Internal jugular vein catheterization and catheter insertion slipped into the cervical spinal canal: a special case report.

This paper reports a case of accidental entry of an internal jugular vein catheter into the cervical spinal canal. A male patient in his fifties sustained injuries in a road traffic accident. During the placement of a catheter into the right internal jugular vein, the catheter inadvertently entered the cervical spinal canal, though no damage was caused to the spinal cord or nerves. A second successful puncture and catheter placement was performed under ultrasound guidance, and the patient eventually made a full recovery and was discharged. This case is extremely rare and of particular clinical…

Influence of Posterior Decompression for Lumbar Spinal Canal Stenosis on Intervertebral Disc Degeneration on Magnetic Resonance Imaging.

Introduction Posterior decompression surgery for lumbar spinal canal stenosis (LSCS) is minimally invasive but may cause intervertebral instability and disc degeneration due to the removal of posterior support structures. This study aimed to evaluate the impact of decompression surgery on intervertebral disc degeneration using magnetic resonance imaging (MRI)-based signal ratio (SR) and disc ratio (DR) parameters. Methods Patients who underwent MRI three months to two years after decompression surgery for LSCS were included. Exclusion criteria included decompression at three or more levels, in…

Spinal Canal Remodeling and Indirect Decompression of Contralateral Foraminal Stenosis After Endoscopic Posterolateral Transforaminal Lumbar Interbody Fusion.

OBJECTIVE: There is a lack of literature on indirect decompression in uniportal endoscopic posterolateral transforaminal lumbar interbody fusion (EPTLIF). Our aim is to evaluate the dimensions of the spinal canal and contralateral foramen before and after EPTLIF. METHODS: This is a retrospective study of patients who underwent EPTLIF in a tertiary spine centre over a 2-year period. The cross-sectional area of the spinal canal and the contralateral foramen at the level of fusion were measured on magnetic resonance imaging scan at 1-day postoperation and at the final follow-up. Patients were gr…

Machine Learning Predicts Decompression Levels for Lumbar Spinal Stenosis Using Canal Radiomic Features from Computed Tomography Myelography.

BACKGROUND: The accurate preoperative identification of decompression levels is crucial for the success of surgery in patients with multi-level lumbar spinal stenosis (LSS). The objective of this study was to develop machine learning (ML) classifiers that can predict decompression levels using computed tomography myelography (CTM) data from LSS patients. METHODS: A total of 1095 lumbar levels from 219 patients were included in this study. The bony spinal canal in CTM images was manually delineated, and radiomic features were extracted. The extracted data were randomly divided into training an…

Sprengel Deformity with Omovertebral Bone Encroaching the Spinal Canal Causing Progressive Cervical Myelopathy: A Technical Case Report.

An adult with Sprengel deformity and Klippel-Feil syndrome associated with an omovertebral bone has rarely been reported in literature. The omovertebral bone is an abnormal cartilaginous connection between the scapula and the cervical spine. Limited cases have previously been reported in the literature describing surgical intervention when neurologic deficits such as cervical myelopathy or radiculopathy are present. In the present case, an omovertebral bone extended into the cervical lamina resulting in cervical myeloradiculopathy requiring resection of the bony anomaly and cervicothoracic fus

Cerebrospinal fluid leak presented with the C1-C2 sign caused by spinal canal stenosis: a case report.

BACKGROUND: Intracranial hypotension is a disorder characterized by low cerebrospinal fluid (CSF) pressure typically caused by loss of CSF. Although some mechanisms account for the CSF leakage have been elucidated, spinal canal stenosis has never been reported as a pathological cause of intracranial hypotension. C1-C2 sign is a characteristic imaging feature, which indicates CSF collection between the spinous processes of C1 and C2, occasionally observed on magnetic resonance imaging (MRI) in patients with intracranial hypotension. CASE PRESENTATION: A 58-year-old man was presented to our ins…

False localizing sign caused by schwannoma in cervical spinal canal at C1-2 level: A case report.

