Major depression, dysthymia and depressive personality disorder.

The separation of persistent depression into meaningful and useful subcategories, including major depression, dysthymia, recurrent brief depression, and depressive personality disorder, is the subject of much debate. Depressions can be grouped on the basis of their type and severity of symptoms, aetiology, clinical course, or their association with other psychiatric illnesses. Several investigators have conducted epidemiologic and family studies to evaluate the prevalence of depressive disorders, their diagnostic stability over time, and the amount of overlap among the disorders. Although prog…

DSM-III personality disorders in the community.

OBJECTIVE: The aims of this study were to estimate the prevalence and investigate the comorbidity and potential consequences of DSM-III personality disorders in the community. METHOD: A total of 810 adults were examined in the second stage of the Eastern Baltimore Mental Health Survey in 1981, part of the National Institute of Mental Health Epidemiologic Catchment Area program. The subjects were directly examined by psychiatrists using a semi-structured method that allowed diagnosis of all DSM-III personality disorders as well as other DSM-III psychiatric disorders. RESULTS: The prevalence o…

Personality variations in healthy relatives of schizophrenics.

A familial relationship between schizophrenia and schizotypal personality disorder is widely acknowledged; the familial relationship between schizophrenia and the broad continuum of schizoid personality variation is less clear. In a comprehensive family study healthy relatives of schizophrenics were compared by self rated personality features with relatives of unipolar depressed patients and with relatives of controls. The dimension of schizoidia was not able to distinguish the groups of relatives. However, relatives of schizophrenics (in particular male relatives) scored higher on 'normalized…

[Personality disorders in patients with panic disorder].

The relationship between personality disorders and panic disorder in adults is analyzed in this study. The patients included in this work presented anxiety disorder with agoraphobia and were compared with a group of normal controls. The percentage of patients with personality disorder was 46% significantly higher than those encountered in the control group (16%). The existence of these disorders do not seem to modify the severity of panic disorder.

[Structure of the hypochondriacal and hysteroid personality].

Hypochondriacal and hysteroid personalities are considered the extreme variants of defined emotional and motivational dimensions. The hysteroid personality is mainly characterized by a strong need for social attention and respect as well as by a lack of emotional genuiness. For the diagnosis of hypochondriacal and hysteroid personality tendencies, a Hypochondria-Hysteria Inventory was developed and employed in 13 different samples with a total of 1206 persons. The index scores of the individual degrees of hypochondriacal and hysteroid tendencies were demonstrated to be of sufficient reliabilit…

[Hysterical personality and sexuality of the female].

The investigation involved 30 hysterical women, who were interviewed and who completed four questionnaires relating to: psychosexual development; attitude towards sexuality; sexual activity; and reactivity. Dysharmonic sexual development, and low levels of activity and reactivity in their sexual life was revealed. Contrasted with other groupings of female psychiatric patients, from a sexual point of view the female hysterical personality presents a special psychological case.

Personality styles of patients asserting environmental illness.

Case reports and chart reviews of patients asserting environmental illness suggest that they suffer from psychiatric difficulties, typically somatization disorder. We assert that viewing these patients solely as somatizers or hysterical characters searching for a nurturant relationship will undermine the doctor-patient relationship. Rather, many of these patients are obsessive/paranoid characters searching for a medical explanation to their physical symptoms. This distinction is highlighted by contrasting the clinical presentations of hysteric/somatizing patients with those environmental illne

An MMPI description of the narcissistic personality.

This study developed a Minnesota Multiphase Personality Inventory (MMPI) description of the narcissistic personality in a nonclinical population. The Narcissistic Personality Inventory (NPI) and the MMPI were administered to two samples of 57 and 173 subjects. A correlational analysis produced a cross-validated positive relationship between narcissism and MMPI mania (Ma) and cross-validated negative relationships between narcissism and MMPI depression (D), psychasthenia (Pt), social introversion (Si), anxiety (A), repression (R), and ego control (Ec). A correlational analysis of the 7-factor c…

Specificity of personality traits in borderline hypertension.

Patients with borderline hypertension tend to suppress expression of anger and hostility. However, if anything, they are psychologically hyperresponsive. The blood pressure elevation is maintained neurogenically in many with borderline hypertension. Consequently, it is tempting to postulate that suppressed anger causes neurogenic increases in blood pressure. Available data are inadequate to substantiate this hypothesis. It is equally plausible that neurogenic factors affect emotions, behavior and blood pressure. The terms specificity and directionality are introduced and defined to facilitate

[Psychogenic pathologic personality development in advanced age].

A study of 51 patients with psychogenic abnormalities developed in old age has shown that the development of such disorders is a comparatively frequent outcome of neuroses and reactive psychoses with the onset in the presenile period and only as an exception takes place in patients with the onset of psychogenic disturbances in old age. Depressive development has proved most typical for advanced age. Such manifestations as chronic subdepression, diffuse hypochondria, egocentrism and rent orientations are universal for all clinical variants of senile pathological development of personality. Acco…

Hearing and premorbid personality in paranoid states.

The author studied the hearing and premorbid personality of 79 patients whose illnesses began before they were 60 years old. Hearing was assessed by pure-tone audiometry and premorbid personality by interviewing relatives. Thirty-two patients had paranoid states, 32 matched patients had recurrent unipolar depression, and 15 patients had schizophrenia. Three paranoid patients and two depressed patients met DSM-III criteria for premorbid paranoid personality. There was no consistent difference in hearing loss between paranoid and depressed patients or between paranoid patients and a population c…

Prolonged psychotic states in borderline personality disorder.

This paper reviews the literature concerning the phenomenology and duration of psychosis in borderline patients and presents empirical findings concerning a sample of eight borderline women with psychosis. Little work has yet been done to establish the duration of so called "transient psychotic episodes," but the clinical consensus up to now has been that such episodes last less than two weeks. The patients in the current study experienced psychotic symptoms lasting between three weeks and four months. They were characterized by hysterical features, visual disturbances, and a possible history

Incipient multiple personality in children. Four cases.

Despite extensive study of child abuse and increasing interest in multiple personality, there has been no way of identifying abused children who are potential multiples, and only rarely has multiple personality been diagnosed before adulthood. This article describes four cases where dual identity was claimed by the child and/or trance states were evident in response to questions or stimuli related to traumatic events. In addition, the three children who were treated according to a program developed to integrate multiple personalities in adults showed unification of consciousness and immediate …

Spontaneous self-hypnosis in multiple personality disorder.

The authors posits that the central mechanisms in cases of multiple personality appears to be spontaneous self-hypnosis. As the syndrome is a product of hypnosis and as excellent hypnotic subjects are potentially able to induce a variety of psychiatric symptoms, these patients report many symptoms referable to other major syndromes. This accounts for the many diagnoses attributable to them. The study of multiple personalities offers insights into the capabilities of hypnosis, the genesis of an unconscious, and the mechanism of repression.

Specificity revisited: personality profiles and behavioral issues.

The notion of a connection between illness and temperament goes back at least as far as Hippocrates with his typology of melancholy, sanguine, phlegmatic and choleric personality types. In the 1930s, Dunbar studied large numbers of patients with several chronic diseases from which she derived personality profiles which correlated with the illnesses. Many other studies developed similar correlations. Following the work of Alexander, this approach was deemed superficial. More recently, the description of type A behavior has consistently been shown to correlate with coronary artery disease. The c…
