Sentinel lymph-node biopsy for melanoma of the trunk and extremities: the McGill experience.

OBJECTIVE: To determine the effectiveness of sentinel lymph-node (SLN) biopsy for melanoma of the trunk and extremities. DESIGN: Case series review. SETTING: Royal Victoria Hospital, a Canadian university hospital. PATIENTS: Thirty-six patients (18 women and 18 men) seen between October 1996 and December 1998 with melanoma 1 mm or more in thickness with clinically negative lymph-node basins. Follow-up was 396 days. INTERVENTIONS: SLN biopsy. Technetium-99m filtered sulfur colloid (0.5 mCi) was injected intradermally around the melanoma or the excision scar 10 to 15 minutes before the surgi

Tagging sentinel lymph nodes: a study of 100 patients with breast cancer.

The aim of this study was to evaluate in breast cancer patients the feasibility of sentinel lymph node (SLN) identification and the sensitivity of this technique to detect node metastases. Between January and July 1997, SLNs were tracked with Evans Blue dye in 100 patients with breast cancer who then underwent complete level I/II axillary lymph node dissection (ALND). All SLNs were examined by haematoxylin-phloxin-saffron (HPS) staining and immunohistochemistry (IHC) of multiple sections. The findings for the SLNs were compared with results on ANLD. Axillary SLNs were identified in 83 patients…

Positive sentinel lymph node biopsy predicts the number of metastatic axillary nodes of breast cancer.

It remains to be clarified whether a positive sentinel lymph node biopsy (SLNB) can predict the number of metastatic axillary nodes. This study examined a consecutive series of women with unilateral invasive breast cancer who underwent axillary lymph node dissection after an intra-operative positive SLNB. The numbers of positive and negative sentinel lymph nodes (SLNs) were analyzed for a likelihood of pN1a, pN2a, and pN3a diseases as per the UICC TNM classification. Of the 368 study patients, 165 (45%) had one positive SLN and one or more negative SLNs. This result represented the most common…

Factors associated with involvement of four or more axillary nodes for sentinel lymph node-positive patients.

PURPOSE: Sentinel lymph node-positive (SLN+) patients who are unlikely to have 4 or more involved axillary nodes might be treated with less extensive regional nodal radiation. The purpose of this study was to define possible predictors of having 4 or more involved axillary nodes. METHODS AND MATERIALS: The records of 224 patients with breast cancer and 1 to 3 involved SLNs, who underwent completion axillary dissection without neoadjuvant chemotherapy or hormonal therapy were reviewed. Factors associated with the presence of 4 or more involved axillary nodes (SLNs plus non-SLNs) were evaluated…

Omission of Axillary Lymph Node Dissection in Early-Stage Breast Cancer With Limited Sentinel Lymph Node Metastasis: A Propensity Score-Matched Analysis.

BACKGROUND: Whether axillary lymph node dissection (ALND) can be safely omitted clinically node-negative (cN0) breast cancer with limited sentinel lymph (SLN) metastasis remains uncertain, particularly after mastectomy. METHODS: This retrospective cohort included women with T1-T2 cN0 breast cancer and positive SLN between 2015 and 2020. Patents underwent sentinel lymph node biopsy (SLNB) alone or ALND. Propensity score matching (1:1, nearest neighbor, caliper 0.02 on logit) balanced age, T stage, tumor grade, vascular invasion, number of positive SLNs, ER/PR/HER2, Ki-67, surgery type, chemoth…

Potential factors of false-negative sentinel lymph node biopsy for breast cancer and clinical decision-making value of axillary lymph node dissection.

OBJECTIVE: To investigate the potential factors of false-negative sentinel lymph node biopsies (SLNB) for breast cancer and the clinical decision-making value of axillary lymph node dissection (ALND). METHODS: We retrospectively selected 312 patients with breast cancer who underwent SLNB in our hospital from May 2021 to July 2024. These subjects were divided into a false negative group and a true negative group according to the SLNB results. Influencing factors were analyzed by Logistic regression. A total of 300 breast cancer patients who underwent ALND at the same time were selected as the …

10-Year axillary recurrence in the RACS SNAC1 randomised trial of sentinel lymph node-based management versus routine axillary lymph node dissection.

BACKGROUND: Sentinel node-based management (SNBM) is the international standard of care for early breast cancer that is clinically node-negative based on randomised trials comparing it with axillary lymph node dissection (ALND) and reporting similar rates of axillary recurrence (AR) without distant disease. We report all ARs, overall survival, and breast cancer-specific survival at 10-years in SNAC1. METHODS: 1.088 women with clinically node-negative, unifocal breast cancers 3 cm or less in diameter were randomly assigned to either SNBM with ALND if the sentinel node (SN) was positive, or to

The Use of Wearable Activity Monitors to Measure Upper Limb Physical Activity After Axillary Lymph Node Dissection and Sentinel Lymph Node Biopsy.

