This study examined sleep patterns in female adolescents with chronic musculoskeletal pain. Twenty-six participants with chronic musculoskeletal pain completed questionnaires during their clinic visit, and three 24-Hour Sleep Patterns Interviews during the following 2 weeks. Compared to normative data (Acebo & Carskadon, 2002), adolescents with chronic pain reported similar total sleep time (TST) and bedtimes. However, study participants reported significantly longer sleep onset latency, more night wakings, a later morning wake time, and more symptoms of daytime sleepiness. Pain improved after

BACKGROUND: Sleep disturbances in depressive disorders are closely associated with circadian rhythm disruption of the BMAL1-mediated clockophagy. Semen Ziziphi Spinosae (SZS) has demonstrated clinical efficacy in relieving depression-related sleep disorders. However, the specific mechanism by which SZS acts through the BMAL1-dependent regulation of clockophagy circadian rhythmicity remains undefined.
PURPOSE: To investigate the mechanistic role of SZS in ameliorating depression-associated sleep disturbances via modulation of the BMAL1-mediated circadian rhythm of the clockophagy axis.

Sleep-related changes in pharyngeal function result in an increased resistance to airflow and in some people complete pharyngeal occlusion. Clinically, pharyngeal occlusion causes obstructive sleep apnoea syndrome (OSA). This is a prevalent disorder, which is an independent risk factor for the development of systemic hypertension. Several mechanisms contribute to the sleep-related changes in pharyngeal function in both health and disease, including a reduction in respiratory-related muscle activation, and an increase in latency of the pharyngeal reflex to negative intralumenal pressure. In an extraordinary convergence, Emmanuel Mignot and Masashi Yanagisawa employed two entirely different approaches to establish the pathogenesis of narcolepsy, a devastating sleep disorder associated with overwhelming sleep attacks, cataplexy, episodes with sudden loss of muscle tone, and hallucinations resulting from dreaming while awake. Yanagisawa purified brain ligands for orphan G protein-coupled receptors identifying a peptide, orexin, and then used reverse genetics to show that a knockout of the neuropeptide caused narcolepsy in mice. Mignot used forward genetics to identify genetic def

OBJECTIVE: A strong bidirectional relationship exists between epilepsy and sleep, with seizures often occurring more frequently in sleep and, in turn, sleep being disrupted by seizures. However, the mechanistic basis of seizure-sleep interactions is poorly understood. In this study, we characterized the relationship between seizures and sleep in a mouse model of the genetic disorder tuberous sclerosis complex (TSC) and investigated potential mechanisms underlying seizure-sleep interactions related to hypothalamic orexin.
METHODS: Tsc1GFAPCKO mice were used to investigate the relationship betw

A case of periodic hypersomnia in an 11-year-old female with the unique features of mental deficiency, incontinentia pigmenti, acanthosis nigricans and hereditary multiple exostosis (diaphysial aclasis) is reported. The clinical, polysomnographic and Multiple Sleep Latency test features of this case with a follow up of seven years are consistent with a diagnosis of periodic (intermittent) excessive somnolence. The unique presentation, however, does differ from Kleine-Levin syndrome and suggests a relationship between the predominantly ectodermal, congenital disorders and the sleep-wake pattern

We report on a patient with mental retardation and chronic hypercapnic respiratory failure who was found to have severe central apnea and periodic breathing while undergoing an evaluation of low oxygen saturation during wakefulness at rest. Magnetic resonance imaging of the brain, which was performed to uncover potential causes for the central sleep apnea, revealed a "molar tooth sign" consistent with the diagnosis of Joubert syndrome. Joubert syndrome-related disorders are autosomal-recessive disorders characterized by diffuse hypotonia, developmental delay, abnormal respiratory patterns, and

Short sleep duration and circadian misalignment are hypothesized to causally contribute to health problems including obesity, diabetes, metabolic syndrome, heart disease, mood disorders, cognitive impairment, and accidents. Here, we investigated the influence of morning circadian misalignment induced by an imposed short nighttime sleep schedule on impaired insulin sensitivity, a precursor to diabetes. Imposed short sleep duration resulted in morning wakefulness occurring during the biological night (i.e., circadian misalignment)-a time when endogenous melatonin levels were still high indicatin

Background/Objectives: In this study, we aimed to validate and psychometrically evaluate a tool for examining sleep disorders in Polish children. Methods: This study involved a randomly selected sample of 348 children aged 6 to 15 years, sourced from preschools, primary schools, and secondary schools in a city with a population exceeding 100,000, in addition to two smaller towns in Poland. Parents were asked to complete the Sleep Disorders Scale for Children (SDSC) in conjunction with a sociodemographic survey. The tool's reliability was assessed using Cronbach's alpha (α), and correlations am

In a 43-year-old man suffering from hypersomnia with sleep drunkenness, a polygraphic sleep study with direct measurement of blood pressure was carried out. The main findings were lack of blood pressure activation with arousals during the sleep period and persistence of sleep levels after morning awakening. This indicates that cardio-vascular responses to the needs of active behaviour are insufficient. Taking into account feedback mechanisms of blood pressure on alertness, this could be a cause for sleep drunkenness and daytime sleepiness. The blood pressure sustaining drug etilefrine seems to

Insomnia is the most prevalent sleep-wake disorder, with about one in ten individuals meeting diagnostic criteria. Associated with significant impairment in daytime function, insomnia has a vast societal burden and impact on public health. To arrive at an accurate diagnosis of insomnia disorder, clinicians should conduct an effective interview with patients to assess the complaint and sleep history and use assessment tools as part of the evaluation.

