Role of the lateral paragigantocellular nucleus in the network of paradoxical (REM) sleep: an electrophysiological and anatomical study in the rat.

The lateral paragigantocellular nucleus (LPGi) is located in the ventrolateral medulla and is known as a sympathoexcitatory area involved in the control of blood pressure. In recent experiments, we showed that the LPGi contains a large number of neurons activated during PS hypersomnia following a selective deprivation. Among these neurons, more than two-thirds are GABAergic and more than one fourth send efferent fibers to the wake-active locus coeruleus nucleus. To get more insight into the role of the LPGi in PS regulation, we combined an electrophysiological and anatomical approach in the ra

Six-hour selective REM sleep deprivation increases the expression of the galanin gene in the hypothalamus of rats.

The effect of short-term selective REM sleep deprivation (RSD) on the gene expression of galanin in the rat hypothalamus was examined using in situ hybridization histochemistry. Monitoring an electroencephalogram (EEG) and electromyogram (EMG) on an on-line computer screen, as the RSD rats entered REM sleep, they were gently stroked on their backs using a brush to wake them during the RSD period. Galanin mRNA levels in the preoptic area (POA) were significantly increased by RSD for a period of 6 h. RSD had no significant effect on the mRNA levels of corticotrophin-releasing factor (CRF), argin

Differential effects of scopolamine on nocturnal cortisol secretion, sleep architecture, and REM latency in normal volunteers: relation to sleep and cortisol abnormalities in depression.

Scopolamine (SCOP) (3.0 mu/kg and 6.0 micrograms/kg) and saline were administered intramuscularly at 11:00 PM to eight normal male volunteers in a randomized design, and the effects on the sleep electroencephalogram (EEG) and nocturnal cortisol secretion (via blood sampling every 15 min) were evaluated. Compared to saline, SCOP produced a significant dose-related delay in rapid eye movement (REM) latency. In contrast, neither dose of SCOP significantly affected nocturnal plasma cortisol concentrations. These results suggest that the central cholinergic system that regulates the onset of REM sl

Accelerometry-derived features of physical activity, sleep and circadian rhythm relate to non-motor symptoms in individuals with isolated REM sleep behavior disorder.

Accelerometry enables passive, continuous, high-frequency monitoring under free-living conditions. For individuals with isolated REM sleep behavior disorder (iRBD), a potential prodromal phase of Parkinson's disease (PD) and dementia with Lewy bodies, accelerometry has been primarily applied to aid diagnosis and to assess phenoconversion risk. To extend this knowledge, we cross-sectionally combined clinical assessments focusing on non-motor symptoms with accelerometry-derived features of physical activity (PA), sleep, and circadian rhythm of N = 68 individuals with iRBD (age: 69.48 ± 6.01 year

Association of Cumulative Smoking Exposure with REM Sleep Alterations in Obstructive Sleep Apnea: A Cross-Sectional Study Supported by Exhaled Carbon Monoxide Measurement.

Objective: The association of smoking with sleep apnea is often based on subjective data. This study quantified the effects of smoking on sleep architecture using exhaled carbon monoxide (eCO) and polysomnography (PSG). Methods: A total of 183 patients with suspected obstructive sleep apnea (OSA) were included in this prospective study. Following full-night PSG, eCO was measured within 10 min. Data were analyzed using the Generalized Linear Model (GLM). Results: Although initial unadjusted analyses showed an inverse correlation between eCO levels and central apnea count, GLM revealed that male

Mice Lacking Cerebellar Cortex and Related Structures Show a Decrease in Slow-Wave Activity With Normal Non-REM Sleep Amount and Sleep Homeostasis.

In addition to the well-known motor control, the cerebellum has recently been implicated in memory, cognition, addiction, and social behavior. Given that the cerebellum contains more neurons than the cerebral cortex and has tight connections to the thalamus and brainstem nuclei, it is possible that the cerebellum also regulates sleep/wakefulness. However, the role of the cerebellum in sleep was unclear, since cerebellar lesion studies inevitably involved massive inflammation in the adjacent brainstem, and sleep changes in lesion studies were not consistent with each other. Here, we examine the

What Does One Sleep-Onset REM Period—During Either Nocturnal Polysomnography or Multiple Sleep Latency Test—Mean in Differential Diagnosis of Central Hypersomnias?

PURPOSE: The differentiation of narcolepsy without cataplexy from idiopathic hypersomnia is based on the number of sleep-onset rapid eye movement periods (SOREMPs) observed by multiple sleep latency test (MSLT) and nocturnal polysomnography. The main aim of this study was to investigate the utility of SOREMP in differential diagnosis of central hypersomnias. METHODS: The authors retrospectively evaluated consecutive 101 patients with a normal polysomnography other than the presence of SOREMP and/or REM without atonia and a latency of ≤8 minutes in MSLT. RESULTS: The authors classified patien

Long-term cognitive trajectories in isolated REM sleep behavior disorder: effects of sex and longstanding disease duration.

STUDY OBJECTIVES: The long-term trajectory of cognitive changes in isolated REM sleep behavior disorder (iRBD) remains unclear. Increasing evidence suggests potential heterogeneity in disease progression. METHODS: People who maintained an iRBD diagnosis without phenoconversion for at least 5 years were included in this study. Cognitive changes over time were assessed using linear mixed models. Subgroup analyses were conducted for men and women separately, and for people with longstanding iRBD. RESULTS: A total of 318 neuropsychological assessments from 162 people with iRBD (28% women) were a

Speech and Language Changes During Rapid Eye Movement (REM) Sleep with Potential Diagnostic Markers: A Systematic Review.

