Central nervous system control of breathing in natural conversation turn-taking.

Conversations-hallmarks of human social interaction-rely on a deeply rooted evolutionary tool: vocalization. Vocalization productions begin with an airflow from the lungs, shaped by the vocal folds and articulators of the mouth to generate a rich variety of sounds. Speech production therefore involves tight synchronisation between respiratory and articulatory events, especially in natural social interactions. During conversations, turn-taking indeed implies respiration anticipation: lungs must be filled with enough air to produce the next speech segment. Here, we use an fMRI corpus of live, un

Tongue, palatal, hyoid and pharyngeal muscle activity during chewing, swallowing, and respiration.

OBJECTIVE: Chewing, swallowing, and respiration are synchronized oropharyngeal functions. This study aimed to analyze the dynamics and coordination during natural chewing and swallowing in relation to respiratory phases. DESIGN: Eight oropharyngeal muscles in minipigs were recorded using electromyography, X-ray fluoroscopy, and nasopharyngeal dynamics. Chewing cycles and swallowing episodes were analyzed for timing and activity amplitude along respiratory cycles. Digastric and middle pharyngeal constrictor were used as zero-points for timing analysis in chewing cycles and swallowing episodes,

SS-31 does not prevent or reduce muscle atrophy 7 days after a 65 kdyne contusion spinal cord injury in young male mice.

Spinal cord injury (SCI) leads to major reductions in function, independent living, and quality of life. Disuse and paralysis from SCI leads to rapid muscle atrophy, with chronic muscle loss likely playing a role in the development of the secondary metabolic disorders often seen in those with SCI. Muscle disuse is associated with mitochondrial dysfunction. Previous evidence has suggested targeting the mitochondria with the tetrapeptide SS-31 is beneficial for muscle health in preclinical models that lead to mitochondrial dysfunction, such as cast immobilization or burn injury. We gave young ma

Control of the cardiovascular and respiratory systems during sleep.

Sleep and arousal from sleep are associated with profound changes in cardiovascular and respiratory functions. Fluctuations of arterial blood pressure (ABP), heart rate (HR), and respiration occur both during non-rapid eye movement (NREM) and REM sleep and during transitions between sleep and behavioral arousal. These changes reflect complex, state-dependent interactions among several neuronal groups in the hypothalamus and brainstem. These neurons utilize the excitatory amino-acid L-glutamate or the inhibitory amino acid γ-aminobutyric acid (GABA) and are modulated in a state-dependent manner

Evoked pain analgesia in chronic pelvic pain patients using respiratory-gated auricular vagal afferent nerve stimulation.

OBJECTIVE: Previous vagus nerve stimulation (VNS) studies have demonstrated antinociceptive effects, and recent noninvasive approaches, termed transcutaneous-vagus nerve stimulation (t-VNS), have utilized stimulation of the auricular branch of the vagus nerve in the ear. The dorsal medullary vagal system operates in tune with respiration, and we propose that supplying vagal afferent stimulation gated to the exhalation phase of respiration can optimize t-VNS. DESIGN: Counterbalanced, crossover study. PATIENTS: Patients with chronic pelvic pain (CPP) due to endometriosis in a specialty pain cl

Pharmacological modulation of respiratory control: Ampakines as a therapeutic strategy.

Ampakines are a class of compounds that are positive allosteric modulators of α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptors and enhance glutamatergic neurotransmission. Glutamatergic synaptic transmission and AMPA receptor activation are fundamentally important to the genesis and propagation of the neural impulses driving breathing, including respiratory motoneuron depolarization. Ampakines therefore have the potential to modulate the neural control of breathing. In this paper, we describe the influence of ampakines on respiratory motor output in health and disease. We

Mechanical Insufflation-Exsufflation: The Rest of the Story.

Mechanical insufflation-exsufflation has been reported to decrease pneumonia rates by about 90% for patients with Duchenne muscular dystrophy now living into their 40s and 50s without tracheotomy tubes. It greatly reduces respiratory complications and hospitalization rates to less than one per 10 patient-years for advanced spinal muscular atrophy type 1, through 25-30 years of age. It is most successful from the point at which small children become able to cooperate with it, generally from 3 to 5 years of age. However, since the 1950s, successful use to extubate and decannulate ventilator "unw

A new approach to studying the nasal and oral breathing routes.

INTRODUCTION: Nasal obstruction is often reported by patients. It is a consequence of a subjective feeling of impaired airflow through the nasal cavity. Currently, objective measures of nasal patency rates are very limited. Functional tests only analyze nasal breathing; they do not simultaneously assess airflow through the mouth. OBJECTIVE: The aim of this study is to present a new functional test that assesses a single-stage nasal and oral breathing route. METHODS: The NOFA (Nasal-Oral Flow Analyzer) is a three-channel flow meter used to perform continuous and simultaneous measurements of n

[Respiratory support techniques in foals in a newborn intensive care unit for large animals].

