Specific phobia predicts psychopathology in young women.

BACKGROUND: Although specific phobia is characterized by an early age at onset and by high rates of comorbidity, few studies have examined comorbid relationships prospectively. A Clinical Staging Model for Obsessive-Compulsive Disorder: Is It Ready for Prime Time?

Recent changes to the diagnostic classification of obsessive-compulsive disorder (OCD), including its removal from the anxiety/neurotic, stress-related and somatoform disorders chapters of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) and International Classification of Diseases 11th Revision (ICD-11), are based on growing evidence of unique pathogenic signatures and linked diagnostic and treatment approaches. In this review, we build on these recent developments and propose a 'clinical staging model' of OCD that integrates the severity of symptoms and phase …

The differential diagnosis of functional symptoms in adolescence.

Functional complaints constitute the major reason why adolescents visit the physician's office. These complaints may co-exist with organic illness of minor or major significance. Distinguishing between the organic and functional component is the physician's first task. But, just as each organic condition requires a different medical workup and treatment plan, each functional disorder also requires a variety of approaches. The physician therefore must be aware of the need for a further differential diagnosis among functional symptoms in adolescence. This article proposes a definition of functio

Somatic symptom disorders without known physical causes: one disease with many names?

Patients complaining of pain or fatigue in the absence of known physical diseases constitute a high percentage of those seeking general medical care. Depending upon the type of physician/specialist consulted, those individuals may receive disease labels that range from an implied psychological origin such as somatoform or psychosomatic disease, or to a presumed physical disease such as fibromyalgia. Although all these conditions are regularly associated with fatigue, we have provided a new label suggesting another disease category, "systemic exertion intolerance disease," which replaces the pr

Induced delusional disorder. a review of the concept and an unusual case of folie à famille.

Induced delusional disorder (or shared paranoid disorder), also known as folie à deux, is a fairly uncommon disturbance characterized by the presence of similar psychotic symptoms in two or more individuals. Most often the symptoms are delusional. Usually the 'primary' case, i.e. the individual who first develops psychotic symptoms, can be distinguished from one or more 'secondary' cases, in whom the symptoms are induced. We discuss the concept of shared paranoid disorder and consider various aetiological, clinical and diagnostic issues related to the disturbance. We also describe a case of fo

Hysteria, depression and the nosological problem of chronic pain.

In this article the nosological relevance of hysteria and depression for somatically unexplained chronic pain is critically discussed. Despite many clinical controversies and methodological problems, it is concluded from the available literature that both concepts merit a place in the psychiatric nosology of chronic pain. Nevertheless, more research is needed to further clarify the complex relationships between hysteria, depression and pain, and to more exactly determine the relative importance of hysterical and depressive features in chronic pain patients.

The relationship of histrionic personality disorder to antisocial personality and somatization disorders.

The authors examined the association of antisocial personality disorder, somatization disorder, and histrionic personality disorder, both within individuals and within families, in 250 patients. All three disorders overlapped considerably within individuals; the strongest relationship was between antisocial personality and histrionic personality. A high prevalence of antisocial personality was reported in the families of patients with somatization disorder but not in the families of patients with histrionic personality. The authors suggest that histrionic individuals develop antisocial persona

[Chronic fatigue syndrome. Definition, diagnostic measures and therapeutic possibilities].

This article reviews the chronic fatigue syndrome (CFS), a disorder whose etiology is unknown. The diagnostic criteria proposed in 1994 by the CDC and the International Chronic Fatigue Syndrome Study Group are introduced. In contrast to widespread belief, there are no laboratory tests available to underpin the diagnosis of CFS; the diagnosis is made solely on the basis of clinical criteria. In the differential diagnosis, the exclusion of other conditions that can cause chronic fatigue, such as neuropsychiatric or sleep disorders, is of critical importance. In this context, the question as to w
