Presence of cytomegalovirus in the perilymphatic fluid of patients with profound sensorineural hearing loss caused by congenital cytomegalovirus infection.

CONCLUSION: Not all patients diagnosed with congenital infection using umbilical cord assay were found to be positive for CMV-DNA by perilymphatic fluid assay. In addition, a CMV-DNA-positive result was observed in one patient who had not been diagnosed with congenital infection. Sampling of perilymphatic fluid from a large population of patients with congenital SNHL caused by congenital CMV infection or of unknown etiology is required to determine the prevalence of CMV-related profound HL. OBJECTIVES: Sensorineural hearing loss (SNHL) is one of the most frequent manifestations in patients wi…

[Validation of a clinical scale for the diagnosis of perilymphatic fistula].

INTRODUCTION: The diagnosis of perilymphatic fistula (PLF) is difficult because no single clinical situation gives the diagnosis for sure. The goal of this article is to study the clinical situations where you must suspect a PLF and to support a clinical scale described in a previous work (Bussières et al 2003). METHODS: Retrospective study of 15 patients that had an exploratory tympanotomy with a PLF not confirmed preoperatively. An analysis of the symptoms, signs and complementary exams were done. The surgical technique and findings and the postoperative evolution were noted. RESULTS: Ther…

Transtympanic electrocochleography in the assessment of perilymphatic fistulas.

An objective method for the pre-operative diagnosis and the post-operative assessment of a presumed perilymphatic fistula (PLF) using transtympanic electrocochleography is presented. Three cases are reported in which the history of the disease and the symptoms strongly suggested the presence of a PLF. Pre-operative transtympanic electrocochleography (TT ECoG) recordings at rest showed changes similar to those of endolymphatic hydrops and signs of instability of the inner ear hydrodynamic system during raised intrathoracic pressure. Surgery revealed a visible leak in two of the three cases. Bot…

Comparison of intracranial pressure between spina bifida patients and normal subjects using a non-invasive pressure assessment technique.

A comparison of mean intracranial pressure (ICP) between 36 spina bifida patients and 24 patients with clinically normal ICP was undertaken. A new non-invasive method of assessing ICP was used throughout this study. The technique relates tympanic membrane displacement to ICP and has been shown to be a reliable measure of mean ICP by comparison with direct measures via ventricular catheters and reservoirs, subdural catheters and lumbar punctures. Using this non-invasive method the results of this study indicated that the patients with spina bifida and without a ventricular shunt have a higher t

Idiopathic sudden progressive hearing loss and round window membrane rupture.

We investigated 24 cases of idiopathic sudden progressive deafness (ISPD) in which worsening of sensorineural hearing loss after initial examination was confirmed by audiometry. Exploratory tympanotomy was performed in 10 of 24 patients, and revealed perilymphatic leak in 4 patients. However, there was a higher incidence and a more severe degree of vertigo and imbalance in the group that was operated on than in the non-operated group. Our study has shown that there was no clear difference in the degree of hearing improvement between the operated group and the conservatively treated group. In b…

Apical lesions of the cochlea in idiopathic endolymphatic hydrops and other disorders: pathophysiological implications.

Apical lesions of the cochlea may be caused by direct trauma, impairment of blood supply, contamination of perilymphatic fluid and endolymphatic hydrops. It is proposed that endolymphatic hydrops causes apical lesions when endolymph or its components leak into the perilymphatic fluid and pass through the helicotrema to degrade the tissue fluid environment of the sensory and neural structures in this area.

Therapeutic Timing at Mitochondrial Redox-Autophagy-Mitophagy Checkpoints in Age-Related Hearing Loss.

Age-related hearing loss (ARHL) is common, disabling, and still managed largely with compensatory devices rather than mechanism-modifying th … fine a biomarker and detection framework that distinguishes cochlear tissue endpoints from blood or perilymph proxies and separates mouse proof-of-mechanism from human trial stratification. This approach clarifies which evidence is sufficient for prevention studies, which remains missing, and how future ARHL trials can be aligned with mechanism and timing.

The presence of EH could not accurately be assessed in 15.79% of ears in the IT group due to weak perilymph enhancement. After exclusion of these ears, the IT and IV groups showed similar EH detection rates. CONCLUSION: The insufficient success rate of IT method reduced its actual EH detection rate. Using i3D-real IR, the IV method offers the higher succ …

Four-Point Impedance and Utricular Dysfunction Is Associated with Postoperative Dizziness after Cochlear Implantation.

INTRODUCTION: Postimplantation dizziness is common, affecting approximately 50% of patients. Theories for dizziness include utricular inflammation, endolymphatic hydrops, and loss of perilymph. Four-point impedance (4PI) is a novel impedance measurement in cochlear implantation that shows potential to predict hearing loss, inflammation, and fibrotic tissue response. Here, we associate 4PI with dizziness after implantation and explore the link with utricular function. METHODS: Subjective visual vertical (SVV) as a measure of utricular function was recorded preoperatively as a baseline. 4PI was…

Hearing loss following ventriculoperitoneal shunt in communicating hydrocephalus patients: a pilot study.

OBJECTIVES/HYPOTHESIS: Hearing loss can be associated with a decrease in cerebrospinal fluid (CSF) pressure because changes in CSF pressure induce changes in perilymph pressure. Hearing loss after neurosurgical procedures have been reported, but clinical information on hearing loss after the placement of ventriculoperitoneal (VP) shunts, the most commonly used CSF shunt for hydrocephalus patients, is limited. This study is aimed to show the relationship between VP shunt and hearing loss. STUDY DESIGN: Prospective study. METHODS: Pure tone threshold and electrocochleography were preoperativel…

Transepithelial voltage and resistance of vestibular dark cell epithelium from the gerbil ampulla.

Transepithelial voltage (Vt) and resistance (Rt) were measured across the dark cell epithelium of the gerbil ampulla using a micro Ussing chamber of improved design in order to test the view that the histologically similar epithelia in the utricle and in the ampullae exhibit similar electrophysiologic functions. Vt was found to be 8.0 +/- 0.3 mV and Rt was 11.6 +/- 0.4 ohm-cm2 (N = 179) when both sides of the tissue were perfused with symmetric perilymph-like solution. The equivalent short circuit current (Isc = Vt/Rt) was 712 +/- 18 microA/cm2 (N = 179). Isc was reduced from 638 +/- 60 to 48 …

Autoimmunity and otologic disease: clinical and experimental aspects.

Each of the anatomic structures of the ear (external, middle, and inner) is subject to immunologic influence and injury. Studies in experimental animals have shown that primary immune responses to foreign antigens can be induced within the middle and inner ear as indicated by (1) infiltration and local persistence of mononuclear leukocytes and plasma cells, (2) appearance of antibody in perilymph, and (3) eventual development of systemic immunity. Protective effects of inner ear immunization against subsequent viral challenge have also been shown. Clinically, otologic disease can occur in asso…
