The effects of a posterior superior iliac spine support device on upper trunk acceleration during gait in individuals with flat lumbar back posture.

BACKGROUND: The orientation of the pelvis is of particular importance to the sagittal curve of the spine and has geometric relationships with the top of the spine and lumbar lordosis. Changes in spinal shape or disruptions of sagittal balance in the spine, such as a flattened lumbar spine, have significant negative effects. OBJECTIVE: The aim of this study was to investigate the effects of the posterior superior iliac spine support device (PSD) on upper trunk acceleration during gait in individuals with flat lumbar back posture. METHODS: In total, 10 young male subjects with reduced lumbar l

Design and fuzzy logic control of an active wrist orthosis.

People who perform excessive wrist movements throughout the day because of their professions have a higher risk of developing lateral and medial epicondylitis. If proper precautions are not taken against these diseases, serious consequences such as job loss and early retirement can occur. In this study, the design and control of an active wrist orthosis that is mobile, powerful and lightweight is presented as a means to avoid the occurrence and/or for the treatment of repetitive strain injuries in an effective manner. The device has an electromyography-based control strategy so that the user's

Overgrowth after femoral shaft fractures in infants treated with a Pavlik harness.

This study was performed to determine whether overgrowth occurs when treating infantile femoral shaft fractures with a Pavlik harness. Twenty-three patients, aged less than 1 year, treated with a Pavlik harness for a femoral shaft fracture were included. Shortening at the time of injury was recorded on radiographs and compared with those obtained at the follow-up visit 18 months or later. Average overgrowth was 5 mm (1-18 mm). Overgrowth occurs in infants following treatment of femoral fractures with a Pavlik harness. Initial shortening of up to 18 mm was acceptable and resulted in a leg-lengt

Hand therapy versus corticosteroid injections in the treatment of de Quervain's disease: A systematic review and meta-analysis.

STUDY DESIGN: Systematic review with meta-analysis. INTRODUCTION: Although corticosteroid injections are often cited as best practice in the treatment of de Quervain's disease, no reviews have compared their effectiveness to a multimodal definition of hand therapy. PURPOSE: To compare the effectiveness of corticosteroid injections with that of i) hand therapy alone and ii) combined hand therapy/corticosteroid injection approaches in the treatment of de Quervain's disease. METHODS: Searches of key databases were performed to identify experimental studies published between January 1950 and No

Is intramedullary nailing more effective than non-operative treatment in adults with displaced middle-third clavicle fractures?

BACKGROUND: Clavicle fractures are common, accounting for 5-12 % of all fractures. Traditionally, displaced middle-third clavicle fractures have been managed non-operatively but the associated displacement often leads to mal-union with shortening, cosmetic deformity and occasionally non-union, with clinicians looking towards alternative operative methods such as intramedullary nailing (IMN). However, such methods have their own complications. In order to ascertain the effectiveness of IMN in the management of middle-third clavicle fractures compared with non-operative treatment, analysis of re

The balance control effects on sitting posture induced by lumbosacral orthosis wear vary depending on the level of stability.

This study aimed to assess the differential impacts of lumbosacral orthosis (LO) wear in different sitting conditions through posturographic measurements. Twelve healthy subjects sat on a force platform with three variable stability levels (stable and on seesaws with a long and short radius, inferring slightly and highly unstable sitting, respectively) and three orthosis conditions (no LO, neutral LO, lordotic LO). Using fractional Brownian motion modelling of the centre of pressure (CoP) displacements, it appears that a stable sitting position did not highlight any particular differences betw

Nonsurgical treatment of Legg-Calvé-Perthes disease.

It was first postulated in the case Legg-Calvé-Perthes disease that nonweight bearing and prolonged immobilization were the most effective methods for prevention of the destructive changes in the hip joint. However, later developments adopted joint mobilization using ambulatory orthoses. The containment method altered the main concept of orthotic treatment, based on its better results than the conventional method. The surgical treatment also aims at a containment effect. The choice of either nonsurgical or surgical treatment is still difficult. It should be considered after comprehensively exa

Head shape measurement standards and cranial orthoses in the treatment of infants with deformational plagiocephaly.

This review aims to determine how head shape is measured and describes the use of orthoses in the management of deformational plagiocephaly. A systematic review was conducted and papers published in English up to and including 2006 were sourced from nine databases. After initial screening, 20 papers were included; three literature reviews and 17 original papers. Of the original papers, eight concerned the method of head shape measurement. Measurements are important in determining clinical classification and treatment modality of deformational plagiocephaly. All studies were appraised and assig

Charcot-Marie-tooth disease.

Charcot-Marie-Tooth disease (CMT) or hereditary motor and sensory neuropathy constitutes a genetically heterogeneous group of diseases that affect the peripheral nervous system. CMT is characterized by degeneration or abnormal development of the peripheral nerve and is transmitted with different genetic patterns. In most cases, the disease starts in infancy. Its symptoms, among others, are an awkward gait; muscular atrophy of the 4 extremities, particularly distally; and foot deformities, such as cavus foot. People with CMT have an altered gait; most have a high stepping gait and frequently tr

The "baseball cap orthosis": a simple solution for dropped head syndrome.

Dropped head syndrome (DHS) is a well-recognized condition characterized by gradual sagging of the head. At the extreme, the condition may lead to a "chin on chest deformity" where the chin rests on the chest wall and the patient is unable to look straight ahead. Dropped head syndrome tends to develop in patients with severe weakness of the neck extensors. Various neuromuscular disorders and surgical procedures may compromise the stability of the cervical spine and lead to this disorder. The condition may severely compromise the patient's quality of life and result in significant disability. A

Assessment of plantar pressure in forefoot relief shoes of different designs.

BACKGROUND: After reconstructive forefoot surgery, patients require complete or partial forefoot relief, which can be obtained with a variety of shoe designs. The aim of this study was to evaluate the effectiveness of two different types of forefoot-relief shoes frequently used after surgery, especially their safety against unintentional forefoot load. METHODS: Ten healthy volunteers were asked to perform five trials on a treadmill at self-selected speeds. In the first trial, mean peak pressure values in mass-produced shoes and insoles were evaluated and considered as 100%. Two different shoe

The incidence of avascular necrosis of the healthy, contralateral femoral head at the end of the use of Pavlik harness in unilateral hip dysplasia.

We examined the incidence of avascular necrosis (AVN) of the healthy femoral head in unilateral hip dysplasia at the end of the use of the Pavlik harness. The evaluation of AVN was done with the help of standardised roentgenograms. Between 1974 and 1982, 1,064 dysplastic hips (869 children) were treated with the Pavlik harness at the Orthopaedic Department of Semmelweis University. Of these, 674 children who had unilateral hip dysplasia were chosen for this study. In the period of our investigation, ultrasonography was not yet used routinely, so in some cases the hip was mistakenly diagnosed a
