No-suture aphakic epikeratoplasty.

BACKGROUND: Use of sutures with aphakic epikeratoplasty is time-consuming during surgery and can create significant hazards for the patient. Thus, a surgical technique for aphakic epikeratoplasty without sutures was developed. METHODS: The conventional procedure was modified to use an oversized lenticule tucked evenly in an expanded lamellar pocket with no anchoring sutures. The technique was performed on one eye of 27 consecutive patients over a 9-month period. RESULTS: After a mean follow up of 14 months, results showed refractive and visual parameters comparable to a sutured technique. A

Revisiting monovision for presbyopia.

PURPOSE OF REVIEW: Presbyopia is an inevitable age-related loss of accommodation that results in spectacle dependence with common everyday near-vision tasks. Many surgical approaches to presbyopic correction have been investigated, with monovision correction being amongst the most common and attractive options. In this article, we will review the advantages and disadvantages of the new modalities of presbyopic monovision correction. RECENT FINDINGS: New methods such as mini-monovision, corneal inlays, and multifocal corneal ablation aim to maximize stereopsis and decrease the anisometropic si

Modified surgical technique for repeated epikeratophakia surgery in aphakic eyes.

BACKGROUND: Incomplete epithelial removal from the peripheral keratectomy at the time of repeat epikeratophakia surgery can lead to interface downgrowth and consequent failure, as observed in the case reported herein. We present a modification of the standard surgical technique aimed at avoiding such complication. METHODS: A 69-year-old white female underwent a third aphakic epikeratophakia modified to reduce the risk of epithelial interface downgrowth, which had caused a previous failure. The aphakic tissue lens, 8.5 mm in diameter, was sutured into a corneal pocket obtained at a deeper leve

A case of unilateral progression of myopia after strabismus surgery.

We report the case of a 6-year-old boy with stable bilateral simple myopia who underwent strabismus surgery of the left eye for intermittent exotropia. One month following surgery, myopia had progressed to 2.5 D in the operated eye, increasing to 4.5 D by 1 year. Myopic progression was stabilized following treatment with low-dose atropine. The refractive error in the unoperated right eye showed mild increase. The mechanisms that may have precipitated postoperative progressive myopia in this case are discussed.

Consultation Section: Refractive. Consecutive hyperopia after myopic laser in situ keratomileusis.

A 31-year-old woman was seen at our clinic with dissatisfying hyperopic error after a myopic laser treatment of her left eye in April 2018. The surgery took place 1 year previously. There were no abnormalities in her medical history. The preoperative corrected distance visual acuity (CDVA) in the left eye was 20/16 with -7.50 -0.50 × 75. The refraction in the right eye was -3.50 -1.25 × 90. The cycloplegic correction of the left eye preoperatively was -6.50 -0.50 × 68. A femtosecond-assisted laser in situ keratomileusis procedure was performed with an optical zone of 6.3 mm, an ablation zone of

Refractive Lenticule Implantation for Correction of Ametropia: Case Reports and Literature Review.

The ReLEx® technique allows correction of refractive errors through the creation and extraction of refractive stromal lenticules. Contrary to excimer laser corneal refractive procedures, where the stromal tissue is photoablated, the extracted lenticules obtained with ReLEx® can be preserved. Recent studies and case reports have described autologous re-implantation and allogeneic implantation of refractive lenticules into femtosecond-laser created stromal pockets in order to reverse the refractive outcome of a myopic corneal refractive procedure, correct hyperopia, aphakia, presbyopia and treat

[Analyzing and tracking preoperative and intraoperative astigmatism].

Precise evaluation of preoperative astigmatism is the first step optimizing outcomes. This begins with office-based evaluation of astigmatism; corneal astigmatism is evaluated by keratometry, traditionally by Javal keratometry, but now including topography, whether Placido- or elevation-based, which allows for detailed analysis of even irregular astigmatism, including the corneal periphery, which is invaluable. Aberrometers, essentially "super-auto refractors", allow the incorporation of additional data into the qualitative analysis of astigmatism. The correlation between these multiple preope

Central and peripheral corneal thicknesses in cases with and without keratoconus.

Keratoconus is pathological condition of eye characterized by paraxial stromal thinning leads to corneal surface distortion. It is well documented that the corneal thickness is abnormally altered in keratoconus; but it seems that racial differences are exist in the amount of changes. This study compared the central and peripheral corneal thicknesses (CCT and PCT, respectively) in eyes with and without keratoconus in a large Iranian population. CCT and PCT were measured by ultrasonographic pachometry in ten thousands eyes undergoing refractive surgery during a six-year period (2003-2010) in Tab

Eye dryness sensations after refractive surgery: impaired tear secretion or "phantom" cornea?

The cornea is richly innervated by various functional types of sensory nerve fibers. When stimulated, these fibers evoke conscious sensations of different quality including ocular dryness, discomfort, and pain. Refractive surgery involves a variable degree of damage to corneal nerves. This leads to an altered expression of membrane ion channels at the injured and regenerating nerve fibers, giving rise to aberrant spontaneous and stimulus-evoked nerve impulse firing. It is speculated that these abnormal sensory discharges are read by the brain as ocular surface dryness. This would explain the h

Subepithelial reticular cicatrization following radial keratotomy in a patient with inactive trachoma.

BACKGROUND: Opacification of the clear optical zone following radial keratotomy has not been observed before. METHODS AND RESULTS: The authors noted the appearance of subepithelial dense white reticular cicatrization in the central area following radial keratotomy in a patient with inactive trachoma and clear cornea. The corneal changes resulted in regression of the surgical effect and decrease in visual acuity. A good visual result was obtained by performing homoplastic myopic keratomileusis in situ. Clinical and histopathological findings are presented. CONCLUSIONS: Patients undergoing rad

Combined bilateral lateral rectus recession and medial rectus Faden procedure in true divergence excess exotropia: a case report.

True divergence excess, a subtype of intermittent exotropia, presents with a greater exodeviation at distance than at nearby. Its management often requires tailored surgical strategies to address both distance and near deviations. A 9-year-old Saudi girl presented with intermittent right-eye exotropia. Prism and alternate cover test measurements revealed 30 prism diopters (PD) of exodeviation at distance and 12 PD at nearby. Cycloplegic refraction showed mild hyperopia. The accommodative convergence to accommodation ratio was within normal limits. No significant ocular or systemic comorbiditie
