Visual function after optic neuritis: a preliminary study.

We assessed visual function after recovery from optic neuritis in 15 consecutive patients using five objective tests: colour vision testing with Ishihara colour plates and with D15 colour desaturated spots, Humphrey automated perimetry, contrast acuity testing with Regan letter charts and testing of visual evoked response (VER). Recovery of visual function was not found to be dependent on presenting Snellen visual acuity or treatment with oral steroids. The most sensitive measures of residual visual deficit were mean defect on automated perimetry, low-contrast acuity and VER.

Impaired critical flicker frequency in recovered optic neuritis.

Thirteen patients with optic neuritis who recovered normal visual acuity and kinetic perimetry all showed impairment of foveal critical flicker frequency, a psychophysical test of visual temporal resolution. Because the magnocellular visual system demonstrates higher temporal resolution than the parvocellular system, this finding implicates involvement of axonal projections of magnocellular retinal ganglion cells in recovered optic neuritis.

Evolution of visual evoked potentials in optic neuritis.

The visual evoked potential (VEP) latency was either abnormally prolonged or absent in the involved eye of 47 patients with optic neuritis. Twenty-two of these patients with known multiple sclerosis (MS), had similar abnormalities to 25 patients with no clinical evidence of MS. Follow-up clinical assessment and VEP were done 10 to 42 (mean 22) months later in 34 patients. In 15 of 34 patients with no VEP from the involved eye during initial examination, 6 returned to normal, 8 had prolonged latencies and 1 still had no response at follow up. Of 19 patients who initially had prolonged latencies…

Bilateral optic neuritis. A long-term follow-up.

We have endeavoured to determine the fate of the adults and children presenting with bilateral optic neuritis who were reported by Hierons and Lyle (1959) and Meadows (1969). We have follow-up information from a variety of sources on 31 of the 34 adults for up to thirty-seven years, and on 17 of 19 children after up to thirty-two years. Three adult cases were excluded after review. The visual prognosis was variable in the adults but excellent in the children. None of the children has developed clinically definite multiple sclerosis. Two of the 11 adults with simultaneous bilateral optic neurit…

Temporal contrast sensitivity in optic neuritis and amblyopia.

Temporal contrast sensitivity was measured in patients with optic neuritis and anisometropic amblyopia. In optic neuritis, attenuated sensitivity was detected at all frequency ranges; however, its degree was more severe than that of retinal diseases even when visual acuity was the same. At the convalescent stage, the low-frequency sensitivity was restored first, and high-frequency loss was observed even when the visual acuity was restored to 1.0. Two amblyopic patients showed attenuated sensitivity at high- and intermediate-frequency ranges, and in one case all frequency sensitivity attenuated…

Parainfectious optic neuritis. Report and review following varicella.

Severe loss of vision and bilateral optic disc edema occurred in a 10-year-old girl 38 days after the rash of varicella first appeared and subsequent encephalitis developed. Recovery of visual function was nearly complete, but optic atrophy persisted. Parainfectious optic neuritis following other viral infections, with the exception of herpes zoster, has a similar clinical profile: delayed onset, severe visual loss, optic disc edema, bilaterality, good recovery, and residual optic atrophy. Conduction delays in our patient's patterned visual evoked potentials implicated demyelination within the…

Supraliminal contrast sensitivity functions in recovered optic neuritis.

The supraliminal contrast sensitivity functions in patients with recovered optic neuritis were classified into three function types: 1) type with a decrease in sensitivity in the high and an increase in the low spatial frequency region, 2) type with flat shape as found in normal subjects, and 3) type with a decrease in the low and an increase in the high frequency region. These abnormal functions may be explained by an inhibitory interaction between two parallel channels of high and low frequency, which may have a dependence on the most central and the paracentral region in the visual fields, …

The afferent pupillary defect in acute optic neuritis.

Twenty-two patients with acute optic neuritis were studied by the techniques of infrared pupillometry and visual evoked responses (VER) to pattern reversal. A relative afferent pupillary defect was found in all cases and the magnitude of this defect was found to be related to the amplitude, but not to the latency, of the VER. During follow-up the afferent defect was found to remain persistently abnormal while other methods of clinical evaluation could not demonstrate abnormality reliably. The amplitude of the VER also remained low.
