Prevalence and characterization of neuropathic pain in a primary-care setting in Spain: a cross-sectional, multicentre, observational study.

BACKGROUND AND OBJECTIVE: Different studies have shown pain to be one of the most frequent causes of health-care resource utilization, and a major public health concern because of its social repercussions. In Spain there are no recent data on the prevalence and management of neuropathic pain in the general primary-care setting. METHODS: This study aimed to gain epidemiological insight into neuropathic pain in the Spanish primary-care setting. This was a cross-sectional, one-day, multicentre, observational epidemiological study involving 623 primary-care physicians in Spain. Patients who exper

[Cervicogenic headache: etiopathogenesis, characteristics, diagnosis, differential diagnosis and therapy].

The term "cervicogenic headache" (CH) implies a chronic hemicranial pain syndrome caused by upper cervical spine disorders. According to the clinical researches, in 15-20% of the patients with chronic unilateral headache, it is the case of the headache of cervical origin. The sources of the referred pain manifested as CH are the disorders of anatomical structures innervated by the first three cervical spinal nerves and/or direct irritation/lesion of these nerves (spinal nerves C1-C3, intervertebral/i.v. joints C0-C3, i.v. disc C2-C3 muscles, ligaments, bony structures, dura mater, vertebral ar…

Cervical musculoskeletal impairments in migraine.

BACKGROUND: Neck pain is common and disabling amongst individuals with migraine. Cervical musculoskeletal interventions are often sought but there is currently no evidence to support such interventions for this population. Improved understanding of how cervical musculoskeletal impairments present in migraine can elucidate neck pain mechanisms and guide clinicians and researchers in the management of patients with migraine and neck pain. MAIN BODY: Migraine hypersensitivity is a major consideration when assessing for cervical impairments as it can aggravate migraine and confound findings. Curr

A Rare Complication of Temporomandibular Joint Viscosupplementation Associated With the Intra-Articular Agent: A Case Report.

Viscosupplementation, which has been referred to as intra-articular injections of hyaluronic acid, is preferable in the treatment of patients with degenerative disease of the temporomandibular joint who have not responded to conservative therapy. The complications of intra-articular injections are reported as pain, swelling, heat, rash, itching, bruising, or redness. This article describes a patient who underwent arthrocentesis followed by viscosupplementation and subsequently experienced pain and malocclusion related to the applied agent.

Plantar fasciitis.

Thirty patients presenting with the heel pain syndrome, commonly referred to as "plantar fasciitis", were studied prospectively over a two year period. The pain was associated with a calcaneal spur in 21 patients (70%). In a control series of 25 patients without heel symptoms, calcaneal spurs were present in only 4 out of 50 heels (8%). This difference is highly significant (p 0.001). Seven patients (22%) in the plantar fasciitis group complained of ipsilateral sciatica.

[Acute jaw-thoracic pain and syncope in a 41-year-old man].

A 41-year-old physically active man with no significant past medical history presented with sudden thoracic pain. The patient was referred to the next tertiary care hospital. A CT scan showed an ectasia of the ascending aorta with irregularities of the aortic wall without dissection. Despite initial refusal, the patient was referred to a university hospital with experience in aortic surgery. A triphase ECG-synchronized cardiothoracic flash protocol performed on a 256 line CT scanner confirmed an aortic intramural hematoma and a covered aortic perforation. Shortly afterwards the patient collaps

Atypical bilateral stress fractures of the femoral shaft diagnosed by bone scintigraphy in a woman with osteoporosis.

Recent case series have identified the presence of atypical insufficiency fractures at the diaphyseal femur of osteoporotic patients, which are possibly related to the long-term use of biphosphonates. We present images of a 72-year-old woman with a history of colon cancer and osteoporosis referred for bone scintigraphy because of bilateral thigh pain. No trauma or intense exercise was reported. Bone scan revealed bilateral femoral shaft stress fractures, which were confirmed by plain radiographs. In oncologic patients with osteoporosis referred for bone scintigraphy, atypical stress fractures

Psychosocial Intervention and Rehabilitation Utilization Together Improve Pain and Function After Traumatic Orthopaedic Injuries: A Secondary Analysis of a Randomized Controlled Trial.

BACKGROUND: Poor psychological response to injury is associated with the development of chronic pain after traumatic orthopaedic injury, but psychological response to injury is modifiable with appropriate identification and intervention. Rehabilitation (physiotherapy, occupational therapy) is often a critical part of pain and functional recovery. It is unknown whether rehabilitation utilization improves psychological response to injury and whether the combination of rehabilitation and a psychosocial intervention may be most effective at improving pain and functional outcomes for patients after

Development of mirror pain following trigeminal nerve injury: a case report and review of neuropathic mechanisms.