BACKGROUND: We suspect that morbidity from both sentinel lymph node biopsy (SLNB) and axillary lymph node dissection (ALND) has been inadequately evaluated to date. Current methodologies are subjective and susceptible to bias. Objective assessment using wearable activity monitors (WAMs) would allow quantitative analysis of recovery by measuring physical activity (PA) and could provide evidence for axillary de-escalation. PATIENTS AND METHODS: A prospective, single center, observational study was conducted from February 2020 to May 2022. Consecutive patients undergoing breast and/or reconstruc…

Overall survival after surgical staging by lymph node dissection versus sentinel lymph node biopsy in endometrial cancer: a national cancer database study.

OBJECTIVE: Substituting lymphadenectomy with sentinel lymph node biopsy for staging purposes in endometrial cancer has raised concerns about incomplete nodal resection and detrimental oncological outcomes. Therefore, this study aimed to investigate the association between the type of lymph node assessment and overall survival in endometrial cancer accounting for node status and histology. METHODS: Women with stage I-III endometrial cancer who underwent hysterectomy and lymph node assessment from January 2012 to December 2015 were identified in the National Cancer Database. Patients who underw…

Predictive factors of lymph node metastasis and effectiveness of intraoperative examination of sentinel lymph node in breast carcinoma: A retrospective Belgian study.

Recently, several trials demonstrated the safety of omitting axillary lymph node dissection in clinically N0 patients with positive sentinel nodes in select subgroups. However, this fact is still troublesome to clarify to surgeons and clinicians, as they used to perform intraoperative examination of the sentinel node and axillary dissection for many years. Hence, we decided to review our practice. This is to firstly highlight the predictive factors of node metastasis and secondly, to evaluate the effectiveness of intraoperative examination of the sentinel node. There were 406 total procedures.

Should Sonographic Abnormal Axillary Lymph Nodes Be Excised in Sentinel Node Biopsy of Breast Cancer?

BACKGROUND: In sentinel node biopsy (SNB) procedure for breast cancer patient, whether the abnormal node was excised is uncertain. The impact of abnormal node on the accuracy of SNB results is unknown. METHODS: In this prospective study, breast cancer patients with sonographically abnormal axillary lymph nodes (SAALNs) and negative FNAC results were eligible. Before the SNB operation, a localization wire was placed at the SAALN by ultrasound guidance. In the SNB operation, the wire-localized nodes (WLNs) and radioactive nodes were excised and labeled separately for pathological examination.

Does Lymph Node Status Prior to Neoadjuvant Chemotherapy Influence the Number of Sentinel Nodes Removed?

BACKGROUND/OBJECTIVE: Recent prospective trials support the use of sentinel lymph node biopsy (SLNB) in breast cancer patients after neoadjuvant chemotherapy (NAC) with a lower false-negative rate if three or more sentinel lymph nodes (SLNs) are identified. In this study, we investigated whether the pre-NAC axillary lymph node status influences the number of SLNs identified. METHODS: Stage I-III breast cancer patients who received NAC and underwent SLNB from May 2014 to April 2016 were identified from an institutional prospective database. Clinical and pathological factors among clinically no…

Prognostic significance of drainage to pelvic nodes at sentinel lymph node mapping in patients with extremity melanoma.

Patients undergoing sentinel lymph node (SLN) mapping for lower extremity melanoma may have drainage to pelvic nodes (DPN) in addition to superficial inguinal nodes. These nodes are not sampled routinely at SLN biopsy. Factors predicting DPN and its prognostic significance were assessed in a large cohort of patients undergoing an SLN biopsy. Three hundred and twenty five patients with single primary melanomas of the lower extremity or buttocks who underwent SLN mapping were identified from our prospective melanoma database (December 1995-October 2008). Associations of clinical and pathologic f…

Can axillary lymph node dissection be omitted in patients with breast cancer and positive sentinel nodes?

Sentinel lymph node biopsy has evolved as the surgical procedure of choice for women with clinically negative axillae, as part of an effort to move toward the less invasive surgical management of breast cancer. Axillary lymph node dissection remains the standard of care for patients with a positive axillary node and was previously performed on all patients with breast cancer prior to the implementation of the sentinel lymph node biopsy. There is, however, controversy regarding whether or not all patients with a positive sentinel lymph node need to undergo completion axillary dissection for eit…

Sentinel lymph node biopsy in breast cancer: an analysis of the maximum number of nodes requiring excision.

Sentinel lymph node biopsy (SNB) is now the standard of care in assessment of patients with clinically staged T1-2, N0 breast cancers. This study investigates whether there is a maximum number of sentinel lymph nodes (SLN) that need to be excised without compromising the false-negative (FN) rate of this procedure. Data were prospectively collected for 319 patients undergoing SNB between February 2001 and December 2006 at our institution. This data were analysed, both in terms of the order of SLN retrieval and relative isotope counts of the SLNs, in order to determine the maximum number of SLNs…

Sentinel lymph node biopsies in melanoma: how many nodes do we really need?

BACKGROUND: Sentinel lymph nodes (SLN) biopsy is a widely used method to detect lymphatic spread in patients with cutaneous melanoma. Several methods are used to detect SLNs. Recent reports attempted to identify the adequate number of SLNs to reliably detect malignant spread in regional lymph nodes. METHODS: The radiotracer counts and the pathologic reports of all patients undergoing SLN biopsies were collected prospectively, to determine which of the nodes harboring radiotracer needed to be removed for examination by histopathology between 1998 and 2005. All patients with positive SLN biopsi…
