Biomechanical and histological evaluation of a prototype bovine pericardial patch for veterinary abdominal surgery: An approximation study.

In veterinary medicine, especially within large animal medical and surgical clinics, addressing cavitary defects is a primary concern. In bo … gentina. Tissues derived from the extracellular matrix are considered for this purpose, with bovine pericardium standing out due to its architectural characteristics and mechanical properties. For an ideal membrane, the pericardium must retain its biomechanical properties while being unreactive and safe for the host. This study aimed to biomechanically and hi …

[Acute and chronic-constrictive pericarditis].

Acute pericarditis is an inflammatory disease of the pericardium of variable etiology. A viral infection may sometimes precede symptoms but frequently the etiology re-mains unknown (idiopathic pericarditis). The disease is typically associated with left-sided chest pain and ECG abnormalities mimicking acute myocardial infarction. At physical examination the characteristic finding is a pericardial friction rub. A pericardial effusion of varying extent may be present or develop in the course of the disease. Pericardial tamponade, which may develop insidiously, represents a life-threatening compl…

[Further comment on mesotheliomas].

11 cases of malignant mesothelioma (0.153%) were among 7184 autopsies in Tambov Bureau of Pathology from 1978 to 1987. There were 4 cases of pleural mesothelioma (0.055%), 5 cases of pericardial mesothelioma (0.069%), one case of peritoneal mesothelioma and one case of a simultaneous involvement of the peritoneum, pleura and pericardium. Two cases of malignant epithelioid mesothelioma of the pericardium are described, which characterized by a diffuse growth along both surfaces of the pericardium with the accumulation in its cavity of two litres of hemorrhagic liquid. In the case of a 50-year-o…

[Cefotaxim for peri-operative short-term prophylaxis of infection in cardiac surgery. Concentrations in myocardial, pericardial, fat and skeletal muscle tissues].

Cefotaxim was administered intravenously in a dose of 50 mg/kg body-weight before and immediately after cardiac surgery. The drug's concentrations were measured in serum, myocardium, peri-operative fat, pericardium and skeletal muscle. The highest concentration was found 15 minutes after injection: 32 micrograms/g in pericardium, 22 micrograms/g in myocardium, 17.9 micrograms/g in fat and 12.3 micrograms/g in skeletal muscle. After 2 hours the average concentration in pericardium was 10.7, in myocardium 3.8, in fat 5.4 and in skeletal muscle 2.4 micrograms/g. The corresponding mean serum level…

Challenging case of pleuropericardial effusion: Primary extraskeletal Ewing's sarcoma masquerading as tubercular pericarditis.

Ewing's sarcoma is an aggressive bone and soft tissue cancer of children and young adults, which can rarely affect extraskeletal soft tissues and visceral organs as extraskeletal Ewing's sarcoma (EES). Primary EES of the pleura and pericardium is an extremely rare malignancy, and herein we report a case of Primary EES of the pericardium, which to the best of our knowledge is the first case report of the same from India.

Autologous pulmonary artery patch repair: a promising technique for type B aortic arch interruption.

The ideal patch material for aortic arch reconstruction in neonates and infants has not been yet identified. Several materials have been used, such as autologous pericardium, bovine pericardium and homografts. The goal is to maximize the use of autologous tissue to allow growth and minimize future reinterventions/reoperations. We present the technique of autologous pulmonary artery patch reconstruction of the aortic arch in a case of type B interruption in a neonate.

Cardiac xenotransplantation technology provides materials for improved bioprosthetic heart valves.

OBJECTIVES: Human subjects and Old World primates have high levels of antibody to galactose-α-1,3 galactose β-1,4-N-acetylglucosamine (α-Gal). Commercially available bioprosthetic heart valves of porcine and bovine origin retain the Gal antigen despite current processing techniques. Gal-deficient pigs eliminate this xenoantigen. This study tests whether binding of human anti-Gal antibody effects calcification of wild-type and Gal-deficient glutaraldehyde-fixed porcine pericardium by using a standard subcutaneous implant model. METHODS: Expression of α-Gal was characterized by lectin Griffonia…

Double-layer pericardial patch for atrial septal defect closure.

After repair of atrial septal defects with or without patch, residual shunt, shrinkage, haemolysis and thromboembolic complications may occur. Even though the pericardium is quite a suitable material for the closure of atrial septal defects, the external surface of the pericardium is not smooth and may be a nidus for thrombus formation. We present a new technique to prevent these complications by using a folded, double-layer, durable pericardial patch without chemical pretreatment.

An study on the influence of collagen fiber directions in TAVs performance using FEM.

Transcatheter Aortic Valve Implantation (TAVI) or Replacement (TAVR) is a promising treatment for aortic valve stenosis, consisting of a procedure to replace a damaged native aortic valve by a bioprosthetic one. This replacement valve control the flow of blood using leaflets that are similar to the ones of a native aortic valve. Commonly manufactured using bovine or porcine pericardium, it is a tissue histologically composed of collagen fibers embedded into a nearly-isotropic matrix, where their distribution makes the pericardium behave as an anisotropic hyperelastic material. Because of such …

Grand rounds: asbestos-related pericarditis in a boiler operator.

CONTEXT: Occupational and environmental exposures to asbestos remain a public health problem even in developed countries. Because of the long latency in asbestos-related pathology, past asbestos exposure continues to contribute to incident disease. Asbestos most commonly produces pulmonary pathology, with asbestos-related pleural disease as the most common manifestation. Although the pleurae and pericardium share certain histologic characteristics, asbestos-related pericarditis is rarely reported. CASE PRESENTATION: We present a 59-year-old man who worked around boilers for almost 30 years an…

Clinical experience with expanded polytetrafluoroethylene Gore-Tex surgical membrane for pericardial closure: a study of 110 cases.