Background: Rapid Eye Movement (REM) sleep behavior disorder (RBD) is a parasomnia resulting from degeneration of pontine and medullary circuits responsible for muscle atonia during REM sleep, leading to dream-enactment behaviors and vocalizations. It is strongly linked to α-synucleinopathies, particularly Parkinson's disease. Current biomarkers such as neurophysiological measures and imaging support diagnosis and monitoring, but remain invasive or costly. Aim: This study aims to evaluate vocal and speech alterations as exploratory, non-validated candidate biomarkers of REM sleep behavior diso

Sex-specific effects of fluoxetine on REM sleep deprivation-induced behavioral impairments and BDNF downregulation in rats.

Rapid eye movement (REM) sleep is a critical phase in which most dreaming occurs, and its deprivation (REM SD) can lead to disturbances in mood, cognition, synaptic plasticity, and neuroplasticity-related genes such as brain-derived neurotrophic factor (BDNF). Fluoxetine, a selective serotonin reuptake inhibitor (SSRI), is known to modulate BDNF expression, and sex-dependent variations in its effects have been suggested. This study investigated the effects of REM SD, fluoxetine, and their combination on behavioral and molecular outcomes in male and female rats. Results: Following 24 hours of R

REM sleep suppression after penetrating traumatic brain injury is associated with alterations in the supramammillary-hippocampal circuit.

Sleep-wake disturbances are among the most prevalent and persistent consequences of traumatic brain injury (TBI), yet the circuit mechanisms underlying post-traumatic alterations in rapid eye movement sleep (REM) and associated cortical oscillations remain incompletely understood. Previous work has shown that REM-associated theta oscillations are determined by a Supramammillary (SuM) - hippocampal vesicular glutamate transporter 2 (Vglut2) circuit. First, we examined whether severe focal penetrating TBI (pTBI) suppresses REM and REM-associated theta power. Secondly, we examined whether these c

Estradiol and non-REM sleep attenuate physiological and emotional responses to social-evaluative stress in healthy women.

BACKGROUND: This study examined whether individual differences in estradiol and sleep predict autonomic and cognitive responses to social-evaluative stress in healthy young women. METHODS: Forty-two healthy women underwent overnight in-laboratory sleep monitoring, followed by a social-evaluative stress task the next morning, which involved listening to a playback of their own karaoke singing. Pre-task estradiol levels were measured via blood sampling. Autonomic responses were assessed using pupil dilation during the playback, while cognitive responses were indexed by subjective stress ratings

Brain MRI Detection of an Abnormal Peak Width of Skeletonized Mean Diffusivity in REM Sleep Behavior Disorder.

BACKGROUND AND PURPOSE: Peak width of skeletonized mean diffusivity (PSMD) is a novel marker of white matter damage, which may be related to small vessel disease. This study aimed to investigate the presence of white matter damage in patients with isolated rapid eye movement sleep behavior disorder (RBD) using PSMD. METHODS: We enrolled patients with newly diagnosed isolated RBD confirmed by polysomnography and age- and sex-matched healthy controls. Diffusion tensor imaging (DTI) was conducted using a 3-Tesla MRI scanner. We measured the PSMD based on DTI in several steps, including preproces

99mTc-HMPAO SPECT Perfusion Signatures Associated With Clinical Progression in Patients With Isolated REM Sleep Behavior Disorder.

BACKGROUND AND OBJECTIVES: Idiopathic/isolated REM sleep behavior disorder (iRBD) is associated with dementia with Lewy bodies and Parkinson disease. Despite evidence of abnormal cerebral perfusion in iRBD, there is currently no pattern that can predict whether an individual will develop dementia with Lewy bodies or Parkinson disease. The objective was to identify a perfusion signature associated with conversion to dementia with Lewy bodies in iRBD. METHODS: Patients with iRBD underwent video-polysomnography, neurologic and neuropsychological assessments, and baseline 99mTc-HMPAO SPECT to ass

Decreased heart rate variability in sympathetic dominant states in Parkinson's disease and isolated REM sleep behavior disorder.

INTRODUCTION: Parkinson's disease (PD) presents with decreased heart rate variability (HRV) from its early stages. However, most of its evidence originates from HRV measurements in parasympathetic dominant states. In this study, we aimed to examine whether HRV in sympathetic dominant states during the head-up tilt table test (HUT) serves as a marker of autonomic dysfunction in PD and isolated REM sleep behavior disorder (iRBD). METHODS: We retrospectively assessed 102 patients with PD, 10 patients with iRBD, and 43 healthy controls. We then measured the coefficient of variation of RR interval

Peripheral α-synuclein isoforms are potential biomarkers for diagnosis and prognosis of isolated REM sleep behavior disorder.

INTRODUCTION: Isolated REM sleep behavior disorder (IRBD) represents an early manifestation of the synucleinopathies Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Aggregation of abnormal α-synuclein and its increased expression in the brain is crucial in the development of the synucleinopathies. Whereas α-synuclein gene (SNCA) transcripts are overexpressed in brain, a concomitant reduction occurs in blood of DLB patients. We assessed whether this decrease is also detectable in IRBD. METHODS: 108 IRBD patients and 149 controls were included of which 29 IRBD and 32 control cases

Neuroanatomical findings in isolated REM sleep behavior disorder and early Parkinson's disease: a Voxel-based morphometry study.

Isolated rapid eye movement (REM) sleep behavior disorder (iRBD) is a parasomnia characterized by loss of physiological atonia of skeletal muscles with abnormal behaviors arising during REM sleep. RBD is often the early manifestation of neurodegenerative diseases, particularly alpha-synucleinopathies, such as Parkinson's disease (PD). Both structural and functional neuroimaging studies suggest that iRBD might share, or even precede, some of the features commonly found in PD, although without a definitive conclusion. Aim of the study is to evaluate the presence of structural abnormalities invol
