Axial medial plantar rotation flap with medial plantar artery or its perforators for reconstructing soft tissue defects in plantar heel: Anatomical observation and report of a clinical series of 11 flaps.

INTRODUCTION: Limited information is available on axial medial plantar rotation (AMPR) flaps. This study aimed to evaluate the clinical outcomes of the AMPR flaps with the medial plantar artery (MPA) or medial plantar arterial perforators (MPAPs) for reconstructing soft tissue defects in the plantar heel and explore their vascular basis. METHODS: Three perfused amputated specimens were analyzed to identify fasciocutaneous perforators in the intermuscular septum between the abductor hallucis and flexor digitorum brevis muscles. Nine AMPR flaps with the MPAPs and two with the MPA were used for

Lymph-interpositional-flap transfer (LIFT) based on lymph-axiality concept: Simultaneous soft tissue and lymphatic reconstruction without lymph node transfer or lymphatic anastomosis.

BACKGROUND: Lymphatic system is important to maintain homeostasis. Lymph-axiality concept has been reported, which suggests possibility of lymphatic reconstruction using flap transfer without lymph node or supermicrosurgical lymphatic anastomosis. METHODS: Medical charts of 122 free flap reconstruction cases, either with conventional flap transfer (control) or lymph-interpositional-flap transfer (LIFT), for extremity soft tissue defects including lymphatic pathways were reviewed. Lymph vessels' stumps in a flap were placed as close to those in a recipient site as possible under indocyanine gr

The laparoscopically harvested omentum as a free flap for autologous breast reconstruction.

BACKGROUND: In autologous breast reconstruction, abdominal based flaps are by far the most common choice from the wide range of free flaps available. In selected cases, a laparoscopically harvested omental free flap (LHOFF) can be used. Patient satisfaction has not been reported until now. In this article, we report our experience using LHOFF in breast reconstruction including our technique, patient satisfaction, and donor/recipient site complications. PATIENTS AND METHODS: Between 2007 and 2014, six patients underwent autologous breast reconstruction with LHOFF. Four patients had undergone r

Outcome of kidney transplantation from young pediatric donors (aged less than 6 years) to young size-matched recipients.

INTRODUCTION: Pediatric donation is underutilized because of presumed increased risk of vascular thrombosis (VT) and graft loss. Using young pediatric donors (YPDs) for young pediatric recipients (YPRs) is suggested to be even at greater risk and therefore precluded in many centers. The aim of this study was to analyze the outcome of kidney transplantation (KT) from YPD to age-matched YPR. PATIENT AND METHODS: A retrospective study of 118 pediatric KT performed between January 2007-July 2017. The authors identified KT with YPD (considered as those aged <6 years) and age-matched YPR. Ultrastructural organization of normal and transplanted rat fascia dentata: II. A quantitative analysis of the synaptic organization of intracerebral and intraocular grafts.

As part of an ultrastructural analysis of the normal rat fascia dentata and intracerebral and intraocular dentate transplants the synapses in the dentate molecular layer were quantified. Hippocampal and dentate tissue from 21-day-old rat embryos were grafted into the brain of developing and adult rats and to the anterior eye chamber of adult rats. After 100 or 200 days of survival the recipient rat brains and the recipient eyes were processed for electron microscopy, and the graft dentate molecular layer with the adjacent granule cell layer selected for ultrastructural analysis. Candida graft arteritis after kidney transplantation: two cases and literature review.

BACKGROUND: Candida arteritis of a kidney allograft represents a severe yet rare complication in transplant recipients. Its nonspecific presentation and diagnostic difficulties necessitate a high level of clinical suspicion and a multidisciplinary approach to management. CASE PRESENTATION: We report two cases of Candida arteritis in kidney transplant recipients who presented with life-threatening bleeding from the graft anastomotic site shortly after transplantation. Histopathological examination revealed fungal invasion of the arterial walls, with periodic acid-Schiff (PAS) staining demonstr

Further evidence that NK cells may contribute to the development of cardiac allograft vasculopathy.

In prior experiments, we found that recipients, even though specifically tolerant of donor antigens, will develop striking cardiac allograft vasculopathy (CAV) in allogeneic mouse heart transplants. This suggested that innate immune responses, in addition to conventional adaptive immunity, may be involved in the development of CAV. We accordingly performed transplants in the parental-to-F1 combination seeking supportive evidence of NK cell activity directed against the vessels of parental donor transplants as a manifestation of "hybrid resistance." When such lesions were indeed found, we inves

The histo-blood group ABO system and tissue transplantation.

In general, one might expect that ABO incompatibility of donor and recipient would be important to some degree if viability of the transplanted allograft is important for graft incorporation and function. This is true for some recipients of organs. However, ABO incompatibility appears to play a minor role, if any, in the clinical success of viable cornea and viable skin allografts. Even though A and B antigens may be present on the transplanted tissue, other factors that can contribute include the strength of the immune response, the avidity of the antibody, and the dose of the antigen present

Hypothermic machine perfusion for uterus transplantation.

