Pediatric Central Diabetes Insipidus due to Arginine Vasopressin Deficiency in the Perianesthetic Period.

The purpose of this concise review is to provide a practical approach to inpatient management of pediatric patients with central diabetes insipidus due to arginine vasopressin deficiency (AVP-D), as well as provide recommendations specific to the perianesthetic period. AVP-D poses significant challenges in perianesthetic management. A multidisciplinary approach to the perianesthetic management of AVP-D can optimize patient outcomes and reduce the risk of complications. Implementing structured protocols that address fluid management, medication dosing, and postoperative care is essential for en

The promise of precision: exploring the role of remimazolam in paediatric anaesthesia.

Remimazolam represents a breakthrough in paediatric anaesthesia, with predictable pharmacokinetics and safety profile. A recent study provides key dosing data for children while noting the need for comparative trials, pharmacogenetic studies, and long-term safety evaluations. We highlight its potential for broad clinical use, but call for further research to optimise its integration into paediatric anaesthesia practice.

Beyond the Clinic: why new bioactive restorative materials have really changed Paediatric Dentistry.

BACKGROUND: The aim of this paper is to remind how evidence-based paediatric dentistry should not only consider the best available scientific evidence relating to the patient's oral conditions but it should also consider the patient's needs beyond the clinic. In a child-family-oriented approach, the best restorative treatment for a primary tooth could not be the proper one for the child as a whole. Preservation of dental tissues as much as possible, without involving the pulp, is called minimal intervention dentistry (MID) and it is closely related to "selective caries removal" and "bioactive

Fundamentals and innovations in regional anaesthesia for infants and children.

Regional anaesthesia in children has evolved rapidly in the last decade. Although it previously consisted of primarily neuraxial techniques, the practice now incorporates advanced peripheral nerve blocks, which were only recently described in adults. These novel blocks provide new avenues for providing opioid-sparing analgesia while minimising invasiveness, and perhaps risk, associated with older techniques. At the same time, established methods, such as infant spinal anaesthesia, under-utilised in the last 20 years, are experiencing a revival. The impetus has been the concern regarding the po

Anesthesia for shared airway surgery in children.

Shared airway surgery in children is a complex, high-risk undertaking that requires continuous communication and cooperation between the anesthetic and surgical teams. Airway abnormalities commonly seen in children, the surgical options, and the anesthetic techniques that can be used to care for this vulnerable population are discussed. Many of these procedures were traditionally carried out using jet ventilation, or intermittent tracheal intubation, but increasingly spontaneously breathing "tubeless" techniques are being used. This review has been written from both the surgical and anesthetic

A comprehensive systematic review and meta-analysis of the safety of procedural sedation in pediatric emergency rooms.

PURPOSE: In pediatric emergency departments (PEDs), procedural sedation is frequently used to reduce pain and anxiety to support therapeutic and diagnostic procedures. The purpose of this meta-analysis was to evaluate the safety profiles of common sedatives in the setting of PEDs. METHODS: An extensive search of four databases was conducted to identify studies reporting adverse effects associated with procedural sedation in pediatric emergency settings. Studies that reported on sedation-related outcomes and involved patients younger than 18 were considered eligible. A proportionate meta-analys

Guideline update: Neonatal and infant airway management.

Managing the neonatal and infant airway is challenging due to several anatomic and physiologic differences compared to adults. In 2024, the European Society of Anaesthesiology and Intensive Care and British Journal of Anaesthesia published joint guidelines on airway management in neonates and infants (defined as <1 year). The guidelines recommend using history and physical examination to predict a difficult airway. They recommend using sedatives such as etomidate or ketamine to ensure appropriate anaesthesia combined with neuromuscular blocking agents. Videolaryngoscopy is the first choice for

Efficacy of dexmedetomidine in preventing emergence agitation in children undergoing tonsillectomy and/or adenoidectomy: a meta-analysis and systematic review.

INTRODUCTION: Emergence agitation (EA) affects 10–80% of children after general anesthesia, with high rates following tonsillectomy and/or adenoidectomy. Although dexmedetomidine shows potential for preventing EA, its efficacy and safety in this specific pediatric population remain uncertain, with conflicting evidence from previous studies. METHODS: Computerized searches of PubMed, The Cochrane Library, Web of Science, Embase, and OpenGrey were performed, and the search timeframe was from the time of construction to September 26, 2024, for all searches. Literature screening, data extraction, a

A review of the perioperative management of direct oral anticoagulants for pediatric anesthesiologists.