Complete closure of the pericardium after cardiac operations has the advantage of avoiding injury of the heart and great vessels during reoperation. Between 1985 and 1987, the pericardium was closed with Gore-Tex Surgical Membrane (SM) in a selected series of 110 patients 1 month to 76 years of age. Fifty-three patients had congenital heart lesions and 57 patients had acquired heart disease. Overall hospital mortality was 3/110 cases. In no instance was there a relationship between occurrence of death and pericardial closure with SM. There was one episode of cardiac tamponade on the seventh po…

A failing right heart in an eggshell: A case report of idiopathic constrictive pericarditis.

KEY CLINICAL MESSAGE: Constrictive Pericarditis is a rare fibrotic conversion of the pericardium that results in non-specific clinical symptoms such as hepatomegaly, ascites, pleural effusions, and lower extremity edema. A multi-modal diagnostic approach with cardiac imaging tools, cardiac hemodynamic measurements, and tissue biopsy can be used to diagnose Constrictive Pericarditis. ABSTRACT: Constrictive Pericarditis is a rare complication resulting in the fibrotic conversion of the pericardium secondary to idiopathic, infective, post-surgical, or post-radiation etiologies. The rigid and res…

Pericardial Effusion in a Dog with Pericardial Hemangiosarcoma.

An adult Jack Russel terrier dog presented for evaluation of large-volume peritoneal and pleural effusion. Echocardiography revealed scant pericardial effusion and abnormally thickened pericardium. Electrocardiography revealed complete atrioventricular block with junctional and ventricular escape beats and occasional ventricular premature complexes. Computed tomography of the thorax confirmed diffuse abnormal thickening of the pericardium, and a tentative diagnosis of constrictive-effusive pericarditis was made. The dog underwent subtotal pericardiectomy to remove the parietal pericardium and …

Gastric ulcer perforation into the heart.

Penetration of the pericardium and heart is a very rare complication of benign peptic ulcer. The case is reported here of a 76-year-old woman with advanced senile dementia, who was admitted due to melena. Endoscopy revealed a large gastric ulcer located in a giant hiatal hernia. The ulcer base was protruding and strongly pulsatile, and seemed to be mobile and free in relation to the ulcer margins. This effect was particularly obvious during the pulsatile movements. Endoscopic findings suggested ulcer perforation to the pericardium. The patient's relatives denied consent to surgery. She was the…

Mechanical failure analysis of patch materials used in aortic arch reconstruction: implications for clinical practice.

OBJECTIVES: Thick-patch pulmonary homograft, autologous pericardium and CardioCel Neo are common patch materials for aortic arch reconstruction. Insufficient data exist on sutured patch strength and limits of use. We evaluated failure strength of these materials to develop a failure prediction model for clinical guidance. METHODS: Patch failure strength was evaluated via sutured uniaxial and burst pressure testing. In sutured uniaxial testing, patches were sutured to aortic or Dacron tabs and pulled to failure. In burst pressure testing, patches were sewn into porcine aortas or Dacron grafts …

Primary repair of complete sternal cleft with pectoralis major muscle flaps.

We report the successful coverage of a complete sternal cleft with exposed pericardium using bilateral pectoralis major muscle flaps in a newborn. The patient was transferred to our institution with nearly complete sternal aplasia, rectus diastasis, and only dessicated pericardium covering the visible heart. Complete closure without compromise of cardiac function was achieved at 2 days of age with bilateral pectoralis major muscle flaps and fasciocutaneous advancement flaps.

Reoperation for calcific constrictive pericarditis 43 years after pericardiectomy.

The case of calcific pericarditis in a 63-year-old male with a 1-year history of progressive right heart failure 43 years after pericardiectomy in 1960 is reported. The recurrent constriction was caused by the calcification of the residual posteroinferior pericardium and the impressing sternum of a pectus excavatum encasing the heart like a skull. The patient underwent total pericardiectomy of his residual pericardium through median sternotomy using cardiopulmonary bypass resulting in complete relief of his symptoms.

Multimodality imaging of pericardial diseases.

Pericardial disease is an important cause of morbidity and mortality in patients with cardiovascular disease. Inflammatory diseases of the pericardium constitute a spectrum ranging from acute pericarditis to chronic constrictive pericarditis. Other important entities that involve the pericardium include benign and malignant pericardial masses, pericardial cysts, and diverticula, as well as congenital absence of the pericardium. Recent advances in multimodality noninvasive cardiac imaging have solidified its role in the management of patients with suspected pericardial disease. The physiologic …

[Echinococcosis involvement of the heart].

A case report is given of a 37-year-old patient who had echinococcus infestment the heart. Diagnosis was established after drainage of the pericardium, when cysts of a diameter between 0.5 and 3 cm were evacuated. During this procedure the pericardium and the left ventricle were searched and revised extensively and drug therapy with Mebendazole was instituted. In spite of that a new cyst developed in the pericardium and a big cyst in the abdomen, was found which had practically transformed the whole left lobe of the liver into a cyst. Both lesions were removed surgically. It must be emphasized…

[Surgical technique of leaflet extension in aortic valve regurgitation].

Detailed procedure of the leaflet extension technique in aortic valve regurgitation is described. The concept of the technique is to create a wide and deep coaptation zone by extending the prolapsed leaflets using glutaraldehyde-preserved autologous pericardium, and by suspending the commissures. The leaflet extension technique is a procedure of choice for aortic valve regurgitation in young patients who need to avoid strict anticoagulation therapy. A careful long-term follow-up is necessary to evaluate the durability of glutaraldehyde-preserved pericardium in aortic position.
