Clinical Implications of Physiologic Flow Adjustment in Continuous-Flow Left Ventricular Assist Devices.

There is increasing evidence for successful management of end-stage heart failure with continuous-flow left ventricular assist device (CF-LVAD) technology. However, passive flow adjustment at fixed CF-LVAD speed is susceptible to flow balancing issues as well as adverse hemodynamic effects relating to the diminished arterial pulse pressure and flow. With current therapy, flow cannot be adjusted with changes in venous return, which can vary significantly with volume status. This limits the performance and safety of CF-LVAD. Active flow adjustment strategies have been proposed to improve the syn

Extracorporeal mechanical circulatory assist.

There are currently several safe and effective options to provide temporary MCS for patients presenting with cardiogenic shock or refractory heart failure. Newer device designs are currently being developed that will increase the options available to patients. Due to the technological advancements, it will be difficult to predict what devices will ultimately prove to be the most efficacious. It is likely that a variety of devices will be necessary, depending on clinical circumstances and patient characteristics.

The conservative management of first trimester miscarriages and the use of colour Doppler sonography for patient selection.

This study of patients with first trimester miscarriage evaluates whether conservative management is a feasible strategy and assesses the value of colour Doppler ultrasonography for patient selection. After confirmation of the diagnosis by transvaginal sonography all patients were offered the choice of immediate dilatation and curettage or conservative management. The presence of a gestational sac, the occurrence of spontaneous complete miscarriage within 28 days, detectable pulsatile blood flow within the placenta in the presumed region of the intervillous space and post-treatment complicatio

Investigation of the mechanism of soft tissue conduction explains several perplexing auditory phenomena.

Soft tissue conduction (STC) is a recently expounded mode of auditory stimulation in which the clinical bone vibrator delivers auditory frequency vibratory stimuli to skin sites on the head, neck, and thorax. Investigation of the mechanism of STC stimulation has served as a platform for the elucidation of the mechanics of cochlear activation, in general, and to a better understanding of several perplexing auditory phenomena. This review demonstrates that it is likely that the cochlear hair cells can be directly activated at low sound intensities by the fluid pressures initiated in the cochlea;

Effects of intra-aortic balloon pumping on graft flow in coronary surgery: an intraoperative transit-time flowmetric study.

BACKGROUND: We investigated hemodynamic effects of intra-aortic balloon pumping (IABP) in in-situ and aorta-coronary (A-C) grafts during coronary artery bypass grafting (CABG). METHODS: One hundred seventy-two grafts, including 84 in-situ left internal thoracic arteries (LITAs), were examined intraoperatively with a transit-time flowmeter in 84 patients who had prophylactic IABP. The following measurements were obtained for each graft during off-IABP and on-IABP: mean flow, maximal flow, pulsatility index, and diastolic filling index. Coronary angiograms were performed 14 +/- 5 days after cor

Effect of external cephalic version at term on fetal circulation.

OBJECTIVE: We sought to investigate the subclinical effect of external cephalic version on fetal circulation. STUDY DESIGN: A prospective observational study was conducted on 136 subjects who had external cephalic version at or beyond 36 weeks of gestation without clinical complication. Doppler ultrasonographic studies of the umbilical and middle cerebral circulations were performed before and after the external cephalic version. The following Doppler indexes were measured: (1) the pulsatility index of the umbilical artery, which represents disturbance of placental circulation, and (2) the pu

Doppler evaluation of the effects of propofol, etomidate and alphaxalone on fetoplacental circulation hemodynamics in the pregnant ewe.

INTRODUCTION: Umbilical artery (UA) hemodynamics reflect blood flow and vascular resistance in the placental circulation. We examined non-invasively the hemodynamic effects of propofol, etomidate, and alphaxalone on the placental circulation of a sheep model by means of UA Doppler ultrasonography. METHODS: Eleven sheep fetuses were examined at 90-109 days of gestation. UA Doppler ultrasound was performed before and after administration of a single intravenous bolus of propofol, etomidate, or alphaxalone. UA Doppler velocities (peak systolic velocity, end diastolic velocity, and mean velocity)

Pulse Doppler ultrasound measurement of central artery elasticity: an exploratory study.

BACKGROUND: Arterial stiffness, as a biomarker of cardiovascular diseases, is most often described by a surrogate global stiffness index, pulse pressure wave velocity (PWV), a measure of the time for pressure pulses to transit to sensors normally placed on different arteries. However, local arterial elasticity also influences the transient response of an artery to an impulse of flow, as characterized by the local pulsatile flow velocity waveform. Such transient response to pulsatile flow has not previously been analyzed as an indicator of arterial stiffness and a biomarker of cardiovascular di…

Flow Analysis of Central Venous Outflow Tract: A New Approach to Understanding Pulse-Synchronous Tinnitus.

OBJECTIVE: Pulse-synchronous tinnitus (PST) has been linked to multiple anatomical variants of the central venous outflow tract (CVOT) including sigmoid sinus (SS) dehiscence and diverticulum. This study investigates flow turbulence, pressure, and wall shear stress along the CVOT and proposes a mechanism that results in SS dehiscence and PST. STUDY DESIGN: Case series. SETTING: Tertiary Academic Center. METHODS: Venous models were reconstructed from computed tomography scans of 3 patients with unilateral PST. Two models for each patient are obtained: a symptomatic and contralateral asymptom

The effects of smoking on the relationship between metabolic syndrome and arterial stiffness.