RATIONALE: False localizing sign means that the lesion, which is the cause of the symptom, is remote or distant from the anatomical site predicted by neurological examination. This concept contradicts the classical clinicoanatomical correlation paradigm underlying neurological examinations. PATIENT CONCERNS: A 54-year-old man consulted for the right sciatica-like leg pain that had aggravated 1 year ago. Radiological examinations revealed degenerative spondylolisthesis with instability and right-sided recess stenosis at the L4-5 level. After initial improvement following 3 transforaminal epidu

Firearm bullet settling into the lumbar spinal canal without causing neurological deficit: A report of two cases.

BACKGROUND: Uncertainty still exists regarding the treatment of the patients presenting with gunshot wounds to the spine. Neurological insul … gical studies showed two bullets, one was in the psoas muscle on the left side and the other was in spinal canal that had caused a burst fracture of the L5 vertebra. Following L5 laminectomy and bilateral L5-S1 facetectomy, the bullet was removed from the spinal canal and L5-S1 transpedicular posterior stabilization was performed. The postoperative period of b …

Decompression surgery for lumbar spinal canal stenosis in octogenarians; a single center experience of 121 consecutive patients.

INTRODUCTION: With increasing age, ubiquitous degeneration processes lead to a narrowing of the spinal canal. Degenerative lumbar spinal stenosis (DLSS) causes functional disability and is associated with back and lower extremity pain in older individuals. The goal of this study was to determine if decompression surgery can be performed safely and provide a stable benefit in our octogenarian patient population. METHODS: In this retrospective study, 121 patients over the age of 80 (mean age: 82.7 years SD: 2.4 years) with the diagnosis of central spinal canal stenosis who underwent posterior d…

Interlaminar epidural steroid injection for degenerative lumbar spinal canal stenosis: does the intervertebral level of performance matter?

BACKGROUND: Interlaminar epidural steroid injections (ILESIs) are commonly employed in the management of patients with symptomatic degenerative lumbar spinal canal stenosis despite little experimental evidence to guide technique optimization. One untested performance parameter is the intervertebral level at which the ILESI should be performed for maximum patient relief. METHODS: This study randomized patients with symptomatic degenerative lumbar spinal canal stenosis to receive an ILESI at the level of maximal spinal canal stenosis or at a normal/less stenotic intervertebral site 2 interverte…

Reoperation rates after fenestration for lumbar spinal canal stenosis: a 20-year period survival function method analysis.

PURPOSE: Fenestration is the gold standard surgery for lumbar spinal canal stenosis in Japan. Several previous studies have analyzed the reoperation rates in large numbers of patients undergoing several surgical procedures such as laminectomy with or without instrumented spinal fusion; however, there have been few studies focusing solely on fenestration. The purpose of this study was to calculate the reoperation rates after fenestration using the survival function method. METHODS: Form 1988-2007, 6,998 surgeries for lumbar spinal canal stenosis occurred in Miyagi prefecture, Japan, and these …

Clinical significance of postoperative changes in redundant nerve roots after decompressive laminectomy for lumbar spinal canal stenosis.

OBJECTIVE: The postoperative time course of redundant nerve roots (RNRs) in patients with lumbar spinal canal stenosis (LSCS) is currently unknown. The purpose of this study is to investigate the relationship between postoperative morphologic changes in detected RNRs and the clinical outcome of patients with LSCS. METHODS: A total of 33 symptomatic patients with LSCS who demonstrated RNRs on magnetic resonance imaging (MRI) were treated with decompressive laminectomy alone. On the basis of the MRI scans obtained 7 days after surgery, patients were stratified into two groups: group 1 included …

Factors influencing spinal canal stenosis in patients with long-term controlled hydrocephalus treated with cerebrospinal fluid shunt.

PURPOSE: Spinal canal stenosis (CS) occurs in patients with hydrocephalus who are treated with cerebrospinal fluid (CSF) shunting. The pathophysiology of CS comprises CSF overdrainage. We analyzed the incidence of CS and the factors causing it. METHODS: Thirty-three patients who underwent ventriculoperitoneal shunt during childhood visited the Outpatient Department in Yamaguchi University Hospital in 2006. Diameters of spinal canal at C(4) were measured. Treatment procedure, age, and type of hydrocephalus in the patients with CS were compared with those without CS. RESULTS: Of the 33 patient…