BACKGROUND: Although direct oral anticoagulants (DOACs) have been used in the adult population for over a decade, DOACs use has begun to rise in pediatric populations since FDA approval of rivaroxaban and dabigatran, DOACs offer several advantages for pediatric patients, to other anticoagulants, including a similar safety profile, minimal lab monitoring, and ease of administration. The rise in DOAC use has led to an increasing number of pediatric patients managed on DOACs presenting for elective and urgent procedures. Perioperative management of anticoagulation is often challenging for provide

Point-of-Care Ultrasound-Guided Serratus Anterior Plane Block for Chest Tube Placement in a Spontaneous Pneumothorax.

Chest tube placement is a common procedure in the pediatric emergency department. There is general emergency medicine literature as well as pediatric cardiac surgery literature supporting the use of an ultrasound-guided serratus anterior plane block for regional anesthesia with no prior pediatric emergency medicine studies to our knowledge. This case describes a pediatric patient who required chest tube placement twice for a pneumothorax and describes his preference for the nerve block over the more commonly used procedural sedation.

Safety and Efficacy of Using Sugammadex in Pediatric Patients: A Narrative Review.

Neuromuscular blockade reversal is essential in perioperative care of pediatric patients undergoing general anesthesia, as delayed recovery may generate a plethora of complications. Traditional reversal agents like neostigmine, an acetylcholinesterase inhibitor, are widely used but have variable efficacies, adverse side effects, and the need for additional sugammadex, a novel selective relaxant binding agent with a rapid and predictable pharmacologic profile, is effective in adults and has growing interest in pediatric populations. This narrative review summarizes findings from clinical trials

Combined use of neuraxial and general anesthesia during major abdominal procedures in neonates and infants.

With the advent of ultrasound and improvements in equipment, the applications of regional anesthesia in the pediatric population have continued to expand. Although frequently used for postoperative analgesia or as a means of avoiding general anesthesia in patients with comorbid conditions, the adjunctive use of regional anesthesia during general anesthesia may effectively decrease the intraoperative requirements for intravenous and volatile agents, thereby providing a more rapid awakening and earlier tracheal extubation. More recently, the limitation of the requirements for volatile and other

Peters anomaly: A 5-year experience.

BACKGROUND: Peters anomaly is a rare, congenital eye malformation characterized by an opaque cornea and blurred vision. Central corneal opacification can lead to delayed progression of visual development caused by defects in Descemet membrane and the posterior stroma. These children require several anesthetics for multiple eye examinations under anesthesia and corneal transplantation. AIMS: We sought to review the anesthetic management of patients with Peters anomaly for ophthalmologic procedures at Westchester Medical Center, a major referral center for Peters anomaly. METHODS: A retrospect

[Characteristics of the fetal and infant respiratory system : What the pediatric anesthetist should know].

Respiratory complications are the most frequent incidents in pediatric anesthesia after cardiac events. The pediatric respiratory physiology and airway anatomy are responsible for the particular respiratory vulnerability in this stage of life. This article explains the aspects of pulmonary embryogenesis relevant for anesthesia and their impact on the respiration of preterm infants and neonates. The respiratory distress syndrome and bronchopulmonary dysplasia are highlighted as well as the predisposition to apnea of preterm infants and neonates. Due to the anatomical characteristics, the low si…

Airway Management in a Patient with Wolf-Hirschhorn Syndrome.

We present a case of a 3-month-old female with Wolf-Hirschhorn syndrome (WHS) undergoing general anesthesia for laparoscopic gastrostomy tube placement with a focus on airway management. WHS is a rare 4p microdeletion syndrome resulting in multiple congenital abnormalities, including craniofacial deformities. Microcephaly, micrognathia, and glossoptosis are common features in WHS patients and risk factors for a pediatric airway that is potentially difficult to intubate. We discuss anesthesia strategies for airway preparation and management in a WHS patient requiring general anesthesia with end
