A huge sphenoid sinus mucocele: report of a case.

Sphenoid sinus mucoceles are exceedingly rare. Most have occurred in men between the ages of 30 and 40 years. We report a case of sphenoid sinus mucocele that was unusual not only for the age of the patient (13 yr), but for the massive size of the lesion (59 x 53 mm). The patient presented with a 20-day history of unilateral proptosis, decreased vision, diplopia, and photophobia. He also complained of severe bilateral nasal obstruction, nasal stuffiness, and a mucoid nasal discharge. Endoscopic decompression of the left sphenoid sinus was performed, and approximately 200 ml of thick, sterile m…

Black iris-diaphragm intraocular lens for aniridia and aphakia.

We present the first reported use in the United States of a black iris-diaphragm intraocular lens (IOL) for the treatment of traumatic aniridia and aphakic bullous keratopathy. The patient presented to a university-based practice with contact-lens-intolerant aniridia and aphakia with painful bullous keratopathy from a failed corneal graft. He was treated with combined penetrating keratoplasty and transscleral fixation of an aniridia IOL. The patient's preoperative symptoms of debilitating glare and photophobia resolved substantially after surgery, despite mild postoperative inflammation that r…

Acute hemorrhagic conjunctivitis in Central America: first enterovirus epidemic in the western hemisphere.

Acute hemorrhagic conjunctivitis (AHC) is an eye infection that was first noted in 1969 and became a pandemic disease in Africa, Southeast Asia, and Japan in the 1970s. It was not noted in the western hemisphere until 1981. This report describes the clinical experience with AHC in Honduras where it was first seen in the western hemisphere. Clinical presentation includes acute onset with symptoms of foreign body sensation, burning, and photophobia. Signs include lid edema, periauricular lymphadenopathy, conjunctival injection, follicular reaction, and typically subconjunctival hemorrhages. Sinc…

Viral Meningitis: A Pediatric Case Study.

Meningitis is a significant viral, bacterial, or fungal infection of the meninges that cover and protect the brain and the spinal cord. Symptoms of meningitis may present rapidly or develop gradually over a period of days, manifesting with common prodromal flu-like symptoms of headache, photophobia, fever, nuchal rigidity, myalgias, and fatigue. Character and significance of symptoms vary by patient age. Symptoms of infection may improve spontaneously or worsen, becoming potentially lethal. Early recognition and treatment of meningitis are crucial to prevent morbidity and mortality. The case r…

The management of accidental dural puncture in pregnant women: what does an obstetrician need to know?

Post-dural puncture headache (PDPH) also known as spinal (or post-spinal) headache still remains a disabling complication of needle insertion into the subarachnoid space. Pregnant women are at particular risk of dural puncture, and the subsequent headache, because of sex, young age, and the widespread application of regional anesthesia. Accidental dural puncture complicating epidural anesthesia varies in incidence from 0.19 to 4.4%. The incidence of epidural needle-induced PDPH headache in pregnant women has been reported to range 76-85%. The classic symptoms of PDPH consist of photophobia, na…

Ophthalmomyiasis in Kuwait: first report of infections due to the larvae of Oestrus ovis before and after the Gulf conflict.

We report for the first time four cases of ophthalmomyiasis due to the larvae of Oestrus ovis in Kuwait, before and after Operations Desert Shield and Desert Storm. The larvae were easily removed under local anaesthetic. The symptom complex of acute foreign body sensation, irritation, redness, lacrimation and photophobia resolved rapidly. Ophthalmic antibiotic and corticosteroid drops were also instilled and recovery was uneventful. The ophthalmomyiasis may not always be associated with contact with sheep-rearing per se. Medical personnel should therefore be aware that in cases of what might a…

Rubeola keratitis.

Twenty-seven young adult patients with rubeola were evaluated during the acute contagious phase of their disease. Koplik's spots were present in all patients. Conjunctivitis was present in 15 of the 27 (56%), and photophobia was present in 14 of the 27 (52%). Bilateral epithelial keratitis was present in all 27 patients. This keratitis, in otherwise healthy young adults, as in healthy children, was a benign process and required no medical therapy. The epithelial lesions were very slow to resolve completely and continued to be seen at the slit lamp after the patients had become asymptomatic. Op…

Brain trauma impacts retinal processing: photoreceptor pathway interactions in traumatic light sensitivity.

BACKGROUND: Concussion-induced light sensitivity, or traumatic photalgia, is a lifelong debilitating problem for upwards of 50% of mild traumatic brain injury (mTBI) cases, though of unknown etiology. We employed spectral analysis of electroretinographic (ERG) responses to assess retinal changes in mTBI as a function of the degree of photalgia. METHODS: The design was a case-control study of the changes in the ERG waveform as a function of level of light sensitivity in individuals who had suffered incidents of mild traumatic brain injury. The mTBI participants were categorized into non-, mild

What initiates a migraine attack? Conclusions from four longitudinal studies of quantitative EEG and steady-state visual-evoked potentials in migraineurs.

OBJECTIVES: Quantitative electroencephalograpic (QEEG) frequency spectra and steady-state visual-evoked potentials (SSVEP) are indicators of … h before the attack, slow and asymmetric EEG activity developed. Increased trigger sensitivity and photophobia correlated with higher theta power and depressed photic responses. Attack duration, migraine history duration, and pain intensity were associated with EEG slowing. CONCLUSIONS: A general tendency toward EEG slowing and depression of photic response …

An unusual case of headache.

We report an unusual case of carcinomatous meningitis presenting as a headache in a previously well woman. It occurs in only 3-5% of solid tumours and is an unusual form of presentation for underlying primary malignancy. This 44-year-old woman initially presented with subacute headache of 3 weeks duration and was treated as migraine, and later thought to have viral meningitis. The diagnosis was made from cytology examination on repeated lumbar puncture of cerebrospinal fluid (CSF). Within a month of onset of symptoms, the patient developed gradually worsening headache, right 6th cranial nerve

Severe herpes simplex virus encephalitis complicated by atypical Takotsubo cardiomyopathy with left ventricular systolic dysfunction.

A previously healthy woman in her 30s presented with high-grade fever, photophobia and fluctuating consciousness, and was diagnosed with herpes simplex virus type 1\ encephalitis. Her condition deteriorated, requiring intensive care support for type 1 respiratory failure. During her intensive care unit stay, she was found to have significant left ventricular systolic dysfunction with basal-mid akinesia and apical sparing on echocardiography. Cardiac MRI demonstrated features consistent with stress-induced (Takotsubo) cardiomyopathy. She made a complete neurological and cardiac recovery with su…

Sleeping with your eyes open: a fortuitous case of nocturnal lagophthalmos.

The eyes close at sleep onset, except for the 5% of the population in whom eyelid closure is only partial. This condition is called nocturnal lagophtalmos. We observed nocturnal lagophtalmos in a 67-year-old woman during video-polysomnography. She reported occasional eye redness, photophobia, and nocturnal tearing and was prescribed an eye lubricant for evening use. During video-polysomnography, her eyes were closed during N1 and N2 sleep but partially open during N3 sleep with occasional slow eye movements. Her eyes were wide open during REM sleep with numerous distinct rapid eye movements. T…
