Real-world ocular safety performance of dermatologic laser and intense pulsed light goggles: a clinical safety audit.

Effective ocular protection is essential during dermatologic laser and intense pulsed light (IPL) procedures to prevent retinal and corneal injury. Despite widespread clinical use, the real-world safety performance of protective goggles has not been systematically evaluated. To assess the real-world optical density (OD) performance of dermatologic laser and IPL goggles and to identify potential risks associated with inadequate eye protection. Optical density was measured across 350–920 nm using a xenon lamp–based spectrophotometric system for goggles used with IPL, long-pulsed Nd: YAG, Q-switc…

Equity-focused patient safety: reframing cultural competence, cultural humility and structural competence as determinants of patient safety.

Cultural competence and cultural humility have previously been framed as concepts reflecting the knowledge, skills and commitment to self-reflection concerning person-centred healthcare. Emerging evidence has linked cultural misunderstanding, language barriers and implicit bias with diagnostic error, medication mismanagement and inequitable health outcomes for patients. Beyond this, the concept of structural competence has acknowledged how organisational, institutional, social and political structures can shape care and impact patient safety. There has yet to be appreciation of these terms tog…

Monitoring vaccine safety using the vaccine safety Datalink: Assessing capacity to integrate data from Immunization Information systems.

BACKGROUND: The Vaccine Safety Datalink (VSD) uses vaccination data from electronic health records (EHR) at eight integrated health systems to monitor vaccine safety. Accurate capture of data from vaccines administered outside of the health system is critical for vaccine safety research, especially for COVID-19 vaccines, where many are administered in non-traditional settings. However, timely access and inclusion of data from Immunization Information Systems (IIS) into VSD safety assessments is not well understood. METHODS: We surveyed the eight data-contributing VSD sites to assess: 1) statu…

Assuring safety without animal testing: Unilever's ongoing research programme to deliver novel ways to assure consumer safety.

Assuring consumer safety without the generation of new animal data is currently a considerable challenge. However, through the application of new technologies and the further development of risk-based approaches for safety assessment, we remain confident it is ultimately achievable. For many complex, multi-organ consumer safety endpoints, the development, evaluation and application of new, non-animal approaches is hampered by a lack of biological understanding of the underlying mechanistic processes involved. The enormity of this scientific challenge should not be underestimated. To tackle thi…

Near real-time surveillance for influenza vaccine safety: proof-of-concept in the Vaccine Safety Datalink Project.

The emergence of pandemic H1N1 influenza in 2009 has prompted public health responses, including production and licensure of new influenza A (H1N1) 2009 monovalent vaccines. Safety monitoring is a critical component of vaccination programs. As proof-of-concept, the authors mimicked near real-time prospective surveillance for prespecified neurologic and allergic adverse events among enrollees in 8 medical care organizations (the Vaccine Safety Datalink Project) who received seasonal trivalent inactivated influenza vaccine during the 2005/06-2007/08 influenza seasons. In self-controlled case ser…

Corporate whistleblower system and workplace safety performance: from the dual perspectives of safety investment and internal control.

INTRODUCTION: While corporate whistleblower system helps prevent and mitigate corporate violations within business operations, yet its effectiveness in the field of workplace safety remains largely unsubstantiated. Thus, it is both theoretically and practically important to examine whether and how the adoption of whistleblower system is associated with workplace safety performance. METHODS: Grounded in an integrated framework of accident causation theory (explaining the direct effect on safety performance), deterrence theory (elucidating the mediating role of safety investment), and signaling…

From Safety Science to Operational Reality: Competing Institutional Logics in the Translation of National Patient Safety Recommendations.

BACKGROUND: National patient safety investigation bodies conduct system-level investigations and produce recommendations intended to drive improvement. Evidence that such recommendations translate into meaningful improvement is weak and the reasons for this are poorly understood. This paper examines how recommendations produced by the Health Services Safety Investigations Body (HSSIB), England's independent national patient safety investigation body, are produced by investigators and received by the national organisations to whom they are directed. METHODS: A qualitative multi-method design w…

Interplay between Safety Climate and Emotional Exhaustion: Effects on First Responders' Safety Behavior and Wellbeing Over Time.

Various job demands continuously threaten Emergency Medical Service (EMS) first responders' safety and wellbeing. Drawing on Job Demands-Resources Theory, the present study examines the effects of the organizational context-safety climate-and the psychological context-emotional exhaustion-on safety behaviors and wellbeing over time. We tested our hypotheses in a longitudinal study of 208 EMS first responders nested within 45 stations from three fire departments in US metropolitan areas over 6 months during the beginning of the COVID-19 pandemic. Multilevel modeling showed that the relationship…
