The use of free vascularized corticoperiosteal grafts from the femur in the treatment of scaphoid non-union.

Most scaphoid non-unions can be treated successfully with conventional bone graft and screw fixation. Only a few cases with established avascular necrosis of the proximal fragment and previous failed conventional bone grafting should undergo vascularized bone graft. Cases in which the fragments are too small to accommodate a vascularized bone graft are better treated by alternative procedures. This article describes the role of vascularized bone grafting for scaphoid non-union, indications, donor bone selection, the authors' operative technique for vascularized bone graft from the supracondyla

Dual-energy CT in the diagnosis of occult acute scaphoid injury: a direct comparison with MRI.

OBJECTIVES: Scaphoid injuries occult on plain radiography often require further imaging for definitive diagnosis. We investigate the utility of dual-energy computed tomography (DECT) for the detection of acute bone marrow oedema and fracture of scaphoid compared to MRI. MATERIALS AND METHODS: Twenty patients who presented acutely (without prior injury) to the emergency department with clinically suspected occult scaphoid fracture and had MRI of the wrist were prospectively recruited to have DECT (GE Revolution CT). Material decomposition images of the water-calcium base pair were generated an

[Treatment for non-union scaphoid fractures with headless cannulated screws and distal radius cancellous autograft].

INTRODUCTION: scaphoid fractures comprise 50-80% of the fractures of the carpal bones. Ten percent of the scaphoid fractures evolve to non-union and show degenerative changes in the carpus in 75-97% at five years and in 100% at 10 years. The objective of this work was to evaluate the rate and time to union in patients with diagnosis of scaphoid non-union without fragmentation of the proximal pole after treatment with two cannulated headless screws and distal radius cancellous autograft. MATERIAL AND METHODS: case series with short-term follow-up of four patients with scaphoid non-union withou

Trans-scaphoid Perilunate Fracture-dislocation with Concomitant Lunotriquetral Ligament Disruption: A Case Report.

We report a case of trans-scaphoid perilunate fracture-dislocation with concomitant lunotriquetral ligament disruption of the right wrist in a 44-year-old man, sustained from a 10-m fall landing on his outstretched right hand. Open reduction was performed 1 day after injury; at first the palmar dislocation of the lunate was reduced with the palmar approach. Under direct view with the dorsal approach, the scaphoid was comminuted and then treated with open reduction and internal fixation with a double threshold screw using a dorsal approach and a bone graft from the distal radius. Although the s

Role of magnetic resonance imaging in assessing factors affecting healing in scaphoid nonunions.

This study evaluates the role of magnetic resonance imaging in assessing the factors affecting the rate and healing time in scaphoid nonunions after surgery. Nineteen patients were assessed before surgery by radiographs, tomography, and magnetic resonance imaging. Fifteen had viable bone marrow and 4 patients had nonviable bone marrow on magnetic resonance imaging. All patients with normal preoperative magnetic resonance imaging healed in an average time of 4.7 months. Of the 4 patients with abnormal magnetic resonance imaging, but normal plain radiographs, 2 went on to heal in 10.5 months. Th

[Unilateral congenital aplasia of the scaphoid and dysplasia of the trapezium (a case report)].

Aplasia of the carpal scaphoid bone in association with dysplasia of the trapezium is a rare disorder. A right-handed, 66 years old male, suffering from a trapeziometacarpal arthritis on the right side is reported. X rays, MRI and CT-scan reveal an absence of scaphoid and a dysplasia of the trapezium. The lunate remains in place on the radius but is oriented in dorsi flexion. The capitate is dorsally and radially subluxated. Trapezium inclination of the joint at the base of the first metacarpal (trapezial tilt) is 32 degrees greater than on the opposite side which is normal (48 degrees). Radia…

Volar approach and screw fixation technique for fractures and non-unions of the carpal scaphoid.

Delayed or non-union of the carpal scaphoid may be treated in a variety of ways. This article describes the operative technique and clinical results of a volar approach and screw fixation technique, which offers distinct advantages over other approaches. The volar approach to the scaphoid is simple, safe and rapid. It allows access to the fracture for fixation, the radioscaphoid joint for assessment and the distal radius for the procurement of a bone graft where necessary. Access to the volar aspect of the scaphoid is also biomechanically sound as it allows insertion of a wedge-shaped bone gra

Posttraumatic cystlike defects of the scaphoid: late sign of occult microfracture and useful indicator of delayed union.

