A dynamic ensemble model for short-term forecasting in pandemic situations.

During the COVID-19 pandemic, many hospitals reached their capacity limits and could no longer guarantee treatment of all patients. At the same time, governments endeavored to take sensible measures to stop the spread of the virus while at the same time trying to keep the economy afloat. Many models extrapolating confirmed cases and hospitalization rate over short periods of time have been proposed, including several ones coming from the field of machine learning. However, the highly dynamic nature of the pandemic with rapidly introduced interventions and new circulating variants imposed non-t

Learning from prepandemic data to forecast viral escape.

Effective pandemic preparedness relies on anticipating viral mutations that are able to evade host immune responses to facilitate vaccine and therapeutic design. However, current strategies for viral evolution prediction are not available early in a pandemic-experimental approaches require host polyclonal antibodies to test against1-16, and existing computational methods draw heavily from current strain prevalence to make reliable predictions of variants of concern17-19. To address this, we developed EVEscape, a generalizable modular framework that combines fitness predictions from a deep lear…

Bad law or implementation flaws? Lessons from the implementation of the new law on epidemics during the response to the first wave of COVID-19 in Switzerland.

After the 2009-2010 H1N1 pandemic, Switzerland overhauled its 1970 law on epidemics. The reform aimed at improving early detection, surveillance, and preparedness for future outbreaks of infectious diseases. Notably, the law introduced stronger coordination between Federal and Cantonal authorities, better management tools and international cooperation. The new law entered into force in 2016 after a long legislative process. During the process, the law survived a referendum fuelled by concerns about vaccine safety and pharmaceutical industry interference. The law was first applied during the CO

Cold-adapted X-31 live attenuated 2009 pandemic H1N1 influenza vaccine elicits protective immune responses in mice and ferrets.

The 2009 pandemic influenza H1N1 (pdmH1N1) is characterized by rapid transmission among humans and disproportionate infection to children and young adults. Although the pdmH1N1 demonstrated less lethality than initially expected and has now moved into its post-pandemic period, it remains highly possible that through antigenic shift or antigenic drift the pdmH1N1 might re-emerge in the future as a more virulent strain than before, underscoring the need for vaccination prior to an outbreak. Using X-31 ca as a backbone strain, we generated a live attenuated pdmH1N1 vaccine and evaluated its poten

Developmental outcomes in infants treated with therapeutic hypothermia before and during the COVID-19 pandemic.

BACKGROUND: Infants with hypoxic ischemic encephalopathy (HIE) are at high-risk for adverse neurodevelopmental outcomes, which may be further impacted by disruptions to social and healthcare systems such as during the COVID-19 pandemic. This study aimed to compare outcomes of infants with HIE born before versus during the pandemic. METHODOLOGY: We retrospectively compared neurodevelopmental outcomes in infants with HIE born pre-pandemic (10/2017-12/2019) versus during the pandemic (03/2020-09/2021), using the Bayley Scales of Infant Development-4 at 12 and 18-24 months. RESULTS: Among 73 inf

Strengthening microbial genomics capacity in Africa for epidemic preparedness: key lessons from the AFROSCREEN programme.

The AFROSCREEN initiative, implemented in 13 countries, has strengthened regional genomic surveillance through investments in sequencing infrastructure, workforce training, and cross-country coordination, evolving from a SARS-CoV-2-focused effort into a multi pathogen platform. By building local capacity, AFROSCREEN reduced dependence on external laboratories, helped close critical surveillance gaps, and generated genomic data that could inform mitigation of national health risks. However, persistent structural challenges related to inequity, including procurement delays, limited bioinformatic

Psychosocial experiences of cancer patients across the COVID-19 era: Lasting implications for oncology.

The COVID-19 pandemic profoundly affected the psychosocial well-being of cancer patients, who were simultaneously confronting a life-threatening illness and a global health crisis. This study explored how individuals with cancer made meaning of their experiences across four temporal phases of the pandemic (2020–2023). Using an interpretivist qualitative design, we conducted an inductive thematic analysis of written reflections from 120 patients who responded to the open-ended prompt, “What do the words ‘cancer’ and ‘pandemic’ mean to you?”. Four overarching themes emerged: Fear of Death, Lonel

Influenza: epidemiology and hospital management.

