OBJECTIVE: To characterize the epidemiology of invasive pulmonary aspergillosis (IPA).
DESIGN: A retrospective case series.
SETTING: An 850-bed, academic, tertiary-care medical center.
PARTICIPANTS: Adult inpatients, between January 1, 1990, and December 31, 1998, with either a histopathology report consistent with IPA or a discharge diagnosis of aspergillosis.

We assessed whether a 1-year teaching programme in northern Zanzibar would improve prescribing practice. Data on polypharmacy and appropriateness of the treatment of upper-respiratory infection, anaemia, and scabies from the 17 primary health-care units in northern Zanzibar were analysed before and after the teaching programme. There was a significant and sustained reduction in polypharmacy and an improvement in the treatment of upper-respiratory infection, scabies, and anaemia. This teaching programme has been successful in improving prescribing practices in a less-developed country.

AIMS: To assess prospectively the activity and effectiveness of a diabetes specialist on-call service.
METHODS: All requests for specialist advice received by the doctor on-call for diabetes in a large teaching hospital were recorded and analysed over a period of 3 weeks.
RESULTS: The total number of calls was 135 (mean 45 per week) of which 48.1% were outside normal working hours. Four adults with mild mental retardation were taught to sort based on what two pictures had in common and to name each object and common features of two pairs as well as to comply with a series of instructions to sort the pictures selectively. The criterion task was to inspect a sample pair, sort through a deck of 15 pictures to find those showing what the sample pair had in common, and to repeat a series of pairs. Infective arthritis (InfectA) has a variable geographical pattern. A 'Medline' search did not yield any earlier series on epidemiological pattern of InfectA from Kuwait or the Gulf region. Therefore, an observational prospective study of sequential cases seen over a period of 4 years was carried out. Of a total of 2021 patients seen during this period, 36 (1.8%) were diagnosed to have InfectA. There were 6 (16.7%) cases of neisserial and 30 (83.3%) of non-neisserial InfectA. We describe a low cost, easy to construct monitoring and teaching aid for the oesophageal and precordial stethoscopes. It is constructed from two readily available 'state of the art' integrated circuits. The aid allows more than one observer simultaneously to hear heart and breath sounds, without being acoustically isolated from the other monitoring. A comparison between patients of abdominal trauma admitted 10 years ago (1979-1981) and those admitted in the recent past (1989-1991) showed important variations in the frequency, aetiology, severity, morbidity and mortality.
