Advancements in Managing Bone Defects with Limb Length Discrepancies with a Motorized Intramedullary Combined Transport and Lengthening Nail.

Large bone defects that coexist with limb length discrepancy present a challenging problem in limb reconstruction. Historically, management has required complex circular fixation constructs, staged procedures, or separate devices to independently address each problem. However, the development of motorized intramedullary nails capable of performing both bone transport and limb lengthening with a single implant represent a paradigm shift. This review traces the evolution of internal bone transport technology from the initial foundational principles of distraction osteogenesis through first gener

Segmental Sagittal Alignment in Lumbar Spinal Fusion: A Review of Evidence-Based Evaluation of Preoperative Measurement, Surgical Planning, Intraoperative Execution, and Postoperative Evaluation.

BACKGROUND AND OBJECTIVES: Maintaining and restoring global and regional sagittal alignment is a well-established priority that improves patient outcomes in patients with adult spinal deformity. However, the benefit of restoring segmental (level-by-level) alignment in lumbar fusion for degenerative conditions is not widely agreed on. The purpose of this review was to summarize intraoperative techniques to achieve segmental fixation and the impact of segmental lordosis on patient-reported and surgical outcomes. METHODS: In accordance with the Preferred Reporting Items for Systematic Reviews an

The role of stiffness-matching in avoiding stress shielding-induced bone loss and stress concentration-induced skeletal reconstruction device failure.

It is well documented that overly stiff skeletal replacement and fixation devices may fail and require revision surgery. Recent attempts to better support healing and sustain healed bone have looked at stiffness-matching of these devices to the desired role of limiting the stress on fractured or engrafted bone to compressive loads and, after the reconstructed bone has healed, to ensure that reconstructive medical devices (implants) interrupt the normal loading pattern as little as possible. The mechanical performance of these devices can be optimized by adjusting their location, integration/fa

Effect of tendon-to-bone gap on adjustable- versus fixed-loop biceps tendon fixation.

BACKGROUND: Distal biceps tendon ruptures significantly impair forearm flexion and supination strength, particularly in active individuals. Suspensory fixation using cortical button devices is a widely accepted surgical approach. Adjustable-loop techniques allow maximal tendon-to-bone apposition, while fixed-loop methods are simpler but may leave a gap. Biomechanical differences between these techniques specific to the distal biceps tendon remain poorly investigated, especially given the unique loading patterns of the elbow compared to other joints. METHODS: Finite element analysis was perfor

Surgical treatment of combined PCL-ACL medial and lateral side injuries (global laxity): surgical technique and 2- to 18-year results.

The multiple ligament injured knee is a severe injury that may also involve neurovascular injuries, fractures, and other systemic injuries. Surgical treatment offers good functional results documented in the literature by physical examination, arthrometer testing, stress radiography, and knee ligament rating scales. Mechanical tensioning devices are helpful with cruciate ligament tensioning. Some low grade medial collateral ligament (MCL) complex injuries may be amenable to brace treatment, while high grade medial side injuries require combined surgical repair-reconstruction. Lateral posterola

Novel technique for relieving anastomotic tension using halo-vest immobilization after tracheal sleeve resection.

We describe a novel technique of using halo-vest-enforced immobilization to relieve anastomotic tension after tracheal sleeve resection. Immediately after the tracheal sleeve resection, four halo titanium pins were inserted in the skulls of the patients to secure the halo-vest. All patients fitted with halo-vests were able to eat and drink and their clinical course was good. Bronchoscopy confirmed the absence of anastomotic leaks and stenoses, and there were no complications associated with the halo-vest. We believe that ensuring neck flexion using a halo-vest after tracheal sleeve resection i

First report of recurrent bilateral temporomandibular joint dislocation treated with personalized porous bioceramic medical devices.

Various surgical techniques have been proposed to prevent recurrent temporomandibular joint (TMJ) dislocations, with inconsistent results regarding recurrence and preservation of mandibular mobility. We report the innovative use of a custom-made porous bioceramic medical device for the treatment of recurrent bilateral TMJ dislocations. The implants were computer-designed from the patient's preoperative CT scan and manufactured by the Cerhum company. Implantation and fixation were performed through preauricular approaches. At one-year follow-up, no recurrence or surgical complications were obse

The CAM-P-OS study protocol: a prospective randomized multicenter trial evaluating an active controlled motion device in the rehabilitation of surgically treated, isolated ankle fractures of Weber types B and C.