Following injury to a peripheral nerve, patients may complain of pain over the distribution of the same contralateral nerve, a phenomenon referred to as contralateral pain or mirror pain (MP). Symptoms of MP usually begin after the neuropathic pain from the original nerve injury has become chronic. Chronic neuropathic pain can lead to sensitization and spread of pain. Because the diagnosis of MP can be missed, patients may undergo multiple treatment procedures that prove to be ineffective in relieving the pain. This article presents a case of MP that appeared approximately 20 months following

Thinking beyond sickling to better understand pain in sickle cell disease.

Painful vaso-occlusive crises (VOCs) are the hallmark of sickle cell disease (SCD); however, many patients experience frequent daily pain that does not follow the pattern of typical VOCs. This pain of variable severity, also referred as persistent pain in the SCD literature, contributes to significant morbidity and poor quality of life and often fails to respond adequately to standard SCD therapies. In this article, we briefly describe types of pain encountered in SCD with a special emphasis on persistent pain. We discuss altered pain processing as a potential contributing mechanism, which may

An Unusual Lesser Sac Collection Causing Gastric Outlet Obstruction with Coincidental Occurrence of Leriche's Syndrome: A Case Report.

Gastric outlet obstruction in adults is usually caused by pyloric stenosis secondary to peptic ulcer disease or malignancy. However, there are few other causes such as a foreign body and external compression due to pseudocyst pancreas. We present a rare aetiology of a large collection of pus in the lesser sac in our patient causing gastric outlet obstruction. A perforated peptic ulcer was suspected in our patient who had symptoms of sudden onset pain in epigastric region which was referred to back. This was followed by pain in upper abdomen, vomiting, constipation and fever for which patient w

The proteinase inhibitor camostat mesilate suppresses pancreatic pain in rodents.

Camostat mesilate, an orally available proteinase inhibitor, is clinically used for treatment of pancreatitis. Given recent evidence that pancreatic proteinases including trypsin and/or proteinase-activated receptor-2 (PAR2) might be involved in pancreatic pain, we examined if camostat mesilate could suppress spinal Fos expression, a marker for neuronal activation, following specific application of trypsin to the pancreas, and pancreatitis-related referred allodynia. Trypsin, administered into the pancreatic duct, caused delayed expression of Fos proteins in the superficial layer of the bilate

The neurogenic elbow disharmony loop: a pathophysiologic model for chronic elbow pain supported by one hundred and fifty seven radial tunnel decompressions.

INTRODUCTION: Radial tunnel syndrome (RTS) remains an overlooked cause of chronic elbow pain. Clinical overlap with lateral epicondylitis, and the coexistence of other nerve entrapments contribute to diagnostic uncertainty. This study aimed to characterise the clinical presentation, and concomitant pain/nerve entrapment patterns following surgical decompression of posterior interosseous nerve (PIN) compression, and to explore the Neurogenic Elbow Disharmony Loop (NEDL) as a hypothesis-generating model for refractory elbow pain. METHODS: A retrospective review of 157 operated arms in 143 patie

Trends in Outpatient Opioid Prescriptions for Cancer Pain Between 2016 and 2021.

PURPOSE: Increasing opioid regulations have resulted in reduced opioid prescriptions, including for cancer pain, despite guideline exemptions. Data after 2017 following the Centers for Disease Control's 2016 pain management guidelines are limited on opioid prescribing practices of oncologists. The purpose of this study was to examine the trend in dose of opioids prescribed by oncologists to patients with cancer pain referred to outpatient palliative care between 2016 and 2021. METHODS: A single-center, cross-sectional, retrospective study was conducted at a tertiary cancer center's outpatient

Pleuritic chest pain from portal hypertensive gastropathy in ESRD patient with autosomal dominant polycystic kidney disease misdiagnosed as pericarditis.

Portal hypertensive gastropathy (PHG) is a gastric mucosal lesion complicating portal hypertension, with higher prevalence in decompensated cirrhosis. PHG can sometimes complicate autosomal dominant polycystic kidney disease (ADPKD) due to the presence of multiple liver cysts. Besides, PHG is known to present as chest pain, with or without hematemesis. Other causes of chest pain in ADPKD include referred chest pain from progressively enlarging kidney cysts, and rare pericardial cysts. Chest pain, especially if pleuritic, in end-stage renal disease (ESRD) patients, is often ascribed to uremic p