OBJECTIVE: To describe the feasibility of hypothermic machine perfusion (HMP) in uterus transplantation (UT) to potentially improve the preservation of the uterus and enhance graft preservation in the donation after brainstem death (DBD) context. Uterus transplantation is a new surgical approach to treating absolute uterine infertility; it can be performed after living donation or after DBD. In the DBD context, the uterus is typically the last organ removed after other vital organs, with the exception of the Baylor team, which removes the uterus first. This key aspect imposes an unavoidable mi

[Veno-occlusive disease of the liver].

Veno-occlusive disease (VOD) is seen most often in the group of bone marrow transplant recipients. The essence of this disease is the obstruction of the hepatic sinusoidal and centrolobular venous outflow, because of the injury to the endothelium of the liver vessels. It results in congestion of the liver and hepatomegaly. The typical clinical symptoms of VOD are: jaundice, portal hypertension with peripheral oedemas and the weight gain. Depending on the extent of the injury of the hepatic vessels, VOD is divided into three grades: mild, moderate and severe. The clinical markers that inform ab

The usefulness of a single measurement of insulin-like growth factor-1 as a predictor of embryo yield and pregnancy rates in a bovine MOET program.

The objective was to determine if a single measurement of plasma insulin-like growth factor-1 (IGF-1) could predict the number of viable embryos obtained from donors and the likelihood of pregnancy in recipients in multiple ovulation and embryo transfer (MOET) programs in cattle. The embryo yields from 101 embryo recoveries were examined in maiden Holstein heifers (n=75) and multiparous Holstein cows (lactating cows n=20, dry cows n=6). Donors were super stimulated with FSH and embryo recovery was done non-surgically 7 days after artificial insemination. Embryos were classified according to th

A solution to the vessel shortage during free vascularized fibular grafting for reconstructing infected bone defects of the femur: Bridging with vein transplantation.

PURPOSE: The present study aimed to evaluate the feasibility and clinical efficacy of bridging vein transplantation to deal with the vessel shortage during free vascularized fibular grafting for reconstructing infected bone defects of the femur. METHODS: Twelve patients (aged 15-58 years) with infected bone defects of the femur (between 6.0 and 18.0cm) were recruited in this study. Vacuum sealing drainage were applied after extensive debridement of the infected bone defects and irrigated with 0.9% sodium chloride solution for 1-2 weeks. After the drainage was clear and the focal infections we

Soluble donor-like MHC class I proteins induce CD4(+)CD25(+)CD8(-)FoxP3(+) cells with potential to ameliorate graft chronic injury.

Conventional immunosuppressive therapies failed to prevent allograft chronic rejection. New approaches to modulate recipient immune response are needed. Donor-like MHC class I soluble proteins demonstrated therapeutic potential to suppress chronic rejection. The present study was designed to clarify the ability of MHC class I soluble proteins to induce T regulatory cells with true regulatory potential in a fully allogeneic rat cardiac transplant model. Donor-like MHC class I proteins upregulate small population of splenic CD8(-) negative CD4(+)CD25(+)FoxP3(+) positive cells. CD4(+) splenocytes

Vascularized Lymphatic Tissue Transfer for Treatment of Lower Limb Lymphedema: Does the Recipient Site Matter?

INTRODUCTION: Vascularized lymph node transfer (VLNT) is a pivotal component in lower limb lymphedema (LLL) treatment. We aimed to assess the association of using the groin and ankle as single or multilevel recipient sites on both limb volume reduction and recipient site morbidity. METHODS: A total of 51 patients with LLL received VLNT between 2016 and 2023. We assessed volume preoperatively and postoperatively over a 12-mo period. We compared limb volume changes in primary lymphedema (PLE) and secondary lymphedema (SLE) subgroups depending on recipient sites: groin, ankle, and multilevel (gr

Conventional liposuction-assisted debulking of muscle perforator flaps.

Late recontouring of the usual bulky cutaneous flap via conventional liposuction is an accepted practice and usually without fear of tissue necrosis. Muscle perforator flaps, on the other hand, initially depend only on often fragile, minuscule vessels that could readily be traumatized by any form of suction-assisted lipectomy. Single-staged muscle perforator flap debulking was thus performed with some trepidation for 8 flaps in 7 patients. All flaps ultimately remained successful, but in 2 cases obvious tissue ischemia delayed healing. It can only then be assumed that this adjunctive technique

Tensor fasciae latae flap: alternative donor as a functioning muscle transplantation.

The functioning tensor fasciae late muscle was used for a patient with both a complete defect of the deltoid muscle and a defect of overlying skin. The configuration of the tensor fasciae latae including the length of the muscle belly as well as the muscle fiber arrangement was similar to that of the deltoid. In addition, the spatial relationship between the muscle, donor vessels, and motor nerve fulfilled the requirements for the recipient site of deltoid reconstruction. The transferred muscle successfully replaced the function of the deltoid and provided sufficient strength for elevation of
