Paragonimiasis is an infection caused by lung flukes of the genus Paragonimus. In Asia, P. westermani infections are relatively common because of dietary practices. However, in North America, cases of paragonimiasis, which are caused by P. kellicotti flukes, are rare. Only 7 autochthonous cases of paragonimiasis were reported during 1968-2008. In 2009, we reported 3 new case-patients with paragonimiasis who had been seen at our medical center over an 18-month period. Six additional case-patients were identified in St. Louis, Missouri, USA, and treated at Washington University-affiliated health…

Paragonimiasis is a foodborne parasitic zoonosis caused by lung fluke species of the genus Paragonimus. The Paragonimus westermani is the most common human pathogen in Asian countries. In northeast India, Paragonimus heterotremus has been documented as the only human pathogen in the earlier literature. In India, P. westermani infection in humans remained undetermined. Herein, we report a case of pulmonary paragonimiasis due to P. westermani in an adult female in Manipur. The diagnosis was made by morphological and molecular characterisation of the eggs in the sputum. This is the first confirme…

BACKGROUND: Paragonimiasis is a food-borne parasitic infection caused by lung flukes of the genus Paragonimus. Although the most common site of infection is the pleuropulmonary area, the parasite can also reach other parts of the body on its journey from the intestines to the lungs, ending up in locations such as the brain, abdomen, skin, and subcutaneous tissues. Ectopic paragonimiasis is difficult to diagnose due to the rarity of this disease.
CASE SUMMARY: Here, we report a rare case of simultaneous breast and pulmonary paragonimiasis in a woman presenting painless breast mass and lung nod…

Parasitic helminthiases, such as toxocariasis, cysticercosis and paragonimiasis are a public health threat, since they can affect the brain leading to neurological disorders. Epilepsy and paragonimiasis are common in southwestern Cameroon. We reviewed the literature for studies using antigens to diagnose toxocariasis, cysticercosis, and paragonimiasis. Serology revealed that 61 (36.3%), 26 (15.5%) and 2 (1.2%) of 168 persons examined [78 males (15.2 +/- 8.2 years old), 90 females (12.9 +/- 5.9 years old), 143 persons < 20 years old] had antibody responses to toxocariasis, paragonimiasis and cy…

Pleural parasitic infection is an extremely rare disease of the pleura caused by a variety of parasites, with paragonimiasis infection being the most common. The lack of specific clinical symptoms for paragonimiasis makes it easy to misdiagnose as tuberculosis, causing unnecessary drug-related adverse effects and financial burdens from incorrect treatment. We report a case of a pediatric patient presenting with an isolated pleural effusion that was misdiagnosed as tuberculosis; the patient was eventually diagnosed with pleuropulmonary paragonimiasis infection after immunologic and serologic te…

It has been experimentally found that larval and pulmonary paragonimiasis types are caused by the trematode Paragonimus westermani ichunensis. The development of either paragonimiasis in nontypical definitive hosts depends on the stage of larval development when they penetrate into the mammal. Only the P. westermani ichunensis trematode is likely to develop with the participation of Juga mollusks on the territory of the Primorye.

As a rare zoonotic helminth disease, paragonimiasis is easy to be misdiagnosed. Adequate attention to the medical history of the patient and early detection of serological antibodies can improve the diagnosis rate. Praziquantel and trichlorobendazole are commonly used for treatment, and the prognosis is good. This case report mainly introduces the classification, diagnosis, and treatment of paragonimiasis, with an aim to draw the attention of doctors to the disease.

The antigenic components of a Paragonimus heterotremus saline extract were revealed by SDS-PAGE and Western blot analysis using sera from 32 patients with P. heterotremus infection, 60 with other helminthiasis, and 15 normal human sera. It was found that the worm extract was comprised of more than 13 polypeptides, among which 5 components were strongly recognized by paragonimiasis sera. These components had approximately showed molecular weight of less than 12.3, 12.3, 18.5, 31,.5 and 38 kD. Only the 31.5 kD component was recognized by all 32 paragonimiasis sera. Sera from other helminth infec…

Six cases of human paragonimiasis (Paragonimus uterobilateralis) are documented from Liberia and Guinea. These are the first cases reported from these countries. Paragonimiasis should be considered in the differential diagnosis of any West African patient who presents with hemoptysis. Praziquantel was used successfully to treat these patients, and may be the drug of choice for P. uterobilateralis infections.

