The role of altered impedance in the pathophysiology of normal pressure hydrocephalus, Alzheimer's disease and syringomyelia.

Normal pressure hydrocephalus, Alzheimer's disease and syringomyelia appear to be completely unrelated diseases, however, they share a reduction in subarachnoid space compliance as part of their pathophysiology. This paper discusses the physiology of pulsatile fluid flow and its relationship to compliance/impedance. Unlike continuous or non-pulsatile flow where the vessel resistance and pressure gradient are the major determinants of the volume of fluid flowing, when the fluid flow in a vessel pulsates then the vessel compliance/impedance becomes important. A reduction in compliance in the cra…

Cerebral haemorrhage in Moyamoya disease at autopsy.

Nineteen fatal cases of occlusion of the circle of Willis, so-called Moyamoya disease, were examined clinicopathologically. Fresh and massiv … and old haemorrhagic lesions in 3 of the 14 patients. Saccular aneurysm of cerebral arteries in the subarachnoid spaces was present in two of the 19. No rupture involved the perforating arteries in the subarachnoid space. These findings strongly suggest that in patients with Moyamoya disease rupture of overgrown perforating arteries as collaterals in brain may be ma …

Spinal Subdural Hematoma following Epidural Anesthesia.

The spinal subdural space is an avascular, potential space and is a rare location for intraspinal hematomas. Compared to spinal epidural hematomas, spinal subdural hematomas are uncommonly described complications of lumbar puncture for spinal or epidural anesthesia, particularly in patients who have no pre-existing bleeding disorders or history of antiplatelet or anticoagulant intake. We describe a 19-year-old girl who had a large thoracolumbar spinal subdural hematoma following epidural anesthesia for elective cholecystectomy with no pre-existing bleeding diathesis that caused rapidly develop

[Sacral perineural cysts. Contribution of magnetic resonance imaging].

In a 41-year old woman complaining of episodic bilateral sciatic pain, MRI showed large sacral cysts developed in the pelvis. The fact that these cysts communicated with the subarachnoidal spaces was not clearly demonstrated by CT. The mechanism underlying the development of this perineural variety of extradural cysts is discussed.

Relation of cerebral vasospasm to subarachnoid hemorrhage visualized by computerized tomographic scanning.

In 47 cases of verified ruptured saccular aneurysm, we investigated the relationship of the amount and distribution of subarachnoid blood de … should be regarded as preliminary. The results, if confirmed, indicate that blood localized in the subarachnoid space in sufficient amount at specific sites is the only important etiological factor in vasospasm. It should be possible to identify patients in jeopardy from vasospasm and institute early preventive measures. (Neurosurgery, 6: 1--9, 1980)

An autopsy case of meningeal carcinomatosis with parenchymal invasion through the cranial and spinal nerve roots.

Meningeal carcinomatosis is a well-known complication of malignant neoplasms. We report a case of meningeal carcinomatosis of 2 months' duration in a 22-year-old man, in whom the initial symptom was gradually worsening headache. Postmortem examination revealed infiltrating adenocarcinoma of the stomach. Carcinoma cells showed diffuse spread to the subarachnoid space of the brain and spinal cord. In many places, subarachnoid tumor cells had infiltrated to the cranial and spinal nerves. Moreover, carcinoma cells in the nerve roots extended to the parenchyma of the brain and spinal cord beyond th…

Spinal anesthetic for emergency cesarean section in a parturient with uncorrected tetralogy of Fallot, presenting with abruptio placentae and gestational hypertension.

A subarachnoid block is an effective way of providing anesthesia for cesarean sections. However, it can be considered relatively contra-indicated in parturients with uncorrected tetralogy of Fallot (TOF). We report a case of a 22-year-old female patient with TOF and gestational hypertension, who presented for an emergency cesarean section for placental abruption. The surgery was successfully conducted under a spinal anesthetic with a combination of low dose bupivacaine and fentanyl. Fentanyl combined with small-dose bupivacaine in the subarachnoid space can be considered as an alternative tech…

MRI manifestations of leptomeningeal metastasis.

