Dysphagia Lusoria in Childhood: An Uncommon Cause of Swallowing Difficulty.

Dysphagia lusoria is a rare cause of esophageal compression, typically resulting from an aberrant right subclavian artery (ARSA). ARSA is the most frequently occurring anomaly of the aortic arch. Although usually asymptomatic, most patients complain of dysphagia or respiratory symptoms. This case is of a nine-year-old boy with asthma and allergic rhinitis who presented with chronic epigastric pain, vomiting, and progressive dysphagia to solids. His growth parameters were normal but on the lower end of the body mass index (BMI) range. Esophagogastroduodenoscopy showed a pulsatile external compr

Development and Psychometric Evaluation of the Swallowing Satisfaction Scale (SSS).

The purpose of the current study was to develop a new scale to assess a level of satisfaction associated directly with swallowing ability. First, by interviewing 10 adults with dysphagia, 10 experts, as well as reviewing literature, the initial version of the swallowing satisfaction scale (SSS) was developed. The content validity of the SSS was evaluated using two qualitative and quantitative methods and expert opinions. The qualitative method was also used to determine face validity through interviews with 10 adults with dysphagia. Finally, the reliability of the scale was assessed by determi

Continuous Estimation of Swallowing Motion With EMG and MMG Signals.

Oropharyngeal dysphagia (OD) is a symptom of swallowing dysfunction that is associated with aspiration, severe respiratory complications, and even death. OD is a highly prevalent condition in populations including the elderly and patients with neurological diseases (e.g., stroke and Parkinson's disease (PD)). Assessment of swallow function is crucial for managing OD, yet depends on devices for long-term monitoring during daily life and relevant methods for accurately assessing swallow function. The videofluoroscopic swallowing study (VFSS) is usually considered a gold standard method. However,

Endoscopic and videofluoroscopic evaluations of swallowing for dysphagia: A systematic review.

OBJECTIVE: This systematic review was performed to compare the diagnostic accuracy of FEES and VFSS in evaluating swallowing in adults with dysphagia. METHODS: A search for articles published between January 1991 and March 2020 was carried out in the MEDLINE EMBASE, COCHRANE, SciELO, and LILACS electronic databases. Based on the eligibility criteria, six articles were included. RESULTS: FEES demonstrated a higher ability to diagnose pharyngeal residue, penetration, and aspiration compared with VFSS, and slightly better performance in detecting premature spillage. CONCLUSION: There were no s

Device-based solutions supporting patients with swallowing problems.

INTRODUCTION: Swallowing is a highly complex activity requiring the coordination of numerous muscles within the head and neck through connections from multiple areas within the central nervous system. Dysphagia is common and has long been known to lead to significant adverse effects on morbidity, mortality, and quality of life. Classical approaches toward dysphagia management involve input by clinical deglutologists who teach rehabilitative exercises and recommend alteration of the consistency of food and fluids. However, the evidence base in support of some of these approaches is inconsistent

Unsupervised Segmentation of Bolus and Residue in Videofluoroscopy Swallowing Studies.

Bolus tracking is a critical component of swallowing analysis, as the speed, course, and integrity of bolus movement from the mouth to the stomach, along with the presence of residue, serve as key indicators of potential abnormalities. Existing machine learning approaches for videofluoroscopic swallowing study (VFSS) analysis heavily rely on annotated data and often struggle to detect residue, which is visually subtle and underrepresented. This study proposes an unsupervised architecture to segment both bolus and residue, marking the first successful machine learning-based residue segmentation

Automatic detection of swallowing events based on surface electromyography.

Clinical evaluation of dysphagia requires modeling different phases of swallowing. This process is expensive, invasive, subjective, and typically based on Videofluoroscopic Swallowing Study (VFSS) studies. This work evaluates the use of surface electromyography (sEMG) signals for detecting bolus passage through the mandible line (ManL) and upper esophageal sphincter (UES) in patients with oropharyngeal dysphagia. The paper introduces a novel method where GRU networks are fed with spectrograms resulting from processing sEMG signals. Using VFSS as the gold standard, our results yielded F1 scores

The Pathophysiology of Sarcopenic Dysphagia: Consideration of Videofluoroscopic Swallowing Studies.

Background The pathophysiology of sarcopenic dysphagia remains insufficiently understood. The aim of this study was to clarify the pathophysiology of sarcopenic dysphagia by analyzing data from videofluoroscopic swallowing studies, focusing on the pharyngeal constriction ratio (PCR) as an objective indicator of pharyngeal contraction strength and on the anterior movement distance of the hyoid (AMDH), which contributes to the opening of the upper esophageal sphincter. Methods The study had a cross-sectional design and included 52 patients aged 65 years or older who were deemed to require a vide

Swallowable Gastric Balloon: As a Noninvasive Option for Weight Loss.

Background: Obesity is a multifactorial disease that has had an increase in its prevalence. Its treatment requires a multidisciplinary team to prepare the patient for bariatric surgery. But not all patients are willing to undergo a surgical procedure. Other options to surgery were developed, such as the swallowable gastric balloon, a novel device that allows weight loss (WL) due to a restrictive function without anesthesia, endoscopy, or surgery and is completely ambulatory. Methods: A descriptive, retrospective, and observational study was presented. A total of 497 obese patients of both sexe

The swallowing reflex can be induced by peripheral stimulation of the larynx. Although previous studies have suggested that potassium ions e … onic rats, without differences in licking behavior between DW, NaCl, and KCl, the intervals between swallows were the smallest during KCl-associated licking. Inward rectifier potassium channel (Kir)3.1- and Kir6.2-positive cells were detected in the nodose ganglion and vocal folds. The rate of expression of these molecules in immunoreactive cells was relat …

Swallowing Impairments in Patients with Myasthenia Gravis: A Scoping Review.

This scoping review aims to describe the oropharyngeal and esophageal swallowing profiles of individuals with Myasthenia Gravis (MG). Given the high prevalence of dysphagia in this population, this review seeks to synthesize existing literature on swallowing impairments and highlight the need for early identification in dysphagia management. This scoping review was conducted in accordance with Joanna Briggs Institute methodology for scoping reviews. Four databases were systematically searched to identify studies that examined swallowing impairments in MG using instrumental assessments. Studies

Cough Correlates of Functional Swallow Outcomes in Atypical Parkinsonism.

BACKGROUND: Swallow and cough impairments lead to aspiration and reduced clearance of aspirate material. Both behaviors are impaired in Parkinson's disease, but it is unknown whether a similar relationship of dysfunction exists in forms of atypical Parkinsonism (APD). Elucidating this association in APD may lead to early, comprehensive airway protection treatment. OBJECTIVES: We tested the hypotheses that swallow deficits in APD are associated with impaired cough and that airway protective dysfunction is associated with longer disease duration. METHODS: Swallowing difficulty was described by

The Impact of Cervical Spinal Disease on Pharyngeal Swallowing Function.

PURPOSE: Spinal pathology is very common with advancing age and can cause dysphagia; however, it is unclear how frequently these pathologies affect swallowing function. This study evaluates how cervical spinal pathology may impact swallowing function in dysphagic individuals observed during videofluoroscopic swallowing studies (VFSSs). METHOD: A retrospective case-control study was performed on 100 individuals with dysphagia as well as age-/gender-matched healthy controls (HCs) with available VFSS. Spinal anatomy of patients was classified into two predetermined categories, and a consensus de
