BACKGROUND: The biopsy of pulmonary neoplasms localized in close proximity of the thoracic wall and the histological examination of the specimens represent an important diagonstic tool in the work up of benign and malign pulmonary tumours. The biopsy techniques differ with respect to the guiding imaging method, the devices used and the quantity of specimen. This study investigates efficacy and safety of the ultrasound guided transthoracic biopsy.
PATIENTS: 46 consecutive patients (n = 37 male; n = 9 female) with a mean age of 64.4 years were included in this analysis. 13 patients suffered fro

INTRODUCTION: Desmoplastic malignant pleural mesothelioma (DMPM) is a sarcoma-type mesothelioma, comprising approximately 5% of malignant pleural mesotheliomas. Although effusion cytology is commonly used as the primary diagnostic approach for mesothelioma, it may not be useful for DMPM because of the presence of desmoplasia and bland cellular atypia. We report a case, and previously undescribed cytological features, of DMPM that was diagnosed during autopsy.
CASE PRESENTATION: A man in his 60s with a history of occupational asbestos exposure was referred to our hospital with right chest pain

CT is the procedure of choice in evaluating disease extent in pleural-based tumors, as well as in pleural effusions. In particular, this modality can be helpful in determining chest wall, mediastinal, and diaphragmatic involvement by malignant mesothelioma. MR imaging should not be routinely employed in evaluation of these entities, and its use is limited to special anatomic areas (lung apex, diaphragmatic surface, heart and spine).

A cytological and immunological examination of pleural exudates of different etiology indicates that tumour exudates differed from exudates of tuberculous and parapneumonic etiology by distinctly reduced level of immunoglobulin A as well as by an elevated content of circulating immune complexes. Prevalence of mesothelial cells accompanied by an increase of immunological indices in tumour exudates and their reduction in exudates in cases of cardiac pathology is to be considered as an unfavourable prognostic sign.

The method of chemiluminometry was used to examine the citrate blood plasma and pleural exudate of 186 patients with exudative pleurisy of tuberculous, parapneumonic and tumour etiology. It was shown that the proposed method is highly informative, simple in realization, objective in evaluation of obtained data. The method enables in 24 hours to gain information on the nature of pleural lesions.

Malignant pleural effusions due to prostatic carcinoma are rare. We examined the cytologic and clinical presentations of 14 malignant pleural effusions caused by prostate cancer. These cases represented 2.3% of all positive pleural effusions at our institution. All patients (n = 10) had high grade, high stage tumors, including three with small cell anaplastic carcinoma. Three cases had clinically documented metastases to pleura, and in two cases, metastases were documented at autopsy. Most tumor cells had large nucleoli and were arranged in small, loosely cohesive groups. Fluids due to the sma

187 pleural fluids of various etiology are examined with the intention to find hyaluronic acid. The colorimetric method of Morgan-Elson was used for this purpose. The mucopolysaccharid was identified in 34 cases: 18 in 30 cases of diffuse mesotheliomas 1 in 74 cases of secondary tumours of the pleura 8 in 20 cases of tuberculous pleurisy 5 in 37 cases of nonspecific pleurisy 2 in 6 cases of purulent pleursy. A level from more than 212 mg/litre was detected in 10 cases of mesotheliomas only. Such a concentration is consequently the proof for diffuse mesothelioma. The diagnostical improvement in

Most patients with pleural mesothelioma (PM) present with symptomatic pleural effusion. In some patients, PM is only detectable on the pleural surfaces, providing a strong rationale for intrapleural anticancer therapy. In modern prospective studies involving expert radiological staging and specialist multidisciplinary teams, the population incidence of stage I PM (an approximate surrogate of pleura-only PM) is higher than in historical retrospective series. In this Viewpoint, we advocate for the expansion of intrapleural trials to serve these patients, given the paucity of data supporting lice

