acuity from retinal degeneration, alterations in melanocyte proliferation and uveal thickening, or acquired nystagmus. Ocular motor abnormalities related to paraneoplastic syndromes may present with symptoms from

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

Isolated oculomotor dysfunction with downbeat nystagmus as a rare presentation of anti-GAD autoimmunity.

BACKGROUND: Glutamic acid decarboxylase (GAD) antibodies are associated with a range of autoimmune neurological syndromes, including cerebellar ataxia (CA). Ocular motor disturbances such as nystagmus and diplopia are frequently observed in CA but rarely are seen as isolated manifestation. CASE PRESENTATION: We describe the case of a 60-year-old woman who presented with oculomotor dysfunction consisting of persistent diplopia and vertical nystagmus, in the absence of any other neurological manifestation. Brain MRI and PET-CT scans were unremarkable. Videonystagmography documented downbeat nys

Canine tooth syndrome after frontoethmoidal osteoma surgery: a case report.

Introduction: Canine tooth syndrome is a rare condition defined by the simultaneous presence of superior oblique palsy and Brown syndrome, resulting from pathological changes in the trochlear region. This syndrome can develop through various mechanisms, including dog bites, head trauma, infections, inflammation, and scarring. This report highlights its occurrence following sinus surgery for the first time. Methods: Medical and surgical records of an 18-year-old girl who presented with reading position diplopia after sinus surgery for right-side frontoethmoidal osteoma were retrospectively revi

Bilateral adduction palsy with a paramedian midbrain lesion is most likely wall-eyed bilateral oculomotor subnuclear ophthalmoplegia: a case report of a brainstem infarction.

A 73-year-old female was hospitalized in our Department of Neurology for diplopia due to multiple infarctions in the midbrain and pons. A neurological examination revealed bilateral adduction palsy during horizontal gaze with nystagmus of the contralateral abducting eye. Exotropia and impaired convergence were also noticed. The distinctive symptoms indicated wall-eyed bilateral internuclear ophthalmoplegia (WEBINO). We speculate that the cause of WEBINO in our case was bilateral medial rectus palsy via bilateral damage to the medial rectus subnuclei (MRSN) due to the paramedian midbrain infarc
