[Osteosynthesis of combined radius head and capitulum humeri fractures with mini-implants].

INTRODUCTION: Combined injuries of the capitellum humeri and the radial head are rare. Most of them show combined osteocartilaginous lesions and collateral ligament lesions. Recommendations for treatment of these fatal injuries of the elbow are missing. TYPE OF STUDY: Five cases were investigated in an retrospective analysis. The same approach was used for the osteocartilaginous lesion of the capitellum as well as the injury of the radial head. Internal fixation was done with mini-titanimplants, three of them combined with resorbable pins. PATIENTS AND METHODS: Between 1996-1999 five patient

Initial experience with the Forte plate for dorsally displaced distal radius fractures.

Open reduction and plate osteosynthesis is occasionally indicated for dorsally displaced distal radius fractures. We reviewed our medium term results with the Forte plate, one of the recently introduced purpose-made implants.Twenty-five patients operated on during the first year were reviewed 19 (12-24) months after surgery. Median age at operation was 53 (28-80) years. There were seven high energy and eighteen low energy injuries. Fourteen fractures extended into the radiocarpal joint.Three patients had a poor clinical result and were re-operated on before review with an arthrodesis, ulnar sh…

Self-reported disability following distal radius fractures: the influence of hand dominance.

The purpose of this study was to record the spectrum of self-reported disability following distal radius fractures and to gauge for differences in hand dominance in the use of subjective outcome data. Items were generated through patient interviews, literature review, and peer consultation. Fifty-three items were evaluated by a group of 55 patients recovering from a fracture of the distal radius, which established the prevalence, mean severity score, and overall severity score (or impact) of each item as it related to physical function and social/emotional impact. Hand dominance, age, and gend…

Salter-Harris I fractures of the distal radius misdiagnosed as wrist sprain.

Over a period of 5 years, 65 children under 12 years of age were diagnosed as having "wrist sprain" at the American University of Beirut Medical Center, Emergency Department. Thirty-eight children were reevaluated; 33 patients (86.8%) were diagnosed as having a Salter-Harris I fracture of the distal radius that necessitated further treatment to relieve symptoms and allow early wrist function. The diagnosis of "wrist sprain" is often inadequate in establishing a proper treatment regimen. Rather, by taking a careful history, performing an exacting examination, and utilizing appropriate diagnosti

Biomechanical evaluation of wrist motor tendons after fractures of the distal radius.

We conducted a biomechanical study of changes in parameters of wrist motor tendons in fractures of the distal radius in 7 cadaveric extremities. Extra-articular distal radius fractures were simulated by distal radius osteotomy and fracture angulation was maintained by external fixators. Eight positions of the distal radius fractures were studied: dorsal angulation of 10 degrees, 20 degrees, 30 degrees, and 40 degrees and radial angulation of 5 degrees, 10 degrees, 15 degrees, and 20 degrees. Dorsal and radial angulation of the fractures were measured with respect to the shaft of the radius. Ex…

Treatment of unstable distal radius fractures with cancellous allograft and external fixation.

Unstable fractures of the distal radius continue to pose a challenge to the hand surgeon. Adjunctive bone grafting is often required to augment structural integrity and aid healing. Because of the risks inherent to bone autograft harvest, however, freeze-dried, irradiated cancellous bone allograft has been used to treat unstable distal radius fractures with severe metaphyseal comminution. Seventeen patients with such fractures (mean age, 70 years; 2 males and 15 females) were treated with bone allograft and external fixation with or without internal fixation. The outcome was evaluated using th…

Wire-loop fixation of volar displaced osteochondral fractures of the distal radius.

We describe the technique of wire-loop fixation to treat 4 young men with a unique variant of Melone's type 4 articular fracture of the volar lunate facet, characterized by a displaced rotated articular fragment supported by a limited amount of subchondral bone. This is an unusual injury that threatens the integrity of both the radiocarpal and distal radioulnar joints. It is the result of a compressive force to the wrist and may require trispiral computed tomography for delineation. Open reduction and internal fixation is recommended to maintain stability and articular congruity. The displaced

Un-united fractures of the distal radius: a report of 12 cases.

The treatment of 12 distal radius nonunions in 11 patients over a 24-year period is presented. The average age of the patients was 55 years (range, 35-72 years). The comorbid medical conditions in the patients with these fractures included diabetes mellitus, peripheral vascular disease, psychiatric disorders, alcoholism, peripheral neuropathy, scleroderma, and morbid obesity. Nine of the un-united fractures in 8 patients had insufficient metaphyseal bone to allow internal fixation; 6 of these fractures were treated with a wrist arthrodesis. Three un-united fractures in 3 patients had sufficien

Closed reduction and external fixation of unstable fractures of the distal radius.

The anatomical and functional outcome and complications after closed reduction and external fixation of unstable fractures of the distal radius were reviewed in 29 patients. The fractures were reduced by longitudinal traction and closed manipulation under fluoroscopic control, and the reduced position was retained by a standard half-frame Hoffmann external fixator for six to eight weeks. The mean follow-up time was four (3-7) years. The reduction failed in three patients after too early removal of the fixator (three to five weeks). For the rest of the patients the dorsal angulation, the radial

Distal radius fractures: a prospective randomized comparison of fibreglass tape with QuickCast.