OBJECTIVE: To investigate the association between radiolucencies in the scaphoid and trauma, we studied the unenhanced radiographs and CT images of 1087 adult patients. CONCLUSION: Three (0.3%) of the 1087 patients had scaphoid radiolucencies that developed after trauma. We hypothesize that in these patients a microfracture of the scaphoid or subtle fractures are the cause of persisting clinical symptoms. Persistent loading of the scaphoid due to wrist movement can lead to unrestricted compressive forces and ischemia and to the development of a bone cyst-like pathologic change, apparent on th

Scaphoid non-union after simultaneous fractures of the scaphoid and the capitate in a child: a case report.

A nine-year-old boy fell on his outstretched hand and was diagnosed with simultaneous fractures of the scaphoid and the capitate. The patient was treated conservatively with a cast for two months. Fracture of the capitate healed, however, fracture of the scaphoid resulted in non-union. We treated scaphoid non-union with an iliac bone graft and internal fixation. Three years after surgery, the patient had neither complaints nor complications and union of the scaphoid was confirmed on the radiographs.

Acute compression fracture of the scaphoid.

A previously unreported radiological sign of a sclerotic margin to an acute displaced scaphoid fracture due to bone compression is reported. This should to be recognized because of the implied bone loss and hence risks of both mal-union and non-union, and because of the implication that volar compression fractures of the scaphoid may occur.

Scaphoid implant resection arthroplasty. Long-term results.

A silicone scaphoid implant was developed in 1967 to help improve the results of simple resection arthroplasty in the treatment of arthritic changes associated with advanced scaphoid disease. Patients who had scaphoid implant surgery since 1967 were studied. Preoperative and postoperative roentgenograms, charts, surgical pathology, and clinical evaluations were reviewed. A total of 55 cases of scaphoid implant arthroplasty were evaluated. A classification system was developed based on progressive severity, and treatment recommendations were developed for each stage of disease progression. Cyst

Percutaneous fixation of the scaphoid through a dorsal approach: an anatomic study.

PURPOSE: Percutaneous surgical treatment of nondisplaced scaphoid fractures is becoming more common. Although the surgical anatomy at risk has been well described for the volar approach to the scaphoid, we have not found articles elucidating the dangers with a percutaneous dorsal approach. Additionally, direct placement of the screw is not possible with a percutaneous approach, and there is a risk of not seating the screw below the subchondral bone. The purpose of this study was to delineate the anatomy at risk using a dorsal percutaneous approach to the scaphoid and to determine the accuracy

Preiser's disease: identification of two patterns.

PURPOSE: A large series of patients with Preiser's disease was reviewed to compare 2 potentially different categories of this disorder: complete versus partial vascular impairment of the scaphoid bone as determined by magnetic resonance imaging (MRI). METHODS: Nineteen patients with Preiser's disease were identified retrospectively from 2 institutions. Using MRI criteria, 2 disease patterns were identified: diffuse necrosis and/or ischemia of the scaphoid (type 1 disease, 11 cases) and segmental vascular impairment of the scaphoid (type 2 disease, 8 cases). Medial Femoral Trochlea Graft for Scaphoid Waist Nonunion: A Case Report and Review of the Literature.

The medial femoral trochlea (MFT) of the knee is a donor site for convex osteochondral vascularized bone that has been used for the salvage of fractures of the proximal pole of the scaphoid. Chronic nonunited fractures of the scaphoid may lead to a sequence of degenerative change often referred to as scaphoid nonunion advance collapse. The vascularized MFT osteochondral graft has been reported as a salvage procedure for fractures of the proximal pole of the scaphoid, in situations where fixation is not an option. In this "Special review," we describe the technique of free vascularized MFT graf

A modified Matti-Russe technique of grafting scaphoid non-unions.

INTRODUCTION: In treatment of scaphoid non-union bone healing requires beside vital tissue and stability, enduring compression and a good interface between the graft and scaphoid fragments. While fixation techniques show a good primary stability, they reduce sintering and thus compression of fragments in the long term. Therefore, a modified technique optimising the cancellous interface between graft and scaphoid but still providing enough stability without fixating implants should be evaluated. MATERIALS AND METHODS: A corticocancellous bone graft from iliac crest was shaped ellipsoid and was

Distal Scaphoid Excision for Chronic and Nonchronic Scaphoid Fracture Nonunion.

PURPOSE: The indications for distal scaphoid excision are limited to localized wrist arthritis surrounding the scaphoid, as a result of scaphoid nonunion advanced collapse or scapho-trapezio-trapezoid joint arthritis. The procedure historically has led to relief of symptoms and improvement in strength. Our aim was to examine the outcomes of this procedure in patients with scaphoid fracture nonunion. METHODS: This is a single-center retrospective case series of 12 consecutive patients who underwent distal scaphoid excision after scaphoid fracture nonunion. Patients were divided into the follow