Twenty-six cases with leptomeningeal metastasis were analyzed and Gd-DTPA enhanced MR imaging was found to be the most useful tool for making the diagnosis. The important manifestations in order of frequency were: 1) leptomeningeal enhancement; 2) ependymal enhancement; 3) plaque, nodule or mass in the subarachnoid space or ventricles; 4) nerve root thickening; and 5) ventricular dilatation not related to the obstruction of the primary neoplasm. A review of the literature has shown that intracranial leptomeningeal metastasis from spinal cord astrocytoma are very rare. In our series, 3 cases wi…

Familial oculoleptomeningeal amyloidosis.

Members of a German family living in Ohio (United States) have been found to have dominantly inherited amyloidosis causing dementia, seizures, strokes, coma and visual deterioration. Pathological examination of affected individuals has shown severe, diffuse amyloidosis of the leptomeninges and subarachnoid vessels associated with patchy fibrosis and obliteration of the subarachnoid space. Amyloid deposits were prominent on the ependymal surfaces and severe and diffuse neuronal loss and generalized subpial gliosis were found in the cerebrum and cerebellum. On occasion superficial brain infarcts…

Induction of communicating hydrocephalus in mice by intrathecal injection of human recombinant transforming growth factor-beta 1.

Transforming growth factor-beta 1 (TGF-beta 1) is a multi-functional polypeptide, which controls proliferation, differentiation of various cells, and regulates synthesis of extracellular matrix proteins. We injected human recombinant TGF-beta 1 into the subarachnoid space of 10-day-old C57BL/6 mice in order to study the role of TGF-beta 1, which is known to be released from platelets into the cerebrospinal fluid following subarachnoid hemorrhage. The ventricular system became dilated within 3 weeks following the injection and the body weights of injected mice stopped increasing 6 weeks after i…

PRECIPITIN RESPONSE IN THE BLOOD OF RABBITS FOLLOWING SUBARACHNOID INJECTIONS OF HORSE SERUM.

1. Rabbits which have received a single dose of normal horse serum in the subarachnoid space produce precipitins in the blood in greater abundance, of higher titer, and persisting longer than those in control rabbits which have received a similar injection intravenously. 2. Repeated subarachnoid injections of normal horse serum in rabbits induce precipitins in the blood early. These may appear in high titer as soon as 1 week after the initial injection, whereas in rabbits similarly treated intravenously no precipitins are found at this time. They may appear a few days afterward and reach a hig…

Maximizing the Conspicuity of CSF-Venous Fistulas on CT Myelography: Assessment of Contrast Density and Timing Effects.

BACKGROUND AND PURPOSE: Advancements in CT myelography (CTM) have improved visualization of CSF-venous fistulas (CVFs), a frequent cause of spontaneous intracranial hypotension (SIH). However, the relative impact of the timing of image acquisition and the contrast density in the subarachnoid space remain unclear. This study compared the effects of timing and contrast density in the ipsilateral subarachnoid space and assessed the impact of other technical factors on CVF conspicuity using a validated instrument to stratify diagnostic confidence. MATERIALS AND METHODS: A retrospective review of …

Intracranial pressure in the American Alligator (Alligator mississippiensis): reptilian meninges and orthostatic gradients.

The cranial meninges of reptiles differ from the more widely studied mammalian pattern in that the intraventricular and subarachnoid spaces are, at least partially, isolated. This study was undertaken to investigate the bulk flow of cerebrospinal fluid, and the resulting changes in intracranial pressure, in a common reptilian species. Intracranial pressure was measured using ocular ultrasonography and by surgically implanting pressure cannulae into the cranial subarachnoid space. The system was then challenged by: rotating the animal to create orthostatic gradients, perturbation of the vascula…