INTRODUCTION: Fatal familial insomnia (FFI) is a hereditary disease caused by prions and characterized by mutations in the gene of the prion protein. Its clinical, neuropathological and biomolecular properties make this a distinctive and worrying disorder.
PATIENTS AND METHODS: We studied the clinical and laboratory findings of 14 patients belonging to three Italian and two French families. The genetic analysis done on all patients showed a mutation of codon 178 and polymorphism of codon 129 in both mutant and non-mutant alleles.
RESULTS: The signs and symptoms at the start of the disease we

Neurological autoimmune diseases are characterized by an inappropriate immune response that by mistake targets the nervous system. As a result, patients experiment a number of neurological manifestations that may include insomnia, excessive daytime sleepiness, cataplexy, central hypoventilation, and REM sleep behavior disorder. Polysomnographic evaluation may reveal disorganized sleep architecture involving both NREM and REM sleep, and REM sleep intrusions into wakefulness. The study of sleep disorders in the setting of autoimmune diseases (e.g., narcolepsy, anti-IgLON5 disease, paraneoplastic

We describe a 70-year-old man who, after a viral encephalitis associated with pneumonia, progressively developed a parkinsonism associated with lethargy. Encephalitis manifested with persistent hiccups, seizures and impairment of consciousness. After 2 weeks, the initial neurologic symptoms subsided and the patient progressively developed movement disorders (rigidity and bradykinesia, resistant to L-DOPA), lethargy and behavioral hypersomnia. Magnetic resonance imaging showed thalamic and hippocampal signal abnormalities, immunohistochemistry on a mouse brain substrate revealed serum autoantib

PURPOSE: While accurate sleep measurement is vital for older adults, the validity of actigraphy in free-living environments remains controversial, particularly given the flexible sleep-wake schedules common in this demographic. To address this uncertainty, we assessed the accuracy of wrist actigraphy against in-home portable electroencephalography (EEG) among community-dwelling older adults.
METHODS: Relatively healthy community-dwelling older adults underwent concurrent sleep monitoring using a portable EEG device and a wrist-worn actigraphy for five consecutive nights whenever possible, wit

OBJECTIVES: For optimal health and well-being the sleep episode and the circadian timing system should be properly aligned. We evaluated the effectiveness of different dynamic light and sleep/wake shift schedules for rapid circadian entrainment following an 8-hour advance of sleep.
METHODS: Forty-three healthy participants completed an 8-day inpatient protocol in which the 8-hour sleep episode was advanced by 8 hours. Participants were assigned to one of five conditions: (1) dim ambient WHITE light and GRADUAL shift in which the sleep episode was incrementally advanced over 5days; (2) dim GRE

Evening chronotypes exhibit increased rates of affective dyregulation and sleep disturbances (e.g., insomnia and nightmares). Such symptoms are common to military veterans with posttraumatic stress disorder (PTSD); however, the influence of chronotype on this population remains unknown. We examined behavioral, psychological, and neural correlates of chronotype in 36 combat-exposed military veterans with varying degrees of posttraumatic stress symptomatology. We employed FDG-PET to assess neural activity across wakefulness and rapid eye movement (REM) sleep. We used polysomnography and diaries

Narcolepsy with cataplexy is a sleep dysregulation disorder with alterations of REM sleep, i.e., sleep onset REM periods and REM sleep instability. Deficient orexin-A (hypocretin-1) signaling is assumed to be a major cause of narcolepsy with cataplexy. In this study we investigated fourteen subjects with narcolepsy with cataplexy in a within-subject, random-order crossover, placebo-controlled design. Patients received double-blinded intranasal orexin-A (435 nmol) or sterile water (placebo) in the morning. Administration was preceded by an adaptation night and followed by a modified maintenance

STUDY OBJECTIVES: The purpose of the present study was to preliminarily evaluate whether knowing the dim light melatonin onset (DLMO) time is advantageous when treating delayed sleep-wake phase disorder with low-dose melatonin treatment plus behavioral interventions (ie, evening dim light and time in bed scheduling).
METHODS: In this randomized, controlled, double-blind trial, 40 adults with delayed sleep-wake phase disorder were randomly assigned to 4 weeks of 0.5 mg timed to be administered either 3 hours before the DLMO (measured DLMO group, n = 20) or 5 hours before sleep-onset time per a