Cast immobilization remains the most commonly employed method in the treatment of distal radius fractures. In an effort to test a new shrinkable polymer-coated casting material (QuickCast), a prospective randomized study was performed on 30 consecutive distal radius fractures. Fractures were divided into three groups based on the severity of fracture and treated with either a conventional short-arm fibreglass tape cast or a QuickCast. Results showed no significant differences in minor cast complications or maintenance of fracture reduction between conventional fibreglass tape and the QuickCast…

Can cast immobilization successfully treat scapholunate dissociation associated with distal radius fractures?

During a 3.5-year period, 20 of 424 consecutive patients with fractures of the distal radius presented with evidence of scapholunate dissociation upon x-ray films and traction view fluoroscopy. The sequential changes of x-ray abnormalities of the scapholunate joint were consistently observed over a 1-year period, and wrist functions were evaluated 1 year after injury. The scapholunate gaps were 3.5 +/- 0.5 mm at the time of injury, 3.2 +/- 0.4 mm immediately after closed reduction of the fracture, 3.4 +/- 0.5 mm at the time after removal of fixation, and 3.8 +/- 0.4 mm 1 year after injury. [Retrospective analysis of consecutively treated distal radius fractures with the external fixator].

The purpose of this study was to determine retrospectively some subjective and objective parameters following the operative treatment of 32 patients with "complex" (intraarticular and/or comminuted) fractures of the distal radius using the small AO external fixation device. The mean follow-up period was 20 months (minimum 4, maximum 48) and the mean age of the subjects was 62 years (minimum 27, maximum 91). Subjective results such as "general feeling", mobility, strength and pain, expressed with a scoring system (% of maximum obtainable points), showed an overall good result and ranged between

Indications and techniques of open reduction. Internal fixation of distal radius fractures.

Complex distal radius fractures present in a variety of patterns that differ by the area and degree of involvement of the articular and metaphyseal fracture components. No single method of treatment is appropriate for all fractures. This article discusses the types of fractures that either require or are best handled by formal open reduction and internal or external fixation.

Early clinical experience with Orthofix external fixation of complex distal radius fractures.

Twelve patients with complex fractures of the distal radius were treated with the Orthofix external fixation system (EBI Medical Systems, Fairfield, NJ). On follow up physical examination, the average wrist dorsiflexion was 46 degrees, and the average volar flexion was 55 degrees. The average grip strength in the involved hand was 38 lb force. The average final radial angle was 18 degrees, and the average residual radial height was 12 mm. Average volar tilt was 3 degrees. The complication rate was 42%. Very rigid axial stability is afforded by the fixator, but the large 3.5 mm tapered bone scr

Compression-plate fixation in acute diaphyseal fractures of the radius and ulna.

At the Campbell Clinic and City of Memphis Hospital from 1960 to 1970, 244 patients (216 with closed and twenty-eight with open fractures) had 330 acute diaphyseal fractures of the radius and ulna which were treated with ASIF compression plates and followed for from four months to nine years. One hundred and twelve patients had fractures of both bones of the forearm; fifty, single fractures of the ulna; and eighty-two, single fractures of the radius. In all, 193 fractures of the radius and 137 fractures of the ulna were treated by compression plating. Sixty-three patients (25.9 per cent) with

Enhancing Functional Outcomes in Distal End Radius Fracture Rehabilitation.

INTRODUCTION: From a physiotherapist's point of view, helping patients recover from a distal end radius (DER) fracture is a big part of getting them back to their daily lives. This article highlights how important a structured, step-by-step rehabilitation program is for adults, whether their fracture was treated with a cast or with surgery. As physical therapists, our role is crucial in guiding this recovery, working closely with orthopedic surgeons. MATERIALS AND METHODS: We understand that there is not one perfect exercise plan for every patient, but starting exercises early is always bette

Recovery in the symmetry of hand use after distal radius fracture.

INTRODUCTION: Hand use recovery after a distal radius fracture (DRF) depends on whether the injured hand is dominant or not, which can affect laterality and influence functional outcomes. This study aimed to investigate how the injured side impacts changes in laterality and its relationship to functional outcome, aiming to contribute to the development of future hand therapy strategies. METHODS: Patients with DRF were prospectively recruited. Triaxial accelerometers were wrist-mounted to collect data at 1, 4, 8, and 12 weeks postoperatively and examine the laterality index (LI), total hand us

Literature Review of Postoperative Distal Radius Fracture Immobilization Recommendations.

Distal radius fractures (DRFs) are a commonly treated injury in orthopedics. DRFs have a high incidence across patient demographics, including pediatrics, young patients in high-energy trauma, and geriatric patients in low-energy trauma. While DRFs occur across a large range of age groups, they are especially consequential in geriatric patients with osteoporosis. Management of DRFs has extensive variability, ranging from conservative casting to surgical interventions, such as open reduction and internal fixation surgical procedures. The diversity of treatment options for DRFs is due to a consi…

Real-time biofeedback monitoring rehabilitation of distal radius fracture.

BACKGROUND: Elderly patients often face challenges in recovering from distal radius fractures (DRFs), and inadequately guided rehabilitation may lead to delayed healing or secondary injury. OBJECTIVE: This study aims to develop a system that monitors patients during rehabilitation exercises and provides real-time biofeedback to improve healing outcomes and minimize the risk of secondary injuries. METHOD: Volunteers performed a 10-second sequence including relaxation, flexion/extension, radial/ulnar deviation, and fisting. During these exercises, time series data of hand motions and correspon…