BACKGROUND: Displaced ankle fractures classified as Weber types B and C are common across all age groups. The standard treatment involves open reduction and internal fixation, followed by a rehabilitation program that includes 6 weeks of partial weightbearing. During this period, both passive and active range-of-motion exercises are performed under the supervision of a physiotherapist. Recently, a new device has become available, allowing active controlled motion of the ankle joint at home on top of the normal rehabilitation protocol. The purpose of this controlled randomized trial was to eval

External fixation of Colles' fractures. An anatomical study.

In a prospective, controlled study 58 patients aged under 60 years with Colles' fractures were treated either by a forearm plaster or by the application of an external fixator. In 94% of those treated by a fixator it was possible to insert the distal pins of the frame into the fracture fragment, the fixation obtained being sufficient to forgo additional splintage. The external fixator proved more effective at holding the manipulated position, and the radiological loss of position during fracture union was minimal compared with that seen in patients treated in plaster.

Orthognathic positioning system: intraoperative system to transfer virtual surgical plan to operating field during orthognathic surgery.

PURPOSE: To introduce the concept and use of an occlusal-based "orthognathic positioning system" (OPS) to be used during orthognathic surgery. MATERIALS AND METHODS: The OPS consists of intraoperative occlusal-based devices that transfer virtual surgical planning to the operating field for repositioning of the osteotomized dentoskeletal segments. The system uses detachable guides connected to an occlusal splint. An initial drilling guide is used to establish stable references or landmarks. These are drilled on the bone that will not be repositioned adjacent to the osteotomy line. After mobili

Charcot Spinal Arthropathy Secondary to Spinal Cord Injury - A Case Report.

INTRODUCTION: Pressure ulcers are common complications in immobilized patients, typically occurring over bony prominences. However, in rare instances, particularly in high-risk individuals with hematologic disorders such as sickle cell disease, ulcers may develop at unusual anatomical sites due to external medical devices. This case highlights a rare presentation of a vulvar pressure ulcer caused by prolonged application of a Thomas splint in a patient with homozygous sickle cell disease (HbSS). CASE REPORT: A 24-year-old woman with HbSS was admitted with a closed distal femoral shaft fractur

Transcatheter Aortic Valve Implantation With Balloon-Expandable Valve Prostheses in Patients With Pure Native Non- or Mildly Calcified Aortic Regurgitation: A Case-Series and Literature Review.

OBJECTIVE: Transcatheter aortic valve implantation (TAVI) is an off-label procedure for selected patients at high surgical risk with native non- or mildly calcified aortic regurgitation (AR). Traditionally, self-expanding transcatheter heart valves (THV) have been favored over balloon-expandable THV's probably due to assumed better device fixation. We report a series of patients with native severe AR successfully treated with a balloon-expandable THV. METHODS: Between 2019 and 2022, 8 consecutive patients (5 male, 82 (interquartile range 80-85) years old, STS PROM 4.0 % (interquartile range 2

The use of single port surgery for polyps located in the rectum.

Transanal endoscopic microsurgery is a minimally invasive technique for the treatment of rectal lesions which was introduced by Buess. In this report the first clinical experience of transanal endoscopic surgery was performed by a single incision laparoscopic surgical port adapted through the anal canal. In single port surgery, the single incision laparoscopic surgical port has to be stitched around anal orifice. There is no need to use a fixation apparatus. In transanal endoscopic microsurgery procedure, a rigid rectoscope 40 mm in diameter is introduced into the anus by stretching anal sphin

Designing patient-specific solutions using biomodelling and 3D-printing for revision lumbar spine surgery.

PURPOSE: Despite the variety of "off-the-shelf" implants and instrumentation, outcomes following revision lumbosacral surgery are inconstant. Revision fusion surgery presents a unique set of patient-specific challenges that may not be adequately addressed using universal kits. This study aims to describe how patient-specific factors, surgeon requirements, and healthcare efficiencies were integrated to design and manufacture anatomically matched surgical tools and implants to complement a minimally invasive posterior approach for revision lumbar fusion surgery. METHODS: A 72-year-old woman pre

Adhesional small bowel obstruction related to stapling device from previous laparoscopic inguinal hernia repair.

Small bowel obstruction (SBO) is common surgical presenting problem, accounting for roughly 15 000 laparotomies per year in the UK. However, SBO post laparoscopic transabdominal preperitoneal (TAPP) inguinal hernia repair is uncommon with an estimated incidence of 0.2%-0.5%. The common causes for SBO post-TAPP include inadequate closure, port-site herniation and adhesions. Here, we present a case of adhesional SBO related to stapling device from previous laparoscopic inguinal hernia repair and review alternative methods for mesh fixation. This case reports a rare but life-threatening complicat