It has been reported that arterial stiffness assessed by pulse wave velocity (PWV) can predict cardiovascular morbidity and mortality. Metabolic syndrome and smoking are independently associated with increased risk of cardiovascular disease. The purpose of this study was to evaluate the effect of smoking on the relationship between metabolic syndrome and arterial stiffness. We retrospectively enrolled 1,530 men on whom brachial-ankle pulse wave velocity (baPWV) measurement had previously been performed as part of health screening at Kosin University Hospital. We found that PWV was positively c

[Normal values of cerebral blood flow velocities in neonates].

INTRODUCTION: There was used color Doppler ultrasonography (cD-USI), allowing simultaneous examination of parenchymal and vascular cerebral structures. The evaluation of blood flow velocities in cerebral arteries is important in the assessment of cerebral circulation in hypoxic-ischaemic and haemorrhagic brain damage in neonates. OBJECTIVE: The aim of this study was to estimate normal values of cerebral blood flow velocities (CBFV) and Doppler indices--pulsatility index (PI) and resistance index (RI)--in the anterior cerebral artery (ACA) during the first days of life in infants. METHODS: CB

Visual and blood flow responses in low-tension glaucoma.

Low or normal-tension glaucoma (NTG) is a rare clinical condition characterized by a glaucomatous cupping of the optic disk associated with the appearance of typical visual field defects, without ocular hypertension. According to several authors, NTG accounts for 5-15% of primary open angle glaucomas (POAG). In our open study we obtained a complete documentation of clinical and instrumental data-tonometric curve, computerized perimetry, pattern electroretinogram, pulsatile ocular blood flow (POBF) and retinography--on 10 consecutive patients (18 eyes) affected by NTG. We found that the NTG pat

Arterial hemodynamic indexes in Marfan's syndrome.

Aortic impedance, wave reflection magnitude, and compliance were measured in patients with Marfan's syndrome during diagnostic cardiac catheterization. Impedance and wave reflections were calculated from standard Fourier series analysis of ascending aorta micromanometer pressure and electromagnetic flow records. Compliance was estimated by a method recently proposed that uses the area under the pressure-time curve assuming a two-element Windkessel model of the circulation. Measurements were made in the baseline state, during vasodilatation with nitroprusside, after beta-adrenergic receptor blo

[Prognostic Doppler ultrasound examination of fetal arteries blood flow].

UNLABELLED: Early detection of fetal risk is one of the main issues in today obstetrics. Ultrasound diagnostics plays a significant role, as the introduction of Doppler imaging method in the evaluation of blood flow has enabled non-invasive assessment of uteroplacental circulation. Therefore, we have analysed foetal three arteries: umbilical artery, middle cerebral artery and renal artery after determining the normal range for the analysed parameters. AIM OF WORK: 1. Comparison of the obtained blood flow indices (S/D, RI, PI) in the umbilical artery, middle cerebral artery and renal artery of

Hemodynamic analysis of left ventricular unloading with Impella versus IABP during VA-ECMO.

Purpose: The utilization of intra-aortic balloon pump (IABP) and Impella has been suggested as means of left ventricular unloading in veno-arterial extracorporeal membrane oxygenation (VA-ECMO) patients. This study aimed to assess the local hemodynamic alterations in VA-ECMO patients through simulation analyses. Methods: In this study, a 0D-3D multiscale model was developed, wherein resistance conditions were employed to define the flow-pressure relationship. An idealized model was employed for the aorta, and simulations were conducted to contrast the hemodynamics supported by two configuratio

Cerebrospinal Fluid and Cerebral Blood Flows in Idiopathic Intracranial Hypertension.

OBJECTIVES: Cerebrospinal fluid (CSF) and blood flows have a strong relationship during a cardiac cycle. Idiopathic intracranial hypertension (IIH) is a pathology that seems to present hemodynamic and hydrodynamic disturbance. The aim of this study was to establish CSF and blood interaction in IIH. MATERIAL AND METHODS: We retrospectively studied cerebral hydrodynamic and hemodynamic flows by phase-contrast MRI (PCMRI) in 13 IIH subjects (according Dandy's criteria) and 16 controls. We analyzed arterial peak flow, pulsatility index, and resistive index in arterial and venous compartments (PFa

In Xenopus ependymal cilia drive embryonic CSF circulation and brain development independently of cardiac pulsatile forces.

BACKGROUND: Hydrocephalus, the pathological expansion of the cerebrospinal fluid (CSF)-filled cerebral ventricles, is a common, deadly disea … nitiation of the cilia driven CSF circulation where ependymal cilia create a polarized CSF flow. No pulsatile flow was detected throughout these tailbud and early tadpole stages. As development progressed, despite the emergence of the choroid plexus in Xenopus, cardiac forces did not contribute to the CSF circulation, and ciliary flow remained the driver of the …