Influenza is a cause of significant morbidity, mortality, economic and social disruption. Annual seasonal influenza epidemics result in 290,000-650,000 deaths worldwide, while influenza pandemics have resulted in many more - the A(H1N1) pandemic of 1918-1919 caused 20-50 million deaths. Healthcare systems struggle to effectively manage the constant threat because of the evolving nature of the virus. Since the start of 2021, there have been four events of concern related to influenza reported by the World Health Organization. To reduce the burden of disease and protect our global health securit

Adenoviral Vector-Based Vaccine Platforms for Developing the Next Generation of Influenza Vaccines.

Ever since the discovery of vaccines, many deadly diseases have been contained worldwide, ultimately culminating in the eradication of smallpox and polio, which represented significant medical achievements in human health. However, this does not account for the threat influenza poses on public health. The currently licensed seasonal influenza vaccines primarily confer excellent strain-specific protection. In addition to the seasonal influenza viruses, the emergence and spread of avian influenza pandemic viruses such as H5N1, H7N9, H7N7, and H9N2 to humans have highlighted the urgent need to ad

Leveraging Systems-of-Systems Analysis to Strengthen Epidemic Intelligence for Preparedness and Response.

The COVID-19 pandemic exposed significant gaps in the coordination and integration of efforts required to effectively manage large-scale infectious disease outbreaks. A successful response to such crises demands the swift and ongoing synthesis of information and activities across multiple sectors, including government, healthcare, and private industry. However, these systems are often managed in isolation, leading to misaligned policies, fragmented communications, and inefficiencies that hinder pandemic response efforts. To address these challenges, we propose adopting a systems-of-systems (SO

An Ophthalmologist's Insight Into The Viral Pandemics.

In recent past, major pandemics and epidemics have occurred due to the emergence and resurgence of the novel strains of viruses like Influenza [HINI-A "Spanish Flu", H1N1-Novel A "Swine flu"], Corona [Middle East Respiratory Syndrome (MERS), Severe acute respiratory syndrome (SARS-CoV-1, SARS-CoV-2)] and Ebola virus [Ebola virus disease]. These infectious diseases may have several ocular manifestations and rarely might be the presenting symptom of the underlying disease. The eyes can act as a portal of entry and/or route of viral transmission for these pathogens. Therefore, an ophthalmologist/

Rapid Response Subunit Vaccine Design in the Absence of Structural Information.

Prior to 2020, the threat of a novel viral pandemic was omnipresent but largely ignored. Just 12 months prior to the Coronavirus disease 2019 (COVID-19) pandemic our team received funding from the Coalition for Epidemic Preparedness Innovations (CEPI) to establish and validate a rapid response pipeline for subunit vaccine development based on our proprietary Molecular Clamp platform. Throughout the course of 2019 we conducted two mock tests of our system for rapid antigen production against two potential, emerging viral pathogens, Achimota paramyxovirus and Wenzhou mammarenavirus. For each vir

Embracing computer modeling to address pandemic influenza in the 21st century.

Reacting to a pandemic influenza outbreak will require the mass distribution of vaccine, when available, which will require county health departments to set up and operate one or more mass vaccination clinics, also known as points of dispensing (PODs). Carefully planning these PODs before an event occurs is a difficult but important job. First, this article describes a tool--the Clinic Planning Model Generator computer program--designed to help public health agencies evaluate and make adjustments to their POD plans. The Clinic Planning Model Generator was built on data from a smallpox exercise

A year of terror and a century of reflection: perspectives on the great influenza pandemic of 1918-1919.

BACKGROUND: In the spring of 1918, the "War to End All Wars", which would ultimately claim more than 37 million lives, had entered into its final year and would change the global political and economic landscape forever. At the same time, a new global threat was emerging and would become one of the most devastating global health crises in recorded history. MAIN TEXT: The 1918 H1N1 pandemic virus spread across Europe, North America, and Asia over a 12-month period resulting in an estimated 500 million infections and 50-100 million deaths worldwide, of which ~ 50% of these occurred within the f
