Carotid Sources of Stroke.

The ancient Greeks used the term karotides for the great arteries in the neck, compression of which could plunge the victim into karoun: deep sleep or stupor. The artery name aside, studies of the patterns of brain infarction and their clinical severity have been an unbroken chain of growing sophistication in the past 150 years. Two main patterns emerged: perfusion failure with high-convexity infarction from hemodynamically important ipsilateral carotid stenosis or embolism from nonstenosing carotid plaque.

Emphysematous hepatitis: a very rare entity with a poor prognosis.

A 67-year-old female, with no alterations in glucose metabolism or other relevant history, presented to the Emergency Department due to abdominal pain and vomiting after a syncopal episode. Physical examination revealed a poor general condition, mild stupor and a distended abdomen, painful on palpation in the right hypochondrium, with localized rigidity. On arrival at the Emergency Department, the patient's blood pressure (BP) was 150/66 mmHg, heart rate (HR) was 110 beats/minute and temperature 35 °C.

Cyclosporin-induced hypertriglyceridemia with prompt response to plasma exchange therapy.

A case of severe cyclosporin-induced hypertriglyceridemia that prompted plasma exchange therapy is reported. Hyperlipemic retinitis, headache, stupor, and peripheral paresthesias became apparent when the level of triglycerides exceeded the 1,500 mg/dL level. Two plasma exchanges were required to halt disease progression. This is the second time that plasma exchange was used in a bone marrow transplanted patient on cyclosporin-A therapy. In both cases hypertriglyceridemia became apparent and symptomatic after approximately 7 months of therapy with cyclosporin.

[Ageusia as the aftereffect of oxyfedrine use].

Out of oxyfedrine++ side effects known up to the present (mild agitation, stupor, heat sensation, pains in the epigastrium++, skin allergy) 24 cases of ageusia appearing usually after 4 weeks of treatment with oxyfedrine++ were presented. Disturbances of taste are found to be unpleasant for patients, and result in remarkable exacerbation of their general feeling. The complaints subside completely after finishing treatment, and the time of taste disorder withdrawal is prolonged according to the time of oxyfedrine++ treatment.

Beneficial effects of dantrolene in the treatment of neuroleptic malignant syndrome: a report of two cases.

Two schizophrenic patients developed muscular rigidity, stupor, and hyperpyrexia consistent with neuroleptic malignant syndrome, 8 to 10 days after starting haloperidol therapy. Muscle rigidity was not affected by etybenzatropine or diazepam, but dantrolene, a direct-acting skeletal muscle relaxant, provided muscle relaxation with a concomitant decrease of fever and serum creatine kinase. Neuroleptic malignant syndrome and malignant hyperthermia are clinically similar, and dantrolene is effective in both; suggesting a muscular origin of fever in these two diseases.

ACTIONS OF PROSTAGLANDINS E1, E2 AND E3 ON THE CENTRAL NERVOUS SYSTEM.

Prostaglandins E(1), E(2) and E(3), injected into the cerebral ventricles of unanaesthetized cats, produced sedation, stupor and signs of catatonia. The threshold dose was 3 mug/kg. Slight sedation was also observed following an intravenous injection, but a dose of 20 mug/kg was required. In chicks, intravenous injections of prostaglandins (10 to 400 mug/kg) caused respiratory depression, profound sedation, loss of normal posture and, with the higher doses, loss of the righting reflex.

Unspecified catatonia presenting to neurology and psychiatry with no detectable organic aetiology.

A previously healthy professional athlete in his 20s, with no prior psychiatric or medical history, presented with a 4-day history of rapidly progressive neurological deterioration. This manifested as persistent stupor, mutism and profound psychomotor retardation, enduring several weeks with minimal improvement despite escalating doses of lorazepam (up to 14 mg/day). Extensive investigations at a tertiary neurological centre in the UK revealed no identifiable organic cause. The patient's symptoms ameliorated promptly with the initiation of electroconvulsive therapy in conjunction with lorazepa…

Atypical posterior reversible encephalopathy syndrome (PRES): is peritoneal dialysis a risk factor?

A patient with diabetes mellitus who had recently been started on peritoneal dialysis presented with headaches, confusion, falling episodes, and multiple white matter abnormalities affecting the frontal, temporal, parietal, and occipital lobes on MRI of the brain. He developed stupor and frequent seizures despite control of hypertension with intravenous nicardipine. Diagnosis of atypical PRES and institution of successful empiric therapy was based on electronic searches (Google and MEDLINE) facilitated by verbal communication between and among the specialists involved in his care.

[Organic catatonia after apoplexy].

Catatonia is seen in various psychiatric disorders, but also rarely occurs in medical conditions with organic cerebral affection. We present a case of a previously mentally healthy male, who developed catatonia years after a stroke in the right hemisphere. Catatonic symptoms included stupor, mutism, stereotyped movements and repetition of meaningless sounds. The condition responded to benzodiazepine and electroconvulsive therapy. Catatonia should be considered as a differential diagnosis when the described symptoms occur in patients with a known organic cerebral disorder.

Recurrent brief depression in Prader-Willi syndrome: a case report.

