INTRODUCTION: Optic nerve head drusen are congenital calcium deposits located in the prelaminar section of the optic nerve head. Their association with visual field defects has been classically described, but the diagnosis of glaucoma is not easy in these cases of altered optic nerve head anatomy.

Three observers, unaware of the clinical status of the patients and using projected 35-mm color transparencies, independently graded the degree of peripapillary halo and atrophy in 240 eyes. Analysis of the results revealed that, although patients with glaucoma had more prominent (4+) halos, the average degree of halo was statistically identical for both the glaucomatous and normal eyes. Peripapillary atrophy, on the other hand, was significantly (P less than .01) more extensive in glaucomatous eyes, and 4+ atrophy was seen primarily in eyes with more advanced disease of long duration.

The present paper reports on a case of melanocytoma with extensive visual field defects in a left eye. Fluorescein angiography, sonography, and computerized tomography showed the tumor to be located in the prelaminar part of the optic disk. There was no sign of infiltrative growth. There has been no change in the findings in the 9 months since the diagnosis was established.

A 16 year old juvenile diabetic presented with a diabetic papillopathy (D.P.). Visual impairment is known to be minimal in D.P. Our patient presented with all clinical features of diabetic papillopathy (disk oedema, massive leakage). Evolution toward poor visual acuity in one eye, and visual field defect in the other eye is unusual. We discuss relationship between D.P. and anterior ischemic optic neuropathy.