A recombinant protein of Paragonimus westermani yolk ferritin was bacterially produced from a previously cloned complementary DNA and was used as an antigen for an enzyme-linked immunosorbent assay (ELISA) against paragonimiasis- and other helminth-infected sera to evaluate its serodiagnostic potential. The ELISA revealed that paragonimiasis westermani had 88.2% sensitivity and 100% specificity. The positive and negative predictive values of the ELISA were calculated to be 100% and 97.1%, respectively. Sera from cats experimentally infected with P. westermani began to produce immunoglobulin G …

OBJECTIVE: To identify the etiologic agent from a paragonimiasis patient using molecular techniques.
METHODS: The complete nuclear ribosomal DNA second internal transcribed spacer (ITS2) gene sequence of eggs in sputum from a paragonimiasis patient was obtained by directly auto-sequencing its PCR product. ITS2 genes from eggs of Paragonimus westermani and Pagumogonimus skrjabini (both from animal hosts) were also sequenced for comparison. In addition, morphological comparisons were made with the eggs of the two species.
RESULTS: The ITS2 gene from the human case was 100% identical with the s…

A pulmonary nodule is a common radiological finding encountered in routine medical practice. It needs to be extensively evaluated as the differential diagnosis can range from sinister malignancies to fully treatable infectious causes. Here, we present a rare case of pulmonary paragonimiasis presenting as pulmonary nodule. This case report aims to sensitise the medical practitioner regarding pulmonary paragonimiasis which is completely treatable but carries a high risk of being under diagnosed.

It is clinically important to differentiate tissue-invading helminthiasis. The purpose of this study was to assess the specific immunoglobulin G (IgG) antibody positive rates for clonorchiasis, paragonimiasis, cysticercosis, and sparganosis 4 helminthiases from 1996 to 2006 using multi-antigen enzyme-linked immunosorbent assay (ELISA) in Korea. Results of 6,017 samples, which were referred to our institute for serodiagnosis, were analyzed. The subjects with positive serum IgG antibodies were 1,502 (25.0%) for any of the 4 helminthiases. The overall positive numbers for clonorchiasis, paragonim…

Human paragonimiasis was endemic in Korea until the 1960's, and nowadays, the prevalence is decreasing. However, it is still one of the important helminthic diseases. Though it is essentially a pulmonary disorder, it may involve brain, muscle, mesentery, genital tract, pleura, peritoneum, spinal cord, spleen, and liver. We experienced two cases of paragonimiasis in a family who had ingested raw crabs together for 7 months. A 57-year-old female patient was admitted due to abdominal pain, diarrhea and tenesmus for 6 months. And, her 35-year-old son complained of cough, chest discomfort and dyspn…

Adult somatic antigen extract of Paragonimus heterotremus was compared with excretory-secretory (ES) antigen in an enzyme-linked immunosorbent assay (ELISA) for serodiagnosis of human paragonimiasis. The absorbence values in ELISA using the adult somatic antigen were not significantly different from the values obtained using ES antigens (P greater than 0.05). The sensitivity of the ELISA using either antigen was 100%, but the specificity was 96% with somatic and 98% with ES antigens due to a cross reaction with fascioliasis sera. It appears that both somatic and ES antigens are effective antig…

Paragonimiasis does not occur only in the South West Province of Cameroon. Four foci exist in the country. They are the well known focus in the Mount Kupe area, the Mbam focus, the Nyong focus and the Ntem focus. All of them are located in the rainforest within the distribution area of Potadoma. This disease, characterized by the presence of eggs in sputum and feces, has often been confused with tuberculosis. In the villages where paragonimiasis is diagnosed by means of parasitological techniques circulating antibodies, revealed with a P. westermani antigen (ELISA), are often found in blood sp…