Prader-Willi syndrome is caused by a deletion of 15q11-13 or maternal disomy of chromosome 15. A female patient with Prader-Willi syndrome, who was 19 years of age at the first onset of her recurrent brief episodes, is described. The episodes showed a near-monthly rhythm and were followed by a spontaneous remission in 7-18 days. The symptoms during the episodes were anorexia, insomnia, guilt feelings, ideas of being doomed, ostracized and persecuted, and stupor alternating with agitation.

[Clinical aspects and diagnosis of klofelin poisoning].

Examination of 15 patients with clofelin poisoning cleared up clinical symptoms of the condition. The impairment was chiefly inflicted to cardiovascular system and neuropsychic status. There appeared arterial hypotonia, long-term orthostatic hypotonia combined with bradycardia and extrasystolic arrhythmia. Early hours of poisoning can be accompanied with high pressure (hypertensive phase). ECG records sinus bradycardia and early ventricular repolarization. Neuropsychic alterations involve drowsiness, stupor++, psychic inhibition, visual and auditory hallucinations, photopsia. The condition was…

Thoracic empyema: a five-year experience at Queens Hospital Center.

A retrospective analysis of 48 cases of thoracic empyema is presented along with a management protocol, which includes empiric antibiotic therapy or combinations thereof and an aggressive primary surgical procedure that results in minimal morbidity and mortality. In this series, there were eight deaths (16.6%). The contributing etiologic agents, however, have shifted from traditional causes, such as posttonsillectomy, postdental extractions, and bronchiectasis, to stupor caused by excessive alcoholism and drug addiction, also involving direct access through injectables into the cardiovascular …

[Severe hypernatremia within the scope of catatonic schizophrenia].

We report the case of a 57-year-old chronic schizophrenic patient who developed an acute catatonic stupor after postoperative withdrawal of neuroleptics with excessive and firstly therapy-resistant hypernatremia. Only after re-treatment with neuroleptics the hypernatremia could be compensated. A relationship between the catatonic schizophrenia and the electrolyte-disturbances can be supposed; probably in both diencephalic structures are involved. Therefore the syndrome of "acute life threatening catatonia" should be included into the differential diagnosis of uncertain metabolic disorders.

Three cases of unipolar delusional depression responsive to L-dopa.

We reported three cases of unipolar delusional depression which took a characteristic two-stage clinical course. Anxiety and agitation with persecutory delusions predominated the picture at first, but they were soon replaced by severe psychomotor retardation or stupor associated with delusions of poverty and guilt. The administration of L-Dopa in the latter stage brought about a rapid improvement both in mood and psychomotor activity. We considered these cases in the light of the recent biochemical studies on affective psychosis.

Tuberculous meningitis at Wellington Hospital 1962-88.

A review was undertaken of all cases of tuberculous meningitis seen in Wellington since 1962. It showed, in conjunction with an earlier study, a steady decline in the incidence of the disease since 1940. There was a relatively high incidence in Maori and Pacific Island Polynesians. Good outcome was seen in 17/28 (61%). Poor recovery was associated with delayed treatment, confusion or stupor at presentation, and male sex. The diagnosis is not excluded by a normal peripheral white count, ESR, chest x-ray or cerebrospinal fluid.

Natural and experimental bovine immunodeficiency virus infection in cattle.

Since 1989, the LSU dairy herd, with its high seroprevalence of BIV, was recognized to have a high incidence of common diseases that reduced the economic viability of the dairy. The herd had a high percentage of cows with encephalitis associated with depression and stupor, alteration of the immune system associated with secondary bacterial infections, and chronic inflammatory lesions of the feet and legs. The occurrence of disease problems was associated with the stresses of parturition and early lactation and/or with unusual environmental stress cofactors.

Central pontine myelinolysis following 'slow' correction of hyponatremia.

Two patients with central pontine myelinolysis are described for the peculiar mode of development. Both patients were in chronic renal failure and admitted in a stuporous state due to hyponatremia. Both developed central pontine myelinolysis during the hospital stay following slow and judicious correction of hyponatremia. The role of chronicity of hyponatremia prior to its correction, in the genesis of central pontine myelinolysis, particularly in the patients who have chronic debilitating illness, septicemia or malnutrition, is highlighted.

A case of neuroleptic malignant syndrome in a mentally retarded adolescent.

The neuroleptic malignant syndrome is an uncommon but occasionally fatal reaction to antipsychotic medication characterized by fever, movement disorder, and stupor. A case is reported of a 16-year-old retarded male who progressed into coma after an injection of fluphenazine. The syndrome is discussed, and new pharmacological treatment approaches are reviewed. Brain-damaged patients are thought to be more susceptible to this syndrome. As recognition of the syndrome grows, there may well be more cases identified in the mentally retarded population.

Caecal leiomyoma detected by whole-body MRI in a patient with Hodgkin lymphoma: first case report.

INTRODUCTION: Neurological complications are, at the present time, considered among the most important causes of morbidity and mortality after heart surgery. We evaluated their importance and risk factors. PATIENTS AND METHODS: We retrospectively reviewed 2, 528 consecutive patients who underwent cardiopulmonary bypass in a single center. In each one, we attended to previous vascular risk factors, such as surgical and postoperative events. We considered four categories of neurologic outcome: (1) persistent neurological focal deficits, (2) stupor or coma, (3) temporary neurological focal defic…
