Q fever, a zoonotic disease caused by the bacterium Coxiella burnetii, can cause acute or chronic illness in humans. Transmission occurs primarily through inhalation of aerosols from contaminated soil or animal waste. No licensed vaccine is available in the United States. Because many human infections result in nonspecific or benign constitutional symptoms, establishing a diagnosis of Q fever often is challenging for clinicians. This report provides the first national recommendations issued by CDC for Q fever recognition, clinical and laboratory diagnosis, treatment, management, and reporting …

INTRODUCTION: Coxiella burnetii, the causative agent of Q fever, is a notifiable zoonotic pathogen in Ireland. While typically subclinical in ruminants, infection is associated with reproductive losses. In humans, disease can range from asymptomatic to more serious complications. Ruminants have been identified as the main reservoir for human infection. Ireland’s dairy cattle industry has expanded substantially in recent years, yet current data on the national prevalence of Coxiella burnetii in dairy herds are limited. Understanding the herd-level prevalence and associated risk factors is essen…

Q fever, also known as query fever, is a naturally occurring zoonotic disease caused by the bacterium Coxiella burnetii. This paper reported the case of a 37-year-old male patient who developed symptoms of headache, fever, and dyspnea after delivering yaks. Having received ineffective treatment at other medical facilities, he was referred to Tibet Autonomous Region People's Hospital. A diagnosis of Q fever was confirmed through an etiological examination of bronchial lavage sample. The patient was subsequently treated with mechanical ventilation and effective antibiotics. Severe pneumonia resul…

BACKGROUND: Q fever is a zoonotic bacterial infection caused by Coxiella burnetii that is reportable in the USA. This infection is often asymptomatic; acute infection usually manifests as a self-limited febrile illness, hepatitis, or pneumonia. Chronic infection (usually infective endocarditis) often affects patients with valvulopathy or immunosuppression. Herein, we study the inpatient frequency of Q fever in the United States.
METHODS: We used a nationwide inpatient sample (NIS) for our retrospective cohort study to include hospitalizations with a diagnosis of Q fever between 2010 and 2019.…

Q fever is an emerging infectious disease in Europe. Q fever is a zoonosis and infected animals are the main source of infection. Ticks may act as a vector and transmit the pathogen to animals and humans. Q fever has non-specific symptoms and is difficult to diagnose. Results of serological tests are positive many days after manifestation of symptoms. PCR method might be useful in the diagnostic process.

Q fever is an anthropozoonosis whose causative agent is Coxiella burnetii, which has an important impact from the human and animal health point of view. In this review, a brief historical reference of the infection by C. burnetii and Q fever has been made initially. In a second section the basic epidemiological aspects of this infection are described (reservoirs/ sources of infection, form of transmission and epidemiological forms). Subsequently, the data of the infection by C. burnetii in Spain will be are indicated, particularly the clinical series, the seroepidemiological studies in humans,…

Coxiella burnetii is the etiological agent of Q fever, a worldwide zoonosis that can result in large outbreaks. The birth of genomics and sequencing of C. burnetii strains has revolutionized many fields of study of this infection. Accurate genotyping methods and comparative genomic analysis have enabled description of the diversity of strains around the world and their link with pathogenicity. Genomics has also permitted the development of qPCR tools and axenic culture medium, facilitating the diagnosis of Q fever. Moreover, several pathophysiological mechanisms can now be predicted and therap…

Q fever has rarely been reported and can be difficult to diagnose, especially in immunocompromised patients. In the present report, we describe an unusual case of Q fever that presented as peritonitis and was treated with long-term combination therapy with doxycycline, ciprofloxacin and rifampicin for five weeks in a patient who had been on peritoneal dialysis for six years due to hypertensive nephropathy.

Osteoarticular infection is an uncommon presentation of Q fever. Positron emission tomography (PET) scanning is a valuable tool for the diagnosis of Coxiella burnetii graft prosthesis infection and endocarditis. Our objective was to test a series of culture-negative osteoarticular samples using molecular assays for Coxiella burnetii. We tested for C. burnetii by molecular assays targeting the IS1111 and the IS30A spacer regions, using culture-negative osteoarticular samples obtained in our laboratory between January 2011 and December 2012. We examine a total of 1,410 osteoarticular samples, an…

OBJECTIVES: The Q fever skin test is used to measure cell-mediated immunity to Coxiella burnetii in pre-vaccination screening to exclude individuals with pre-existing immunity. We investigated whether this in-vivo test influences subsequent measurements of immune response.
METHODS: We assessed the humoral and cellular immune responses before, and 6 and 12 months after skin testing in 63 individuals who were not vaccinated because of either a positive skin test or positive serology in screening. IgG anti-C. burnetii antibodies were measured using immune-fluorescence assay (IFA). The cellular i…

In 2005, Q fever was diagnosed on two dairy goat farms and 2 years later it emerged in the human population in the south of the Netherlands. From 2007 to 2010, more than 4,000 human cases were notified with an annual seasonal peak. The outbreaks in humans were mainly restricted to the south of the country in an area with intensive dairy goat farming. In the most affected areas, up to 15% of the population may have been infected. The epidemic resulted in a serious burden of disease, with a hospitalisation rate of 20% of notified cases and is expected to result in more cases of chronic Q fever a…

As neglected zoonosis for many years, Q fever is now ubiquitous in Japan. Similarly to elsewhere in the world, domestic animals are considered to be important reservoirs of the causal agent, Coxiella burnetii, a resistant intracellular bacterium. Infected animals shed bacteria in milk, feces, urine, vaginal mucous and birth products. Inhalation of bacteria present in the environment is the main route of animal and human infection. Shedding of C. burnetii in milk by domestic ruminants has a very limited impact as raw milk is seldom ingested by the Japanese population. The clinical expression of…

Coxiella burnetii is the etiologic agent of Q fever, a worldwide distributed zoonosis, accountable for serious health problem both for humans and animals. The exposure to C. burnetii infected animals and their products is the main risk factor for Q fever in humans. Several outbreaks of Q fever have been described in Poland which sources were recognized to be related to imported animals and their products or to wildlife using serological methods. Moreover, some of them have been confirmed by isolation of C. burnetii strains. In this study, multispacer sequence typing (MST) and multiple loci var…

Ecological and epidemiological features of Q-fever throughout its study are represented. The fact of transformation of Q-fever natural focuses to antropurgical focuses is proved. It is established that intraherd focuses, determining epidemiological status of Q-fever, have great persistence. Significant economical loss due to recurrence and chronization of the infection are proved, economical loss from one case of the disease is calculated.

Infections due to Coxiella burnetii, the causative agent of Q fever, are uncommon in the United States. Cases of chronic Q fever are extremely rare and most often manifest as culture-negative endocarditis in patients with underlying valvular heart disease. We describe a 31-year-old farmer from West Virginia with a history of congenital heart disease and recurrent fevers for 14 months who was diagnosed with Q fever endocarditis based on an extremely high antibody titer against Coxiella burnetii phase I antigen. Despite treatment with doxycycline, he continued to have markedly elevated Coxiella …

Q fever is a zoonosis caused by Coxiella burnetii. Infection with C burnetii can be acute or chronic, and exhibits a wide spectrum of clinical manifestations. The extreme infectivity of the bacterium results in large outbreaks and makes it a potential bioweapon. In the past decade, the complete genome sequencing of C burnetii, the exploration of bacterial interactions with the host, and the description of the natural history of the disease in human beings and in experimental models have all added to our knowledge about this fascinating disease. Advances in understanding the pathophysiology and…
