Sarcoidosis (Morbus Boeck) is a frequently found disease of the reticulohistiocytic system. Etiology and pathogenesis were mostly unknown until today. Histologic study of sarcoidosis shown granuloma with epitheloid cells similar to tuberculosis, often with hyalinization but without cheesy necrosis. The disease mostly starts with a mediastinal and cervical lymphadenopathy. Radiographically, the disease shows three typical stages when spreading to the lungs. This is the second known case of retroperitoneal fibrosis in a middle-aged black man whose tissue type was HLA-B27 positive. The HLA-B27 antigen is present in only 4 per cent of black American male subjects and may be important in the diagnosis and management of patients with primary retroperitoneal fibrosis. A genetic predisposition is suggested for the development of idiopathic primary retroperitoneal fibrosis in patients who are HLA-B27 antigen positive.

The nephropathology associated with retroperitoneal fibrosis has been commonly related to compression of the ureters and hydronephrosis. This assumption has been made without the use of immunofluorescent microscopy and electron microscopy. The authors studied renal tissue from a patient with retroperitoneal fibrosis by light microscopy, immunofluorescent microscopy and electron microscopy. A means of assessing the degree of obstruction of the ureter involved in retroperitoneal fibrosis by perfusion studies is described and the results in 3 patients are discussed. This type of urodynamic assessment is recommended when the extent of lysis of the ureter is in question.

Retroperitoneal fibrosis causes almost without exception ureteral stenosis, frequently vascular constriction but rarely intestinal obstruction. Of 17 cases reported in the literature, 14 showed a colon sigma stenosis and 3 an obstruction of the duodenum. The appearance of intestinal stenosis neither correlates with the duration of symptoms nor with the extent of retroperitoneal fibrosis. In three patients with idiopathic mediastinal or retroperitoneal fibrosis, postmortem sections of the aorta showed aortitis (in one case) or extrusions of atheromatous debris into the inflamed adventitia (two cases). It is suggested that damage to the aortic wall, followed by adventitial inflammation due to allergy to a component of the atherosclerotic plaque, is one way in which this disease may develop.