Wilms tumor (WT) is the most common renal malignancy seen in pediatric patients. Although lungs are the most common site of metastasis in Wilms tumor, non-malignant pleural effusion has been infrequently reported. Here, we report a case of an eleven-year-old female who presented with an abdominal mass and progressive breathlessness. On further evaluation, she was found to have a right-sided Wilms tumor with ipsilateral massive pleural effusion. The effusion resolved almost completely after four weeks of chemotherapy. We conclude that patients suffering from Wilms tumor presenting with pleural

BACKGROUND: Thoracocentesis of pleural effusion is a simple technique for pleural fluid examination through cytology. In addition to cytological examination to assess the nature of pleural fluid content, we can also perform more detailed examinations through cytoblocks of residual fluid. These paraffin-embedded cytoblock samples are important because we can perform examinations as in other bioptic samples. In these samples, immunohistochemical and molecular analyses can be performed.
METHODS: Two hundred fifty-five cytological samples from patients with pleural effusion were examined. In case

More than half of all pleural effusions are due to malignancy of which lung cancer is the main cause. Pleural effusions can complicate the course of pneumonia, pulmonary tuberculosis, or underlying systemic disease. We explore the application of label-free surface-enhanced Raman spectroscopy (SERS) as a point of care (POC) diagnostic tool to identify if pleural effusions are due to lung cancer or to other causes (controls). Lung cancer samples showed specific SERS spectral signatures such as the position and intensity of the Raman band in different wave number region using a novel silver coate

OBJECTIVE: Computed Tomography (CT) is considered part of the routine diagnostic workup for pleural malignancy. The definitive diagnosis of pleural malignancy depends upon histological confirmation by pleural biopsy. The aim of this study is to assess the sensitivity and specificity of CT, in view of the latest imaging technologies, in detecting pleural malignancy compared to definitive histology achieved via thoracoscopy (VATS).
MATERIALS AND METHODS: We included in this retrospective study 90 patients (36 F, 54 M) with suspected pleural malignancy evaluated in our Institution with CT scan

Solitary fibrous tumor of pleura (SFTP) is a rare mesenchymal neoplasm that most commonly involves the pleura, is probably derived from fibroblasts, and has no relationship to malignant mesothelioma. Here, we report a case of complete resection of a giant malignant SFTP. A 61-year-old woman developed fever and left flank pain. Computed tomography revealed the tumor to be 13 cm in size and located in the left thoracic cavity, directly invading the left lower lobe of the lung. The patient underwent radical resection and left lower lobectomy. Immunohistochemical examination revealed a dense proli

This paper examines the spectrum of challenges that confront the diagnostic surgical pathologist in distinguishing benign mesothelial hyperplasia from malignant mesothelioma. The current state of the art with respect to the morphological, immunohistological, and molecular features that can assist in the evaluation of benign and malignant pleural disease is examined. Practical guidelines are offered that should reduce the risk of diagnostic error in this problematic area of surgical pathology.

Benign fibrous mesothelioma (BFM) is a primary, isolated tumor of the pleura. In 80% of patients the tumor originates in the visceral pleura. BFM is rare and localized malignant mesothelioma, whose prognosis and treatment is significantly different, must be considered as a differential diagnosis. We report 8 cases of BFM excised by thoracotomy. In 6 asymptomatic patients, diagnosis was based on radiological images. One patient with a large tumor suffered dyspnea, acropachia and hypertrophic osteoarthropathy. The last patient experienced long-lasting chest pain even though the tumor was small (

UNLABELLED: Recently thoracoscopy has been used with increasing frequency for the diagnosis and treatment of pleuropulmonary diseases.
METHODS: The main requirements for thoracoscopy are rigid telescopes, forceps, scissors, stapler and a video recorder. The procedure can be performed either under general anaesthesia with or without double lumen intubation or under neuroleptanalgesia after inducing an artificial pneumothorax. At the end of the procedure a chest tube should always be inserted even if it is only for a few minutes until the lung re-expands after diagnostic thoracoscopy. Complicat