Various techniques for support and control of respiration in neonatal foals are described. It is crucial to evaluate respiration through frequent arterial blood gas analysis. Details for blood sampling from the metatarsal arteries and interpretation of results are provided. Typical diseases in newborn foals, which cause hypoxemia and/or hypercapnia and can be indications for respiratory support are apnea, hypopnea, pulmonary atelectasis, surfactant deficiency, meconium-, fetal fluid- and milk aspiration, maladjustment syndrome, cardiovascular abnormalities, anemia, airway obstruction, compromi

Pediatric Myasthenia Gravis.

Pediatric myasthenia gravis encompasses a heterogenous group of disorders that cause impaired neuromuscular junction transmission. Patients exhibit a characteristic fatigable weakness that can be life-threatening if respiration is affected. In children, myasthenia gravis can be grouped into 3 subtypes: transient neonatal myasthenia (TNM) arising from transplacental antibody transfer, congenital myasthenic syndromes (CMS) related to genetic defects, and juvenile myasthenia gravis (JMG) caused by antibodies to synaptic components. Diagnostic evaluation combines clinical evaluation with serologic

From copper homeostasis to cuproptosis: a new perspective on CNS immune regulation and neurodegenerative diseases.

Copper, an essential trace element for the human body, plays a key role in energy metabolism, mitochondrial respiration, redox reactions, and neural signal transmission. The recently proposed concept of "cuproptosis" has further revealed the unique status of copper in cellular regulation: when copper abnormally accumulates within cells, it can directly bind to the lipoylated proteins of the mitochondrial TCA cycle, triggering protein aggregation and metabolic disorders, ultimately leading to cell death. This form of cell death plays an important role in various neurodegenerative diseases of th

Role of angiotensin II in chronic intermittent hypoxia-induced hypertension and cognitive decline.

Sleep apnea is characterized by momentary interruptions in normal respiration and leads to periods of decreased oxygen, or intermittent hypoxia. Chronic intermittent hypoxia is a model of the hypoxemia associated with sleep apnea and results in a sustained hypertension that is maintained during normoxia. Adaptations of the carotid body and activation of the renin-angiotensin system may contribute to the development of hypertension associated with chronic intermittent hypoxia. The subsequent activation of the brain renin-angiotensin system may produce changes in sympathetic regulatory neural ne

Ondine's curse: myth meets reality.

Ondine's curse is one of the most enchanting mythical tales in the field of Medicine. The nymph Ondine was an immortal water spirit who became human after falling in love for a man, marrying him, and having a baby. In one of the versions of the tale, when she caught her husband sleeping with another woman, she cursed him to remain awake in order to control his own breathing. During the 19th century, the rare syndrome characterized by loss of autonomic breath control, while voluntary respiration remains intact, was cleverly named "Ondine's curse". Nowadays, the term Ondine's curse is usually as

Perception, assessment, treatment, and management of pain in the elderly.

Twenty to 50% of community elderly suffer from pain. Up to 80% of the institutionalized elderly report at least one pain problem. Multiple pain etiologies that occur in elderly patients may be the occurrence of multiple chronic diseases: osteoarthritis, RA, cancer, DJD, bone/joint disorders, osteoporosis, surgical pain, trauma, neuropathic pain, and nociceptive pain. The incidence of unrelieved pain inhibits respiration, decreases mobility, and decreases their functional status, which may lead to iatrogenic events, which include pneumonia, constipation and deep vein thrombosis. Prolonged inpat

Respiratory distress after recurrent laryngeal nerve sectioning for adductor spastic dysphonia.

Adductor spastic dysphonia is a voice sign associated with various neurologic and psychologic disorders. Treatment of spastic dysphonia in selected patients is unilateral recurrent laryngeal nerve sectioning. Except for voice change or, in some patients, return of phonatory spasticity, there have been no long-term sequelae or complications of this treatment. We describe three patients with adductor spastic dysphonia who underwent recurrent laryngeal nerve sectioning and who, 3 to 38 months later, suffered respiratory distress that required tracheostomy. The respiratory distress in all three pa

Panic disorder from a monistic perspective: integrating neurobiological and psychological approaches.

In the literature, psychological and biological theories of panic disorder are often regarded as mutually exclusive. The present article presents an integrative theory that explains how and why cognitive misinterpretations and "false threat alarms" leading to irrational fear and anxiety can arise from a neurobiological dysfunction in the amygdala and ascending transmitter systems. According to this view, physiological symptoms (such as palpitations and respiration manoeuvres) and psychological symptoms of anxiety (perception of threat and anticipation of catastrophe) are elicited simultaneousl

Neuroleptic-induced respiratory dyskinesia.

Respiratory dyskinesia is a common but under-recognized side effect of chronic neuroleptic administration. It manifests as irregular respiration, dyspnea, grunting or gasping, and abnormal chest or esophageal movements. It occurs almost exclusively in association with other tardive effects of neuroleptics, such as tardive dyskinesia and tardive akathisia. Complications of the disorder include respiratory alkalosis and aspiration pneumonia. The authors describe 5 patients with respiratory dyskinesia whose cases highlight the important clinical features of neuroleptic-induced respiratory dyskine
